Align organization on-ramp with merged context system

Follow-up review of merged PR #3, which introduced the organization documentation system, project context contract, and public-safe registry.

Material inconsistencies found

• START-HERE.md still described the repository primarily as stewardship policy and workflow templates, omitting the newly merged documentation contracts and registry.
• Its review-source metadata and smallest-useful map did not include DOCUMENTATION_SYSTEM.md, PROJECT_REGISTRY.yml, or PROJECT-CONTEXT.md.
• PROJECT_REGISTRY.yml referenced coherent-world-simulation as a relationship target without defining that nested project ID.

Proposed changes

• align the on-ramp's current reality, map, next route, and review sources with the merged system;
• add a public-safe nested registry entry for the already disclosed Coherent World simulation role;
• leave PROJECT-CONTEXT.md, README, and the public Wiki unchanged because their working model and project projection remain accurate.

Verification

• every registry relationship target resolves to a declared project or nested-project ID;
• the on-ramp includes all newly canonical organization-context sources;
• no private repository content is copied into this public proposal.
