fix(agent-context): <csv> is not an allowlisted placeholder, and the grounding gate reads it as a generator name

The previous commit's opt-in table wrote `--generators <csv>`.
`skill-catalog-grounding.test.ts` matches `--generators[= ]+(...)` and treats each
comma-separated token as a generator name to resolve against the manifest, so
`<csv>` read as a generator that does not exist — twice, one per table row.

The gate was right and the prose was the outlier: every other place in these
skills already writes `--generators <names>`, which is in the gate's placeholder
set. So the table now writes it too, rather than `<csv>` being added to the
allowlist — widening a grounding gate to admit a synonym is how it stops
grounding.

Corpus regenerated in the same commit, for the reason the last one was.
