[audit-high] SQLite stored false booleans as the word false, which reads as true - #61
Three columns are meant to start out false: whether a refresh token is revoked, whether a permit needs a person to approve it, and whether an approval was only a simulation. The migrations that created them used the word "false" as the database default. On SQLite, which this gateway uses for tests and local runs, that word is stored as text, and the application treats any non-empty text as true. A later migration fixed old permit rows only. It left the default in place, and it left the other two columns alone.
Why it matters
A new refresh token can look already revoked. A permit can look like it needs human approval and then fail its signature check. A real approval can look simulated and be refused.
How it was fixed
The three columns are now created with a real boolean false: 0 on SQLite and false on PostgreSQL.
Migration 024 now rewrites existing text values on all three columns. Text false becomes false, text true stays true, and any other text is left unchanged.
No new migration was added.
Tests
Added gateway/tests/test_sqlite_boolean_defaults.py. One test upgrades to 023, inserts the text false and the text true, upgrades to 024, and checks the values and that SQLite stored them as integers. The other upgrades to head, inserts rows that omit the three booleans, and checks they read false. A fourth flag that already used a numeric default stays false as a control.
Before the fix, from gateway/:
~/metacode/fleet/heavy ~/metacode/fleet/venvs/gw/bin/pytest tests/test_sqlite_boolean_defaults.py -q --tb=line
2 failed in 4.36s (exit 1). After 024, refresh_tokens.revoked still read true (the permit row had already been repaired). At head, omitted requires_human_approval, revoked, and simulated all read true, while allow_identical_repeats read false. That run did not include the later "text true stays true" rows. Those rows were added after this failure and passed on the fixed code.
After the fix, from gateway/:
~/metacode/fleet/heavy ~/metacode/fleet/venvs/gw/bin/pytest tests/test_sqlite_boolean_defaults.py tests/test_migrations.py::test_024_repairs_sqlite_boolean_backfill -q --tb=short
3 passed in 33.74s. The existing permit-repair test still passes.
Ruff on the same files, using ~/metacode/fleet/venvs/gw/bin/ruff directly: check passed (All checks passed) and format --check passed (4 files already formatted). The same two commands were also submitted through ~/metacode/fleet/heavy. That copy was still sleeping after about 19 minutes, with 523 waiters and all 3 slots busy, so it was stopped rather than left running. The pytest runs above did go through the slot queue.
An earlier pytest from the repo root exited 4 because the test path is under gateway/. That was not a product failure.
Notes for the reviewer
Editing migrations 022, 023, and 024 does not re-run them on a database already stamped past 024. A long-lived SQLite file that already applied those revisions keeps the old text default until that file is recreated. Production PostgreSQL already stores this default as boolean false, checked on a throwaway local database: both DEFAULT 'false' and DEFAULT '0' insert false, and pg_get_expr returns false. A new revision was not added, because it would move the packaged head and force a rollout note for a PostgreSQL no-op. Say if you want a follow-up revision that rebuilds these defaults on databases already past 024.
Migration 038's numeric default was left as it is. It already reads as false on both SQLite and PostgreSQL. Its concurrent index path was not re-run. The rest of gateway/migrations was read. Other items (a nullable SQLite hash anchor, a historical sequence default of 0, a singular branch label Alembic ignores) were left unchanged. The full gateway suite was not run.
Draft PR from the Muse Code QA fleet (instance qa5-am-gateway-migrations), reviewed by Grok Bot. Not merged or deployed; needs C.Lee's yes.
Note
Medium Risk
Touches auth/permit/approval booleans and advances Alembic head with an irreversible SQLite migration; mis-rollout could block older app images or leave incorrect flag semantics until 051 runs.
Overview
Fixes a SQLite bug where boolean columns used the string default "false", which stored as text and read back as true through SQLAlchemy—breaking refresh-token revocation, permit human-approval flags (and signatures), and approval simulation state.
Schema fixes for new databases: migrations 022 and 023 now use sa.false() instead of "false" for revoked, requires_human_approval, and simulated. 024’s SQLite data repair is generalized to rewrite known text values on all three columns, not only permits.
Already-deployed SQLite: new head migration 051 (after 050_demo_key_first_used) repeats the text→integer row repair and replaces lingering DEFAULT 'false' with integer 0 defaults; PostgreSQL is a no-op. SQLite downgrade is blocked via sqlite_boolean_defaults_downgrade_unsupported, documented in schema-051-rollout.md (backup, rehearsal, irreversible rollback boundary).
Tests: new coverage for 024 repair, head defaults, and legacy DB upgrade through 051; some existing migration tests stop at 044 so they do not cross the irreversible 051 path.

