Bumps every workspace to 0.23.0 with scripts/bump_version.mjs and renames the CHANGELOG Unreleased section to 0.23.0, adding a migration note.

Why 0.23.0 rather than 0.22.2: #758 makes every RunFormatting field nullable, which breaks the docx-core public type, and changes the read_file JSON shape (#752). The repo bumps the minor version for breaking changes (0.21.0, 0.22.0).

• refactor: derive replace_text hidden-construct warning from buildParagraphIndex; readZipText drops BOM #1147: buildParagraphIndex fields, readZipText BOM handling

Local preflight on this branch:

• build, lint:workspaces, check:tool-docs, check:mcpb-manifest, the Gemini manifest check, the changelog-data check, coverage plus its threshold check, spec coverage and pack dry-run all passed.
• npm run test had one hook timeout in docx-core generation-package-structure.test.ts, at the point where a separate LibreOffice job was running on this machine. Rerun on its own, the file passes 6 of 6.
