Plain chat turns surface "Tool added: track_links/track_tasks" lines in the transcript #1101

Reproduce

SelfA1025 / SelfB1025 → https://social.botawaken.com, tip e109361, retest round 2026-10-25:

1. Friend DM on B → A's Chief: send What is 5+7?
2. A shows approval card; Approve.
3. Look at both transcripts for that turn.

Also seen on the face-room cross-home @ math turn the same day.

Expected

User-visible transcript shows the question and the answer only (plus normal status / approval UI). Internal tool registration is not printed as chat lines.

Actual

• B transcript: Tool added: track_links
• A transcript: Tool added: track_tasks

on a plain math turn that needed no such tools.
