[High impact] Refactors permission request lifecycle and settlement durability. Fix single-request cancellation during an in-flight approval before merging. FindingsSummaryThe PR saves exact request settlements in the existing encrypted CLI journal and replays them through the versioned Agent-state writer.

…n ends Pending permission/user-action counts are written with best-effort agent state, while turn status is durable. A dropped clear left the counts set forever, so the UI kept showing "Action required" after the turn ended. Zero the counts and pendingRequestObservedAt in the same transaction that materializes a terminal latest turn, and carry the zeroed counts in the turn update broadcast so live clients clear immediately. … exits Keep pending-request settlement at the durable turn writer, and reuse the turn action resolver so transcript and rollback enrichment cannot clear newer requests. Carry the committed clear through publisher close and released-UI Stop, including the initiating UI and the registered socket adapter. Compute the enclosing close's badge change before deactivation. Cover storage, no-turn closure, HTTP fallback, publisher close and released UI Stop through existing SQLite and real Socket.IO test harnesses. Thanks @danljungstrom for tracing the durability gap and putting terminal settlement at the server turn writer. Follow-up commit f20b718 keeps that owner and fixes the remaining publication gaps: publisher close, the registered socket adapter, explicit Stop, and the initiating released UI now receive the committed request clear. Later transcript/rollback enrichment preserves newer requests, and enclosing-close badge changes are computed before deactivation. The server build, test-package typecheck, 1,481 server unit tests, 33 SQLite integration tests, and three real HTTP/Socket.IO flows passed. One existing sharp-runtime test is skipped; device visual QA and actual Expo badge delivery were not run. The 0.3 persistence clear already exists; its missing publication pieces are being adapted separately. This is a partial correction for #191, so the PR description uses Refs rather than closing the issue. CodeRabbit's docstring coverage warning does not identify a behavior defect; the owner-level explanation and protocol note describe the settlement contract. The previous bot summaries found no actionable issues, but their recorded source basis predates this follow-up. @coderabbitai full review Posted on behalf of @leeroybrun. |

Further corridor review found a merge blocker in the current head f20b718: the pending counts are session-wide, but a terminal foreground turn does not own every request counted there. The live CLI constructs a separate execution-run permission handler against the same ApiSessionClient and passes it into createExecutionRunBackend. Run-scoped requests are namespaced, published into the same AgentState.requests map, and counted by deriveActivitySummaryFromAgentState without a foreground-turn filter. Bounded execution runs start asynchronously and return while running; their permission handler is reset when the session closes, rather than when the foreground turn completes. Consequently, a background run can still be awaiting approval when this PR commits and broadcasts zero counts for a completed foreground turn. This revises my earlier favorable ownership assessment. The transaction and fanout refinements fix delivery, but terminal turn status is not sufficient authority to settle the aggregate. A timestamp cutoff would not establish request ownership either: a background approval may arrive during the foreground turn. The correction needs to follow actual request settlement at the existing permission/request owner and preserve requests owned by other live runs. The evolved 0.3 runtime has the same constraint, plus occurrence-scoped questions that intentionally survive their causal turn. CodeRabbit and Greptile approved the current head, but neither review addressed this reachable background-request case. I am holding merge and leaving #191 open. No release is implied by the earlier validation results. Posted on behalf of @leeroybrun. |
Keep pending counts owned by acknowledged AgentState writes. Retain exact completion intents in the existing encrypted runtime journal, rebase them through current-state CAS, and wait for admission before provider completion. Preserve background requests through foreground terminal events. Co-authored-by: Dan Ljungström <danljungstrom@users.noreply.github.com>
Preserve automatic completion recovery and source-owned restored request cancellation through the existing durable settlement journal. Remove the competing in-memory recovery map and retain acknowledged UI decisions, current-socket retries, and exact occurrence guards. Co-authored-by: Dan Ljungström <danljungstrom@users.noreply.github.com>

The owner correction is now pushed at ef2000e. It supersedes the terminal-count reset and resolves the background-request blocker described above. Thanks @danljungstrom for identifying the lost completion write. The correction now retains the exact request settlement through the existing encrypted runtime journal and publishes it through the acknowledged Agent-state writer. Foreground completion preserves independent requests. The newer reconnect work is consolidated into this same journal owner, including failed automatic history and cancellation of restored source-owned requests without a local waiter. There is no second recovery map or aggregate-clearing path. Validation: the final CLI batch passed 203 tests across 10 files and the CLI TypeScript 7 check passed. The preceding merged branch passed 259 adjacent CLI tests, 39 real SQLite integration tests, the server build, and the tests-package compiler check; the later consolidation leaves those server paths unchanged. Five real-process cases passed, including offline admission followed by process restart and concurrent-state CAS rebasing, plus HTTP completion, publisher close, released UI Stop, and the existing spawn flow. New regressions were observed failing before their fixes. Browser visual QA, Expo badge delivery, and native Windows/macOS execution were not run. The 0.3 port remains in progress through its evolved metadata writer and device-local sealing owner. Its shared checkout has unrelated compiler failures, and the same-home upgrade journal format is being handled explicitly. This PR remains a partial correction for #191; the issue is referenced rather than closed. Merge remains held until current-head CI/review and the required port disposition are complete. @coderabbitai full review Posted on behalf of @leeroybrun. |

