chore: initialize beads integration

Problem

• Repository-local Beads and Codex integration exists only in an unpushed commit.
• Bare Dolt pushes recreate the deleted informational branch.

Solution

• Add Beads tracking, hooks, and repository-specific Codex configuration.
• Use the canonical fs issue prefix and tracked audit log.
• Require DOLT_REMOTE_INFO_BRANCH='' for all Beads Dolt pushes.
• Symlink CLAUDE.md to the canonical AGENTS.md instructions.
