The order is now data (MiddlewarePipelineBuilder.CreateDefault named steps), customizable through a configure overload with startup-validated load-bearing adjacencies, and frozen by MiddlewarePipelineOrderTestsBase. Retires the two costs the record carried as open trade-offs and refreshes every path:line anchor to the new step definitions.