Migrations 022 and 023 change affected Boolean server defaults to sa.false(). Migration 024 normalizes recognized text Boolean values in three SQLite columns. New migration 046 normalizes those columns and sets integer-false defaults on SQLite. Tests cover the earlier repair, head-upgrade defaults, and upgrades from legacy SQLite schemas. Rollout documentation describes Schema 046 requirements and rollback behavior.
This change fixes SQLite boolean defaults and repairs existing text values that were read as true. The remaining risk is operational: downgrading from the new revision reports success without reverting the schema. The rollout guidance already documents this, so it is a bounded follow-up rather than a merge blocker.
Security architecture risk: Moderate · up to f9920
The Boolean repair preserves recognized true values while correcting false defaults. However, the new migration requires a predecessor absent from the reviewed revision. Shipping this state can block upgrades and startup, including deployments that do not need the SQLite repair. The documented merge prerequisite must be satisfied before release.
Retained concerns
Medium · reliability · inferred: The new migration depends on an absent revision, leaving the reviewed release’s migration graph unresolved. Until the documented PR #195 prerequisite is integrated, upgrades and stamped-schema startup verification can fail, preventing deployment of the security-state repair. This packaging dependency affects PostgreSQL deployments too, despite the repair itself being SQLite-only.
Security review details
Security Blast Radius
observed — The repair operates across matching rows throughout each migrated SQLite database, not within one wallet or tenant. Its affected state controls token revocation and approval enforcement.
Trust Boundaries and Controls
observed — The migration’s interpolated SQL identifiers come from a fixed table-and-column tuple, not request input. Its updates discriminate by SQLite storage type and recognized value rather than clearing all truthy data.
Resilience and Maintainability Implications
observed — Existing startup controls fail closed on migration failure or mismatched stamped schema rather than silently accepting an incomplete upgrade. This contains schema drift but makes the missing predecessor a deployment-availability concern.

Head 874515d, clean merge (main merged in), required CI green (24 ok / 3 skipped; Postgres validated by CI only), 0 open threads, no force-push (ff from 89a5956), no fixtures removed.
047.down_revision = 046_api_key_usage_events; alembic heads → single head 047.
SQLite: upgrade 046→047 OK; downgrade raises RuntimeError sqlite_boolean_defaults_downgrade_unsupported and DB stays at 047 (explicit, safe refusal). Migration tests 38 passed / 1 skipped.
Also edits historical 022/023/024 (sa.false()); acceptable because 047 repairs existing DBs.
DeepSeek never passed the old head (874515d), so this is a full review of main...6b4c2f2, not just the delta.
The remerge-diff of 6b4c2f2 (merge of main d8ade3e) is empty.
Alembic chain:
alembic heads returns a single head: 047_sqlite_boolean_defaults.
gateway/migrations/versions/047_sqlite_boolean_defaults.py:12 has down_revision = "046_api_key_usage_events".
No revision number is duplicated against main. The only shared prefix is 025 (025_refresh_token_key_binding → 025_remove_plaintext_owner_keys). That one is pre-existing on main and linear, so it isn't introduced here.
Code:
022/023: server_default=sa.false() renders as 0 on SQLite and false on PostgreSQL, which is correct for fresh databases.
024: the repair now covers all three columns. The table and column names are hard-coded literals (no injection surface). Unknown text is left alone, so no control is silently removed.
047:
Repairs already-migrated SQLite databases in both directions, then batch-alters the server defaults.
It's a no-op on PostgreSQL.
SQLite downgrade raises sqlite_boolean_defaults_downgrade_unsupported instead of lying about state. This is documented in gateway/docs/schema-047-rollout.md.
Verified locally: test_sqlite_boolean_defaults, test_sqlite_legacy_boolean_defaults, test_migrations, test_action_migrations and test_api_key_usage_migration ran 41 passed, 1 skipped.
Non-blocking: the docs say #61 must merge after #195 (046). 046 is already on main, so that's satisfied.
Address review 5642272418 on #234:
- migrations/env.py sets app.rls_bypass=on (transaction-local) on
PostgreSQL, as the archived api env.py did, so the 050 backfill sees
rows under FORCE ROW LEVEL SECURITY with a non-superuser owner.
- REQUIRED_TRUST_COLUMNS for mcp_dispatch_attempts now lists the six
047 reconcile columns; legacy create_all boot test covers each.
- 048 docstring matches env.py.

Address codex review of f563483 on #234:
- Settings.TENANT_RLS_MODE ported from agent-middleware-api d7bc941 (same
name, default 'off', off/enforce validator). 048 reads it directly.
- 048 applies amw_tenant_isolation to receipt_resolutions and
resolution_chain_heads when they exist (fresh chain); 053 covers
api-chain DBs. Downgrade drops them too.
- schema-048 rollout note: env.py bypass, fail-closed warning (no
runtime app.current_wallet_id in this gateway yet).
- Opt-in PG test (TENANT_RLS_MIGRATION_DATABASE_URL): non-superuser
owner, enforce forces RLS on all 17 tables, 050 backfills.
Signed-off-by: Christopher <131207452+PetrefiedThunder@users.noreply.github.com>
Keep 'off' as default; 'enable' turns on RLS without FORCE; 'enforce' forces it. Adds tests/test_tenant_rls_postgres.py covering off, enable and enforce (including LATE_RLS_TABLES).

What was wrong
~/metacode/fleet/heavy ~/metacode/fleet/venvs/gw/bin/pytest tests/test_sqlite_boolean_defaults.py tests/test_migrations.py::test_024_repairs_sqlite_boolean_backfill -q --tb=short
