fix(planning): show only unanswered questions under Open and keep position after an answer

Summary

On the interview page, Show: Open listed answered questions whenever Claude revised or replied to them after the answer. In a live 111-question interview that put 13 answered questions among the open ones. Answering a question also jumped the page back to the first question.

Fix

• passes(): Open is "needs you and not answered"; a new Review filter is "needs you and answered". The group counter counts only unanswered questions as open and appends , N to review, and a group with items to review stays expanded.
• treeHtml: a filtered-out node drops out and its matching descendants move up a level, instead of every answered node with dependents staying visible.
• railOrder(all) returns the unfiltered rail order, so nextNeeding keeps its position after the answered question leaves the filter, and filters candidates with passes.
• ui_c.js and ui_journey.js assert the new contract: accepted-with-a-later-reply goes to Review, not Open; the counter reads 0 open / N, 1 to review.
• planning 0.61.2 with a CHANGELOG entry.

Verification

• bash plugins/planning/surface/surface.test.sh: PASS=480 FAIL=0 SKIP=1 (the skip is htmlhint, not installed).
• Live check on the real interview data with playwright-cli: Show: Open listed 48 questions and 0 answered ones in Groups, Rounds and Tree view; Show: Review listed exactly the 13 answered-and-revised questions; no page errors.
