docs(design): mark v0.4 delivered after browser acceptance - #61

v0.4 quality passed its exit browser test (delivery import idempotency, SCAR / 8D loop with return and overdue sweep, OTD / lot pass rate / PPM, high-risk admission audit gate, audit findings to SCAR, annual audit plan, quality manager and IQC role sweep). Route table and README now list v0.4 as delivered (17 objects, core total 28) and point at v0.5.
