docs: drop fail-closed slogan headings - #176

Rewrite human-authored docs voice to match the flow README: keep the gates, drop the memes. lint and certify still refuse; run still will not start without its admission checks. Headings no longer sell fail-closed as a posture.

Live docs.openadapt.ai still titles the regulated-execution page "Fail-closed regulated execution: run vs replay". That's a slogan heading. This change drops those headings.
The gates stay put.
lintstill reports gaps.certifystill refuses an unsafe bundle.runstill won't start without its admission checks. The flow README already writes it that way.mkdocs.ymlnav goes from "Fail-closed regulated execution" to "Regulated execution". Same pass on 38 markdown files underdocs/. Doesn't touch the auto-generated what's-new page.Test plan
