Key envelopes no longer name their recipients. Each manifest carries a
power-of-two number of envelopes (minimum 4, cap 4096); padding envelopes are
genuine HPKE seals to throwaway keys, so storage learns only a rounded device
count. A device finds its envelope by trial decryption of the head manifest.
The policy, signer, and signature move into a sealed header, and policies
travel inside the transition manifest instead of separate objects. An
envelope audit lets every reader re-seal and compare all envelopes.
docs/design.md and docs/spec.md describe the v5 format.
Part of the split of #2; stacked on
incremental-fetch-verification.
