Merge current main 240adf9 into the existing PR branch; this does not merge the pull request into main. Keep the Reflective Accountability index entry and main's MVSA/accountability changes. Retain the PR's Adaptive continuity index entry and reuse its research-note blob 034a9a5 unchanged. No issue closures or model-adoption decisions.
