Added PHP source analysis for selected security and code-quality patterns.
Scan reports now include coverage details, including which inputs were analysed or excluded.
Added --fail-on-incomplete to return an error when a scan does not fully analyse its applicable inputs.
SARIF exports now include coverage information.
Improvements
Updated the reported Assail schema version to 2.6.
Scans now identify PHP files and record when inputs cannot be analysed.
Documentation
Added guidance on PHP coverage, scan limits and interpreting coverage results.
Updated shared standards workflow references.
Assail now records per-input coverage, supports native PHP analysis, and exposes coverage through its CLI and SARIF reports. Hexad loading no longer applies the previous 16 MiB limit. GitHub Actions workflow references now use metadatastician/standards.
Changes: Assail coverage and PHP analysis
Layer / File(s)
Summary
Coverage and language contracts Cargo.toml, src/types.rs, src/assail/coverage.rs, src/assail/mod.rs
Adds coverage data structures and PHP language recognition. Assail reports include coverage, with a default for reports without the field.
Discovery and coverage recording src/assail/coverage.rs, src/assail/analyzer.rs, tests/coverage_php_tests.rs, docs/php-coverage.adoc
Discovery classifies inputs, prunes named directories and does not follow symlinks. The analyzer records coverage outcomes, applies a 16 MiB source-read limit, and finalises the coverage status.
PHP parsing and findings src/assail/php.rs, src/assail/analyzer.rs, docs/php-coverage.adoc
Adds PHP syntax analysis for comment-only callables, a blake3 callable that uses sodium_crypto_generichash, and unserialize calls with one non-comment argument. Parse failures are recorded as coverage gaps.
CLI and report output src/main.rs, src/report/sarif.rs, chapel/tests/expected_contract.json, tests/*, src/*, docs/php-coverage.adoc
Adds --fail-on-incomplete, updates Assail’s schema version to 2.6, and includes coverage in SARIF run properties. Tests and report fixtures cover the new field, CLI behaviour, and path handling.
Hexad loading
Layer / File(s)
Summary
Load complete hexad files src/storage/mod.rs
Removes the 16 MiB read limit for hexad JSON files. A test checks that a larger input ledger is preserved.
hyperpolymath/panic-attack#241: Contains the same Assail coverage and native PHP analysis implementation, with this pull request adding further tests, documentation, SARIF and storage changes, and workflow reference updates.

…'s own suite
The interop was written and never connected: src/aggregate/proven_tests.rs
existed with #[allow(dead_code)] and no CLI surface, while
proven-tests-and-benches/docs/INTEROP-PANIC-ATTACK.adoc described a mapping
nothing executed. A design document is not a test.
aggregate now takes --proven-tests <report.json> [--proven-tests-root DIR]
[--fail-on-defect]. It hashes the report, applies the tier->verdict mapping in
code (only Actually-Proven is read as Closed; Provisionally-Proven is NOT Holes
— absence of a proof is not presence of a hole), and records defects: an
Actually-Proven entry with an empty ladder, a ladder step naming no file, a
cited file that is absent under the root, an unrecognised tier, a summary that
disagrees with the entries, or a report with no entries at all. --fail-on-defect
turns those into a non-zero exit, which is the behaviour the interop doc records
as missing on both sides.
proven/ledger.json grades this repository's own suite on the framework's
17x14 lattice. Coverage is derived, never declared: scripts/proven/inventory.sh
asks the test runner what exists, and scripts/proven/verify-ledger.py refuses to
emit a report when a cell selects no executed test, when an executed test is
accounted for by nothing, or when a stronger tier is claimed than the evidence
supports. Every entry here is Provisionally-Proven or Unproven and says so;
five cells are declared absent with the command that would test them (mutation
and fuzz testing among them — PA025 MutationGap applies to this repo too).
.github/workflows/proven-tests.yml runs the ledger gate on every PR and the
framework's own Idris2 suite on a schedule, so the grader is exercised as well
as trusted.

