fix(cli): stop percent-encoding Windows output paths in the build manifest

rewriteOutputPath took pathToFileURL(...).pathname of both directories before
path.relative on Windows, so spaces and non-ASCII characters in the project path
reached the container as %20 / %C3%9C and indexing failed with Cannot find module.
Use path.relative with separators normalised to / on every platform, and run the
indexer in exec form in the Containerfile so literal spaces survive the shell.
