Adds "Delete Form" to the Application Forms Actions menu, protected by a "Delete Forms" permission granted to Program Manager by default. Deletion hides forms from the forms list and new parent-form selections from the Payment Config and blocks configuration/edit URLs, while preserving related records, existing parent assignments, and integrations.