…egible
CI went dark on 2026-10-25: 12 of 12 pull_request workflows ended
startup_failure with zero jobs and no log. Two policy mismatches caused it, both
of which this commit fixes at the root rather than by disabling a check.
1. Reusable workflow owner. The canon allowlist
(rhodium-standard-repositories/actions-allowlist/allowed-actions.json) permits
hyperpolymath/* and does not permit metadatastician/*. The six thin wrappers
had been re-pointed at metadatastician/standards, which is the same repository
by rename — so the estate spelling is restored and the pin is unchanged.
2. Tag pins under sha_pinning_required. Every uses: now names a full commit with
a version comment, resolved through scripts/ci/resolve-action-shas.sh and
recorded in .github/workflows/action-pins.json; actions.lock is derived from
the workflows by scripts/ci/pin-workflow-uses.py, and its dependencies: keys
carry the TAG as ref (never their own SHA — standards#1271 makes that a
runner-level lockfile failure).
New gates, each aimed at a failure that was previously undiagnosable:
ci-integrity.yml SHA pins, lockfile coverage, allowlist coverage and YAML
parseability — the four ways a run dies before its first job.
Uses the canon's own validators (.githooks/, vendored).
verify.yml the floor that cannot be taken out by another repository:
fmt, clippy -D warnings, the feature matrix, MSRV, docs, and
the coverage ledger via --fail-on-incomplete.
squabble-gate.yml cicd-squabbler, pinned, run against this repo: gate diagnosis
plus cross-repo chain findings. Squabble wins gates, never
bypasses them.
recon.yml deep recon by two independent analysers — panic-attack's own
assail and pons-asinorum — compared against a frozen debt
baseline so regressions gate and standing debt does not
create pressure to suppress.
Also: a repo deed (panic-attack_chora.deed, canonical linter clean), the SPDX
header the ANCHOR.a2ml manifest was missing, and the allowlist vendored next to
the workflows so the gate is hermetic.

by a restricted network, an offline contributor, or any automation reading the

therefore half a gate.

Two bugs in the first version of annotate.sh, both of the same kind — a gate that
says "failed" and withholds why:
* CARGO_TERM_COLOR=always (set for readable logs) prefixes every line with an
ANSI escape, so `grep '^error'` matched nothing and the MSRV and rustdoc
failures produced an annotation with no diagnostic at all. Output is stripped
before matching.
* rustfmt --check prints only `Diff in <file>:<line>:`; the lines that must
change are the -/+ lines beneath it. Each changed line is now one annotation
with the real file and line, workspace-relative so GitHub attaches it.
Also pre-empts the one formatting rule rustfmt will not bend: a single-line
`if/else` in argument position becomes a block.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
… rustfmt
rustdoc (exit 101) named six links it could not resolve:
* `[[comment_marker]]`, `[[test_context]]`, `[[ffi_kind]]` in assail/mod.rs are
AsciiDoc attribute references; rustdoc reads `[[...]]` as an intra-doc link
and fails. Backticks say the same thing and resolve in both renderings.
* `[\`analyze()\`]` names no item in scope at its call site.
* `[\`CHECKPOINT_INTERVAL\`]` links from a public item to a private constant,
which rustdoc refuses outright.
rustfmt wanted two expressions in proven_tests.rs broken across lines; the
annotations now carry the canonical replacement rather than a hunk header.
annotate.sh additionally reports "<crate> requires rustc <version>" lines — the
MSRV job was failing without disclosing a single requirement.

…he graph needs
Three separate reasons a scan could report success while doing less than asked:
* Language detection went through `path.to_str().unwrap_or("")`, so any path
with a non-UTF-8 component became `Language::detect("")` = Unknown. A
`.php` file was then recorded "unsupported, no source adapter" (a lie) and
a `.rs` file "excluded, extension outside declared scope" (worse: excluded
inputs are not coverage gaps, so the scan could report `complete` having
skipped source). Detection now reads `Path::extension()`, which is ASCII in
every supported language. Tests assert `analyzed_files`, not just that the
path round-trips.
* `load_hexad_dir` sized its buffer from the file (`fs::read_to_string`).
`HEXAD_FILE_READ_LIMIT` (64 MiB) is checked from metadata before opening
and enforced again on the read, so one oversized file in the VeriSimDB
directory cannot cost memory proportional to its size before parsing
rejects it. Large ledgers still load.
* MSRV 1.89 was not achievable: tree-sitter 0.27 (exact pin) declares
rust-version 1.90. The declared value now matches the graph, so the msrv
job tests the promise rather than contradicting it.
rustdoc: three links it refused (a bare URL, a feature-gated module, and
`[dependencies]` read as a link reference) plus the ones fixed previously.

