fix(lazy): keep the image's affine rank on the lazy crop/pad path - #9188

Conversation
lazy.utils.resample reduced the affine to the spatial rank and, on the crop/pad/flip shortcut, assigned that reduced affine to the output. A 2D image with the default 4x4 affine therefore came back with a 3x3 affine and lost its out-of-plane spacing and origin (eager mode keeps them), e.g. a lazily cropped 2D slice saved to NIfTI is written with the wrong geometry. Compose the transform at the image's own affine rank instead. The interpolating path is unchanged. Assisted-by: Claude Opus 5.5 <noreply@anthropic.com> Signed-off-by: tritsystem <gbranaa4@gmail.com>

File:
monai/transforms/lazy/utils.py, inresample(). MONAI images carry an affine matrix recording real-world spacing and position. A 2D image still gets a 4x4 affine by default (it reserves a row/column for a z-axis even though the image has no depth).MONAI's lazy mode defers crop/pad/flip transforms for efficiency, and has a fast-path shortcut for them. That shortcut shrinks the affine down to match the image's spatial rank, producing a 3x3 matrix for a 2D image -- but it should keep the 4x4 shape, like the eager transforms and the other (interpolating) lazy path both do. So the same operation gives a different-shaped affine depending on which code path you take.
Real impact: a 2D image lazily cropped and saved to NIfTI with
SaveImageis written with z spacing 1.0 and origin 0.0 (placeholder/default values) instead of the correct 2.0 and 1.0 that the eager path writes. This can corrupt the saved file's spatial metadata, not just a cosmetic difference. It also breaks concatenating a lazy-processed tensor with an eager-processed one, since their affines are different shapes.Fix: build the affine at the image's own full rank instead of shrinking it first.
