Stream Deck: polish follow-ups from #1410 (dedicated action icons; optional pre-populated SD+ profile)

Two small follow-ups deferred from #1410 (PR #1439), both flagged in that lane's plan/review:

1. Dedicated icons for the new manifest actions — send-queue and the Row 2 terminal key reuse existing icons; give them their own (Maker Console asset pass).
2. Pre-populated SD+ profile (optional) — Codev.streamDeckProfile ships Actions:null (deliberate; Stream Deck: merge Fleet Slot into Builder Action (phase-aware press) #1404 precedent, re-ruled by the owner at Stream Deck SD+: two-zone builder workflow (selectors + action palette, dial-driven feedback queue) #1410's dev-approval). If someone can author + hardware-verify an importable profile carrying the two-zone layout, it saves adopters the manual key placement. Do NOT ship an unverified binary profile — a failed import at first contact is worse than the valid blank.

Low priority; natural home is the next Elgato pack build session.

Third item for this bundle — dead icon assets (verified 2026-08-13, zero references repo-wide). #1410's ApproveGate repurpose and the earlier gate-nav removal left six PNGs shipping in every pack with nothing pointing at them (checked against apps/streamdeck/src, manifest.json, and the plugin's docs):

• icons/approve-gate-empty.png + @2x
• icons/approve-gate-pending.png + @2x
• icons/gate-nav.png + @2x

(The still-live icons/approve-gate.* and icons/list/approve-gate.* stay — the repurposed selected-scoped Approve key uses them.)

That makes this issue one coherent asset pass: two icons in (dedicated send-queue and open-terminal, replacing the generic action glyph they share with the catch-all Codev Action — the friction shows in the Stream Deck app's action picker, which is exactly where adopters start given the blank profile), six files out, and the optional pre-populated profile if it can be hardware-verified in the same session.

Note on scope framing: the icon half is a setup-experience fix, not an on-hardware one — since #1428/#1410 the plugin paints composite key faces at runtime, so the static States[].Image is only visible pre-render or when the plugin is not running.

Scoping for the lane (architect, pre-spawn). Protocol AIR — this is mechanical, not a design exercise, because the two icons should be rendered from the glyph vectors the plugin already ships, not drawn fresh:

• send-queue → the existing comment glyph; open-terminal → the existing terminal glyph (both in GLYPHS, apps/streamdeck/src/face.ts:70+). Rendering the picker icon from the same vector the key face uses makes the action list agree with the hardware by construction — no style drift, no judgement call.
• Required outputs per icon, matching the existing convention: icons/<name>.png (72×72), icons/<name>@2x.png (144×144), icons/list/<name>.png (20×20) and icons/list/<name>@2x.png (40×40), then point the manifest's Icon and States[].Image at them instead of the shared action asset.
• Dead assets out (verified unreferenced): approve-gate-empty, approve-gate-pending, gate-nav — each with its @2x. Keep approve-gate.* and list/approve-gate.* (still live).
• Out of scope: the pre-populated SD+ profile (stays deferred — it needs hardware-verified import before it can ship, and that belongs to a session with a deck attached), and any change to key-face rendering or press behaviour.

Decision to surface rather than assume: how the PNGs get rasterized. Prefer a one-off render using tooling already available in the repo over adding a dependency for a one-time asset build; if a generation script is worth committing for reproducibility, say so and justify the dep. Report the approach in the PR body.

Colour note: the glyph functions take a colour argument — pick what reads correctly against the Stream Deck app's list background (the existing list/* assets are the reference).

Shipping record: PR #1443 merged 2026-08-13T07:28:05Z (7/7 green). Dedicated send-queue and open-terminal icons rendered from the plugin's own GLYPHS vectors via a committed script (system rsvg-convert + ImageMagick, no new npm dependency), fitted to the measured fill fractions of the existing assets; manifest repointed; six verified-dead PNGs removed (approve-gate-empty, approve-gate-pending, gate-nav + @2x).

Board was degraded (codex unavailable) → 2-way CMAP + architect verification, which for an image change meant viewing the rendered PNGs, not just reading the diff. That is how the residual was caught: the new Open Terminal icon reads near-identically to the catch-all Codev Action icon, which is itself a terminal glyph — not a regression (those two share one asset today) but tracked as #1444 with the recommendation to re-glyph the catch-all, since the terminal picture belongs to the action that opens a terminal.

Release-notes entry on docs/vscode-changelog. The optional pre-populated SD+ profile remains deferred to a session with a deck attached.
