Summary

rename npm package to @dearcloud09/logseq-mcp because the unscoped logseq-mcp name is already taken
update Codex and Claude npx setup examples
add public publishConfig for scoped package publishing

Verification

npm test
npm audit --omit=dev --audit-level=high
npm pack --dry-run