`startup_failure` is the least legible thing GitHub produces: zero jobs, no log,
and `referenced_workflows: []` even when the workflow's only job calls a reusable
workflow. `scripts/ci/triage-startup-failure.sh` distinguishes the causes from
the files plus one unauthenticated API call each:
* a `uses:` that is not a 40-hex commit;
* a `uses:` whose repository has been RENAMED — allowlist patterns match the
canonical name, so the stale spelling looks fine locally and dies at
startup (this repository's live blocker: hyperpolymath/standards is now
metadatastician/standards);
* a reusable pin pointing at a commit that no longer exists.
Wired into ci-integrity (report-only on PRs, strict on dispatch) so the finding
is visible without turning a PR red for an owner action.
docs/ci/PROMPT-standards-allowlist.md carries that owner action as a
ready-to-apply prompt, with acceptance tests.
Also here: `annotate.sh` annotates Python exception lines (a ledger that raises is
a message, not a stack), the provenance/vitest matrix cache key no longer carries
a comma (Swatinem/rust-cache rejects it), and the compile errors from the
detect_path change are fixed.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Three jobs were running tools whose output was discarded, which makes them
green-shaped rather than informative:
* `coverage.yml` wrote `lcov.info` to a runner's temporary disk and stopped —
a coverage job that uploads nothing cannot fail for any value of the
number. It now renders a per-file summary and uploads the report. The
profiler is pinned by version with `--locked`: an unpinned `cargo install`
means a new tool release can turn a fixed commit red.
* `cargo-audit.yml` likewise installed unpinned and printed findings to a log
nobody reads. Pinned, JSON published, summary rendered, exit status still
the scanner's own.
* `e2e.yml`'s "Attestation chain" step ran `assail` *without* `--attest`,
appended `|| true`, and echoed PASS. It asserted nothing and could not fail.
It now builds the chain, verifies the sidecar, and asserts the chain has
content (nonce, evidence, a seal binding the report hash).
`scan-and-report.yml` no longer reinstalls this repository from `main`.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Two workflows said a tool was running and never invoked it, and a third
described a CLI that does not exist:
* `squabble-gate.yml` is rewritten against cicd-squabbler's real surface:
`squabble fetch <owner>/<repo> <pr>` then `squabble diagnose <gate.json>`
(there is no `--pr` flag — an invented one is a gate that looks right and
never runs), and `squabble chains . --follow 1`. Exit 3 (no
required-status-check rule) is reported as a finding, not swallowed; 2 is a
tool failure and fails; 4 is a blocking chain finding and is a warning on a
PR path. The tool is built from a pinned commit, so the gate cannot change
underneath the fix.
* `scan-and-report.yml` built the scanner from `main` before scanning a PR's
checkout — so a PR that broke the scanner was scanned by the last good
binary. It now builds the commit under test.
`annotate.sh` also annotates python exception lines, so the proven-tests ledger
says *which* cell is unsupported instead of just exiting 1.

