fix: align gateway body with Command Code schema; pass live Laguna

Correct /alpha/generate payload (structure[] + mode=agent), harden live
tests for streamed deltas, and verify Go-plan session end-to-end on
poolside/laguna-s-2.1-free via proxy and opencode run.