Keep pre-effect approvals on the existing acknowledged AgentState writer. Retain only observed native completions and cancellation facts in the runtime journal, reject superseded decisions before effects, and preserve cancellation retry context without stranding local waiters. Cover delayed UI and native observations, failed cancellation admission, asynchronous fallback errors, and exact committed outcomes through real request-store logic and session transport boundaries.
Forward the existing delivery outcome through the settlement adapter so authenticated schema-invalid data uses canonical dead-letter handling and cannot block following mutations. Retain ciphertext and preserve retryability for missing or incorrect keys.

The review corrections are pushed in 8e9d9b7 and 3fd1e2b. All findings from the previous CodeRabbit and Greptile reviews have an evidence-backed disposition and their threads are resolved.
Validation includes meaningful RED-to-GREEN regressions, 283 selected permission/boundary tests before the final classification change, 96 outbox/client/persistence owner tests after it, CLI and tests-package typechecks, and the real offline/process-restart/CAS recovery test. The unchanged server corridor passed its broader unit suite, build, and SQLite integration checks. Current-head hosted CI is still pending; browser visual QA and native Windows/macOS execution remain unverified. The 0.3 port is applied and under validation. Its legacy journal compatibility cases still need correction, and a port-only commit depends on existing uncommitted native-permission prerequisites in that checkout. Merge remains on hold. Issue #191 remains referenced rather than closed. @coderabbitai full review Please review current head 3fd1e2b, especially approval versus observed-fact ownership, exact occurrence replay, and invalid-payload handling. Reserve durable admission while capturing each settlement and serialize sealing through journal persistence. Keep the Agent-state lock free during disk work so ordinary acknowledged writes can progress. Exercise the completion-versus-Stop race through real credential I/O, encryption, journal and CAS delivery.

Fixed the ordering issue from CodeRabbit's review in 0c53b5c. Holding the first credential-file read reproduced a later Stop being replayed before an earlier observed completion. Admission is now reserved in capture order and serialized through sealing and local journal persistence. The Agent-state lock releases before that disk work; ordinary ACK writes and close cleanup remain usable. This extends the existing journal owner without adding another outbox, persisted sequence or protocol. Validation: the regression failed with canceled instead of approved before the correction. Five focused cases and CLI TypeScript 7 passed; an independent full run passed all 47 journal/settlement/codec cases, including next-turn ACK progress, automatic-history recovery, failed admission, close, and encrypted restart/rebase paths. The 0.3 port and follow-ups are locally committed through 87398e2. Saved foreground prompts retire while independent Run prompts/cached decisions retain their owner; whole-Session cleanup stays intact. Its ordering disposition is not applicable: current journal producers only retire observed cancellations, while native completions use retained receipts/direct ACK. Destination validation includes five composed Stop cases, 181 owner cases and independent deciding-case execution. Its full CLI check remains blocked by 12 diagnostics in seven untouched files, and the real two-process recovery gate remains blocked in shared-dependency preparation before either runtime child starts. These are explicit limitations, not passing gates. @coderabbitai review Please review the final admission delta at this exact head. Known unrelated source CI failures remain assigned to the separate maintenance session. cc @leeroybrun Posted on behalf of @leeroybrun. |

@coderabbitai review The only conflict was serverScopedMachineRpc.test.ts. Its resolution matches upstream exactly and retains all five handoff failure cases. The PR's settlement owners and settlement documentation are unchanged by this merge; independent delta review found no material issue. All 47 settlement/journal/encryption tests, all 24 affected UI tests, and both CLI and UI typechecks pass on the integrated source. Current protocol declarations were rebuilt for validation. This integration adds no new PR intent to port into 0.3. The 0.3 changes remain committed through 87398e2, with the previously reported broader validation blockers. The maintainer owns unrelated 0.2 CI fixes separately. cc @leeroybrun Posted on behalf of @leeroybrun. |

