docs: repoint retired code.claude.com slugs and add claim-specific recheck triggers (#6524)

## Summary

Removes the code.claude.com drift that #6498 listed.

- Every citation of a retired slug outside the excluded items now points
at its live page and anchor, each confirmed by fetch.
- `plugins`, `plugin-dependencies`, `plugin-hints`, `plugin-relevance`,
`plugin-marketplaces`, `plugins/mods` and `slash-commands` map to pages
under `plugins/*` and to `commands#all-commands`.
- About 150 verification stamps gained recheck triggers specific to
their claim. Generated copies were updated through the sync script.
- The excluded items, and the findings that need a decision, are filed
in #6523.

## Fix

- **`pluginConfigs` sources.** The three-source wording (user settings,
`--settings` and managed settings, with project and local ignored)
stays. Fact 5 of the hook-config-delivery convention now cites both
pages as of 2026-10-07:

- **Visualization.** The decision matrix's "no native enum" claim is
replaced by a pointer to `userConfig` `options` (v2.1.271+), with an
as-of date and a recheck trigger.
- **harness-ops inventory.** The field-coverage row now lists the
manifest's `types` and `experimental.evals` fields as not scanned, and
its date is 2026-10-07.
- **disk-hygiene safety model.** The `known_marketplaces.json` record
had a trigger that fired. The record was rewritten against
`plugins/loading` and `marketplace-reference`.
- **Versioning.** Fragments were added for the 13 changed fragment-mode
plugins. Every other changed plugin got a patch bump and a dated
CHANGELOG entry. A plugin with no net change got no bump.

## Verification

- `scripts/sync-shared-copies.sh --check`: 398 match.
- `scripts/sync-plugin-options-docs.py --check`: up to date.
- `check-changelog-parity.sh`: all four modes pass.
- `check-changelog-fragments.sh --check` and `--check-required
origin/main`: pass.
- `validate-plugin-contracts.mjs` and `plugins/github/github.test.sh`:
pass.
- Pinned ruff over the changed Python: all checks passed.
- markdownlint-cli2 over the 141 changed Markdown files: 0 issues.
- A residual grep for retired slugs finds only the excluded items. Each
repointed anchor was confirmed by an `id=` match in fetched HTML.
