Request fails with reasoning `encrypted_content` was not issued to this caller.

Description

The first request can work. The next request in the same session fails, especially after a tool call, image attach, idle period, or OpenCode restart.

Looks like OpenCode is replaying stored reasoning.encrypted_content / itemId that the current Zen/Console caller no longer accepts.
