Eliminates API Diff Worker timeline comments for routine metadata-only refreshes, which create notifications without report changes. The worker now records refreshed automation state and feedback only in the PR body, and incremental updates are body-only so a title-only update cannot exhaust the per-run update limit.
Addresses the observation from @svick at #10482 (comment) and eliminates the PR comment noise seen on that PR.
