A fork PR sat open for two weeks with nobody assigned to it. Owning every path means GitHub requests a review from the maintainer the moment an outside contribution arrives, which is the notification that was missing. Code-owner review is deliberately not made *required* in branch protection: as the sole owner I cannot approve my own PR, and requiring it would block every release behind an admin bypass. subscribers_count: 0— nobody was watching it, including the owner. CODEOWNERSis the second net: a review request fires regardless of watch settings.
Code-owner review is intentionally not set to required in branch protection.
As the sole code owner I cannot approve my own PR, so requiring it would put
