Separate provider checks from passive CLI data-file refresh #137

Separate provider-facing quota checks from passive reads of usage/quota files written by other CLIs. Expose independent refresh intervals and counts so faster file refresh cannot silently increase provider traffic.

Current implementation has six direct API readers plus the newly connected Gemini reader, and three helper paths. Reading credentials locally does not make a reader file-only. Verify supported passive quota-file formats before adding readers; do not infer subscription quota from token counts, re-date unchanged snapshots, or record duplicate history samples when rereading files. Keep collector server push/pull choices independent.