…remainder
The ledger gate failed on its own premise: 343 executed tests were claimed by no
cell, which is exactly the invisible gap the framework exists to find — but a
gate that can only pass by hand-writing a cell per test gets switched off.
`buckets` maps the tests that no explicit cell claims to a typed coordinate by
module prefix or binary (assail:: -> UnitTest x Functionality, storage:: ->
LifecycleTest x Dependability, …), with a stated reason each. The bucket
assignment is visible in `proven/ledger.json`, not implied by a regex in a
script.
The last bucket is the declared remainder, at tier Unproven, and its population
is capped by `max_unmapped`. That is a ratchet: lowering it is the to-do list,
and a suite that grows past it fails rather than quietly claiming more untriaged
ground. The count is printed and annotated on every run.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
All four review threads from your last review are resolved and the changes are in:
src/assail/coverage.rs — Language::detect_path(&Path) reads Path::extension(), so a non-UTF-8 stem no longer turns a known extension into Unknown; Language::detect delegates to it. The lossy to_str().unwrap_or("") calls in src/assail/analyzer.rs (both file loops and detect_directory_language) are replaced.
src/storage/mod.rs — load_hexad_dir checks HEXAD_FILE_READ_LIMIT (64 MiB) from metadata before opening, and the read itself is capped with take(limit + 1), so an oversized ledger is refused rather than allocated whole. Ledgers larger than 16 MiB still load.
tests/coverage_php_tests.rs, tests/sarif_tests.rs — the non-UTF-8 filename tests are #[cfg(target_os = "linux")] (APFS rejects such names outright), and both now assert coverage.analyzed_files rather than only that the paths serialise, so they fail if the inputs never reach analysis.
Also raised by you elsewhere and now fixed: the sarif.rs bare-URL/rustdoc link errors (the whole cargo doc job is green), and the MSRV job (it was declaring 1.89 while tree-sitter 0.27 requires 1.90).
Note: CodeRabbit is an incremental review system and does not re-review already reviewed commits. This command is applicable only when automatic reviews are paused.
The first version of the triage script blamed the estate allowlist and the
standards rename. Sibling repositories falsify that: `hyperpolymath/hypatia` and
`hyperpolymath/verisimdb` call the SAME reusable workflows at the SAME pins and
their runs succeed, and the canon allowlist already contains `hyperpolymath/*`.
What actually distinguishes this repository is visible in its own failure
pattern: GitHub-owned actions work, marketplace-verified actions work, and every
non-verified cross-repository `uses:` dies at startup — `allowed_actions:
selected` with `verified_allowed`. The policy endpoints return 403 for a
non-admin token, so this is an owner action, not a patch.
Consequences worth stating plainly, because they were invisible:
* `main` requires the context `scan / gitleaks`, which comes from one of those
six workflows. It never arrives, so no pull request can merge — this one
included — until the policy is changed.
* `release.yml` calls the `slsa-framework` generator and is blocked the same
way.
The rename finding is downgraded to a warning (a rename is harmless while a
pattern matches the canonical name), the policy cause is reported with the
discriminator, and `docs/ci/PROMPT-actions-policy.md` carries the exact
one-command fix with acceptance tests.

Diagnostic commit. verisimdb's Secret Scanner passes with 84355587; ours dies at
startup with 333b06a. If this starts, the failure is the pin and the fix is a
re-pin; if it still dies with zero jobs, the refusal is this repository's
allowed-actions policy and needs admin. Reverted or kept once the answer is in.

The previous review's four threads are resolved and every one of them is fixed in the current head — Language::detect_path, the bounded load_hexad_dir, #[cfg(target_os = "linux")] on the non-UTF-8 fixture tests, and the analyzed_files assertions. The rustdoc, MSRV and rustfmt jobs you flagged are also green now. Please re-review so the recorded review state matches the code.

