Open in v0 failed: v0 did not install the shadcn components the form imported (native-select, field, input), and its preview page uses a default import. The form now uses plain elements styled with the shadcn theme, so it needs no other component, and it has a default export and an optional onSubmit. The README links v0.app, since v0.dev's redirect drops the URL.
Tested: opened in v0 from this branch, where the form renders and checks postcodes; installs and builds in fresh Next.js, Vite, and non-shadcn projects; 38 tests.
