Summary
Workspace-wide pass from a full investigation: fixes provider/protocol schema mismatches verified against first-party SDKs (Gemini usage + finish reasons, OpenAI reasoning round-trip and usage mapping, Anthropic error/stop handling), hardens storage/auth/sandbox/tools, simplifies the TUI overlay visuals, and corrects usage attribution — split into atomic commits.
