The documentation and scope criteria are verified, but the test criterion is not confirmed:

Governance requirement: Implemented with four escalation tiers, contacts by role, response SLAs, and change history.
Maximum two files: Met; exactly two files changed.
No changes outside Governance: Met.
No regression: No runtime code changed, but the test suite could not run.
Tests and standards: Local links and git diff --check passed. Jest could not start because dependencies are missing (cross-env / node_modules); Prettier is also unavailable.
Change documented: Met; the new process is versioned and indexed in the Governance README.