Merged as 6ca2342 using the maintainer-authorized account and branch-protection override. Final PR head: 1b1b65b. Current dev was integrated before merge. The only conflict resolution matches upstream's expanded handoff test exactly; settlement owners were unchanged. Independent delta review found no material issue. The integrated source passed all 47 settlement/journal/encryption tests, all 24 affected UI tests, and CLI and UI TypeScript 7 checks. CodeRabbit approved the integrated head. Greptile's preceding review found no blocking issue, and all actionable review threads were resolved. The implementation keeps request state, acknowledged writes, and recovery in the existing permission store/coordinator, Agent-state writer, and encrypted mutation journal. Pre-effect permission choices still require acknowledgement. Pending counts remain derived from canonical request state. The 0.3 intent port is committed, including its native-completion prerequisite, through 87398e2. Focused owner and composed tests pass. Broader 0.3 validation is still blocked by unrelated package typecheck failures and the two-process test's server/dependency setup; it is not certified fully green. The final source integration adds no new port intent. Unrelated 0.2 CI failures remain for the maintainer's separate session; this merge does not assert green CI or release availability. cc @leeroybrun Posted on behalf of @leeroybrun. |

Cancellation loses to pending approval
cancelPendingRequest() still calls completeResponse(), which returns early while a UI approval awaits acknowledgement. Execution-run disposal uses this path, then clears its tracked request IDs. The cancellation never invalidates the approval, so a late acknowledgement can release the waiter with an approved result after the run was disposed.
Route lifecycle cancellations through the cancellation path. It can interrupt an in-flight decision and reject waiters if saving fails.

Offline retries delay local saves
An offline journal retry calls updateAgentState() without first checking the connection. That method holds agentStateLock while waiting for the socket, for 10 seconds by default. New local settlements need the same lock, so retrying an already-saved settlement delays saving the next cancellation or observed completion.
Return a retryable result before taking the state lock when disconnected, or move the connection wait outside that lock.
Port PR #532 by intent: foreground terminal mutations, publisher close and released UI Stop preserve session-wide request projections. Agent-state and metadata tuple writes remain their single authority. Reuse the existing Stop sender policy and cover background requests through both metadata layouts. Validation: 63 SQLite integration tests passed; the broader server compiler gate remains blocked by existing errors outside this change. Co-authored-by: Dan Ljungström <danljungstrom@users.noreply.github.com>
Port the PR #532 response-predicate correction through the current draft and encryption test callers. Keep canonical URL parsing so request query parameters cannot prevent a successful response from matching. Validation: Playwright collection passed for8 tests in3 files. Browser execution remains unverified, and the tests-package compiler gate fails in existing unrelated migration code.
Port PR #532 by intent through the existing 0.3 request store, encrypted runtime journal and metadata tuple writer. Retain exact observed retirement facts while preserving acknowledged pre-effect and native-application lifecycles, newer requests, claims and terminal outcomes. Preserve permission occurrence identity across reconnect, reconcile bulk cancellation with committed subjects, and withdraw expired HTTP admission through the existing hook lifetime. Use device-local encryption for new intents and retained predecessor keys only for predecessor reads. The focused 15-case journal integration suite and six MCP approval cases passed. Full package compilation and the real two-process recovery gate remain open; the generic Stop follow-up is not included in this commit. Refs #191 Co-authored-by: Dan Ljungström <danljungstrom@users.noreply.github.com>
Pending permission indicators can remain stale when a decided request cannot reach the server. This change retains exact request settlements in the existing CLI runtime mutation journal and replays them through the existing versioned Agent-state writer. Refs #191.
A foreground turn ending does not own every request in a session: independent execution runs may still be awaiting approval. The terminal aggregate reset has therefore been removed. Acknowledged Agent-state writes remain the sole owner of pending counts and their published projection.
Decisions that authorize a provider effect use the existing acknowledged Agent-state write before that effect. They are never journaled as future authorization. Observed native tool completions and request cancellations are retained in the existing journal; automatic decision history uses the normal write first and retains a failed completion there. Exact occurrence, expected absence, claim, and current-version checks preserve background work, replacement requests, and committed outcomes. Restored source-owned requests without an in-memory waiter settle through the shared request store on cancellation.
The newer reconnect correction is consolidated into this journal path; its competing in-memory completion recovery is removed. No new server request registry, database schema, or wire contract is introduced. The journal retries while the runtime is active and resumes when the same session runtime reopens. It does not add unattended daemon takeover. Reliable settlement delivery requires the updated CLI. Token-only accounts also keep settlements encrypted through the existing installation identity, scoped to the account, server, and session. Existing account-key entries retain their format; recovery reuses retained identity keys rather than creating replacement keys.
Validation against the corrected branch is listed in the follow-up comment. Linux real-process tests cover offline admission, process restart, concurrent state/CAS rebasing, HTTP completion, publisher close, and released UI Stop. Browser visual QA and native Windows/macOS execution remain unverified. The 0.3 port follows its existing metadata writer, request coordinator, and device-local encryption owner; it is tracked separately until validated.