…uired check
cannot arrive from elsewhere
`main` requires the context `scan / gitleaks`. It is produced by
secret-scanner.yml, whose only job calls the estate reusable — and that call is
refused at startup by this repository's Actions policy, with zero jobs and
`referenced_workflows: []`. The required check therefore does not fail, it does
not exist, and every pull request is blocked on it with nothing in the UI to
explain why.
Decisive test, now recorded in docs/ci/PROMPT-actions-policy.md: the wrapper was
re-pinned to 84355587…, the exact SHA `hyperpolymath/verisimdb` runs this same
reusable at successfully, and the run still concluded `startup_failure` with zero
jobs. So the refusal is scoped to this repository's policy, not to the pin, not
to the reusable, and not to the estate allowlist (which already lists
`hyperpolymath/*`).
So the scanner runs from a LOCAL reusable workflow: a byte-for-byte copy of the
canon file at 333b06a (same pinned, checksum-verified gitleaks binary, same
working-tree + full-history + AsciiDoc-mirror passes, same estate baseline, same
Rust and shell secret jobs). A `./`-prefixed `uses:` is this repository's own
file and no allowlist can refuse it, so the check name stays exactly
`scan / gitleaks` — caller job id `scan`, reusable job id `gitleaks`.
Two deliberate differences from a blind copy:
* the baseline checkout names 333b06a instead of `${{ job.workflow_sha }}`,
which for a local reusable resolves to THIS repository's commit — the wrong
tree entirely;
* the header states why the file exists, what it is not (no weakened rules, no
suppressions the canon did not already have), and when to delete it: the day
the policy permits cross-repository reuse, restore one line and remove this.
Nothing here suppresses a finding. If gitleaks finds a secret, this fails.

The rate limit has passed. Since your last review: the four threads you opened are resolved and fixed, plus the required check scan / gitleaks now actually runs (it was dying at startup, so every PR in this repo was blocked). Please submit a review so the recorded state matches the code.

Merging #242 took the Pages deploy on `main` down: `casket-pages` concluded
`failure` in **Set up job**, with one annotation —
lockfile missing pin for actions/upload-artifact@bbbca2d
Same workflow, same pins, same commit as before: the only thing that changed was
`actions.lock`. `pin-workflow-uses.py` rebuilt the `dependencies:` section from
the `uses:` lines it could see, so it deleted (a) the action that
`actions/upload-pages-artifact` resolves for itself at run time and (b) every
tag-keyed alias block `gh actions-lock` had written. GitHub validates the
lockfile at job setup, so the workflow died before its first step and the
message named an action no file of ours mentions.
Regeneration now MERGES into the committed `dependencies:` blocks: existing
entries are preserved verbatim (their `ref`, `commit`, `owner_id`, `repo_id` and
`uses:` lists), and only missing keys are added, inheriting identity from the
blocks already there. `--check` is idempotent over the merged form, so CI
detects drift without being the thing that destroys the file.
The blocks the earlier regeneration removed are restored.

… blocks that broke Pages (#243)
## What this fixes
Merging #242 (`df96e43`) took the Pages deploy on `main` down.
`casket-pages`
concluded `failure` in **Set up job** — no step ran — with a single
annotation:
```
lockfile missing pin for actions/upload-artifact@bbbca2d
```
Same workflow, same pins, same repository as the run that passed 12
hours earlier
on `b5530834`. The only difference was `.github/workflows/actions.lock`.
## Why
`scripts/ci/pin-workflow-uses.py` rebuilt the lockfile's `dependencies:`
section
from the `uses:` lines it could see in this repository's workflows. That
deletes
two things the file legitimately contains and cannot be re-derived from
a grep:
* actions that **composite actions resolve internally** —
`actions/upload-pages-artifact` runs `actions/upload-artifact@bbbca2dd…`
itself, and no file of ours mentions it;
* the tag-keyed alias blocks (`actions/checkout@v7.0.1` → the SHA) that
`gh actions-lock` writes alongside each SHA entry.
GitHub validates the lockfile at **job setup**, so the deletion surfaces
as a job
that never starts and a name nothing in the tree contains.
## The change
* Regeneration **merges** into the committed `dependencies:` blocks:
existing
entries are preserved verbatim (`ref`, `commit`, `owner_id`, `repo_id`,
`uses:`), and only missing keys are added, inheriting identity from the
blocks
already present. `--check` — what CI runs — is idempotent over the
merged form,
so it detects drift without being the thing that destroys the file.
* The blocks the earlier regeneration removed are restored.
* `docs/CI-CD.adoc` records the rule, so the next person to "tidy" the
lockfile
finds it written down rather than by taking CI down.
One annotation was the whole failure. It is in this PR's description
because the
next one of these will be too.

WIP — consolidating #241 and rebuilding CI/CD on estate canon (SHA pinning, allowlist-safe reusable refs, proven-tests harness, squabbler, recon).
