Closes #4421.

What

Implements the three skeletonization-based topology metrics requested in #4421 — topology precision, topology sensitivity, and the centreline Dice score (clDSC) from Shit et al., CVPR 2021 (https://arxiv.org/abs/2003.07311) — for evaluation of connectivity in segmentations of tubular structures (e.g. vasculature).

MONAI already ships the differentiable loss form (SoftclDiceLoss); this PR adds the evaluation metric form with hard skeletons, matching the paper's reference implementation (https://github.com/jocpae/clDice).

Implementation notes

  • New CenterlineDiceMetric (CumulativeIterationMetric) + module-level compute_centerline_dice(), following the SurfaceDiceMetric/AbsoluteVolumeDifferenceMetric pattern.
  • The metric is selected with metric_name: "cldsc" (alias "centerline_dice"), "topology_precision" (alias "tprec"), "topology_sensitivity" (alias "tsens"). All three share the same skeleton computation; a single class keeps the API surface minimal (same rationale as ConfusionMatrixMetric's metric_name).
  • Hard skeletons via skimage.morphology.skeletonize: Zhang-Suen for 2D, method="lee" for 3D (as in the paper's reference code). scikit-image is pulled in with optional_import, consistent with other optional-dependency metrics (EmbeddingCollapseMetric).
  • Empty-mask conventions follow SurfaceDiceMetric: class absent from both prediction and reference → NaN (excluded from not-nan reductions); present in only one → 0 (a missing prediction cannot cover the reference skeleton and vice versa).
  • Supports 2D and 3D (BCHW[D]), include_background, all MetricReduction modes, and get_not_nans.

Files

  • monai/metrics/centerline_dice.py — the metric (+241)
  • tests/metrics/test_centerline_dice.py — unit tests (+154)
  • monai/metrics/__init__.py, docs/source/metrics.rst — registration

Testing

  • pytest tests/metrics/test_centerline_dice.py — 18 passed
  • All expected values are hand-computed on one-pixel-wide structures (whose skeleton is the structure itself) plus a thick-bar case with the Zhang-Suen skeleton shape documented inline; empty-mask and NaN conventions, metric aliases, multi-class/include_background, reduction modes, and invalid-input errors are covered.
  • pytest tests/metrics/ — no regressions (test_compute_fid_metric.py::test_results fails identically on a clean dev tree, unrelated environment issue).
  • black --line-length 120 + isort --profile black clean.

Review focus

  1. Single class + metric_name selector vs three separate classes — happy to split if preferred.
  2. Empty-mask semantics (documented above) chosen for consistency with SurfaceDiceMetric.
  3. 3D skeletonization uses Lee et al. (the only 3D-capable method in scikit-image), which builds its LUT on first call — negligible for evaluation-time use.

…nsitivity

Implements the skeletonization-based topology metrics requested in Project-MONAI#4421:
centreline Dice score (clDSC), topology precision, and topology sensitivity
(Shit et al., CVPR 2021), complementing the existing SoftclDiceLoss with the
hard-skeleton evaluation metric form.

- New CenterlineDiceMetric (CumulativeIterationMetric) and
  compute_centerline_dice(), selectable via metric_name
  (cldsc / topology_precision / topology_sensitivity, with aliases)
- Hard skeletons via skimage.morphology.skeletonize (Zhang-Suen for 2D,
  Lee et al. for 3D), imported with optional_import, matching the paper's
  reference implementation
- Empty-mask conventions follow SurfaceDiceMetric: absent in both -> NaN,
  present in only one -> 0

📝 Walkthrough

Added compute_centerline_dice and CenterlineDiceMetric for binary 2D and 3D segmentation masks. The implementation supports centerline Dice, topology precision, and topology sensitivity, with optional background exclusion and metric aggregation. It uses scikit-image for skeletonization. The package exports the APIs, and the metrics documentation and tests cover them.

The new centerline Dice and topology metrics compute skeleton overlap correctly for 2D and 3D masks. No merge-blocking issues remain.

Explanation

Issue [#4421] requires topology precision, topology sensitivity, and clDSC from skeletonized binary prediction and reference masks. compute_centerline_dice() calculates both overlap ratios and their harmonic mean. CenterlineDiceMetric exposes the three metrics and aliases. The tests cover 2D, 3D, empty masks, and metric aggregation.

Full details: Out of Scope Changes check

Explanation

The changes stay within [#4421]. They add the requested metric function and class, public registration, documentation, and focused automated tests. The 3D skeleton fallback supports the requested metric behavior and is covered by a regression test.

Note for reviewers: the GitHub Actions workflows on this PR (tests, build_docs, CodeQL) are currently in the "awaiting approval" state for this fork, so the test matrix hasn't started yet. For reference, local results on top of the latest dev:

  • pytest tests/metrics/test_centerline_dice.py — 18/18 passed
  • pytest tests/metrics/ — no new failures vs a clean dev tree (the pre-existing test_compute_fid_metric.py::test_results env issue aside)

Happy to run anything else locally if that's easier than approving the workflows.

…mask is empty

skimage's Lee-method skeletonize can return an empty skeleton for a non-empty
3D mask (e.g. small solid blobs such as a 2x2x2 cube), which made identical
masks score 0 instead of 1 for clDSC / topology precision / sensitivity. Use
the mask itself as its own skeleton in that case, document the behavior, add a
regression test, and give _skeletonize a Google-style docstring.

Addressed the review feedback in 4d9b860 (plus the _skeletonize docstring nitpick).

The finding is real — confirmed empirically on scikit-image 0.26.0: Lee-method skeletonize returns an empty skeleton for non-empty 3D masks, even small ones (solid 2×2×2 / 6×6×6 / 8×8×8 boxes, all-foreground 16³ and 128³). Pre-fix, two identical 2×2×2 cubes therefore scored cldsc = 0.0 (and tprec = tsens = 0.0) instead of 1.

Fix applied as suggested, at the _skeletonize level so both call sites are covered: when skeletonization of a non-empty mask comes back empty, the mask itself is used as its own skeleton. This restores the sanity property "identical masks → 1" while leaving the documented semantics untouched — an empty mask still yields t_prec/t_sens contributions of 0, so "class present in only one of prediction/reference → 0" behaves exactly as before (regression-covered by EMPTY_PRED/EMPTY_REF).

Also added:

  • test_solid_blob_fallback — identical solid 2×2×2 cubes score 1 for cldsc/tprec/tsens, disjoint solid cubes score 0;
  • a sentence in compute_centerline_dice's docstring documenting the fallback;
  • a Google-style docstring for _skeletonize.

Local checks on top of the new commit: pytest tests/metrics/test_centerline_dice.py — 19/19 passed; black --line-length 120 and ruff check clean.
