CI: 3rd test account + smoke marker coverage expansion #101

What: Added 3rd Keycloak test account + bumped parallelism (api/provider-smoke -n 2→3, consumer-smoke serial→-n 4); expanded the default -m smoke gate to cover all of consumer/smoke and 8 more files in provider/smoke.
Why it matters: consumer-smoke was running fully serial in CI; the smoke/ folder name implied broader coverage than the smoke marker actually gave.
Evidence: provider/smoke collection under -m smoke went from 4 to 11 of 18 files.
