Add NIM smoke/phase-4 scripts, load_env helpers, critic adapter rate limiting, operator_log/SIMULA_VERBOSE pipeline hooks, Track B promotion assessment, and gitignore rules for local .env and regenerable run artifacts. Co-authored-by: Cursor <cursoragent@cursor.com>
