🎯 Functional Correctness | 🟠 Major | 🏗️ Heavy lift

Validate the approval that begin returns.

If replace_scope revokes a tool grant without changing policy_ref, a repeated begin can return the existing Approved row at Line 490 before this check runs. consume_inner then denies that row because the grant no longer holds. Validate the returned row’s live authority on both the existing-row and newly published paths. Checking only the automatic value calculated for the new attempt does not cover an older row.

Validate the live authority of the row returned by `begin` on both the
existing-row and newly published paths in `begin` (the flow containing
`automatic_stands`); do not rely only on the new attempt’s `automatic` value.
Ensure a reused Approved row is checked against the current grant before it is
returned.

P2 Badge Revalidate persisted automatic winners on every begin

When a grant or automatic posture is replaced while persist awaits, the approved row has already been stored before this check rejects the racing call. A subsequent identical begin finds that row in existing_begin and returns early at lines 486–490 without calling automatic_stands, so an idempotent audit backend can still return the revoked row as Approved rather than creating a pending human request. Revalidate existing automatic rows against the live scope as well, or retire/replace the stale durable row before returning the race error. This contradicts the documented behavior that automatic approvals are revalidated before publication and that replacement revokes unsupported approvals.

Preserve the scope if retirement is cancelled.

If the caller drops retire_scope while release_scope(...).await is pending, the removed entry never reaches the reinstatement branch. A later retirement returns Denied, so the subject cannot be released through a retry. Make the remove-and-release transition cancellation-safe, or retain a retryable retirement state until release completes.

Update `retire_scope` so removing an entry and awaiting `release_scope` is
cancellation-safe: if the future is dropped before release completes, preserve
or restore the entry, or retain a retryable retirement state, so a later
retirement can retry instead of returning `Denied`.

Stop waiting if begin exits before CAS.

If the spawned begin returns before its first CAS, cas_calls never increases and this loop yields indefinitely. The test then times out instead of reporting the earlier failure. Wait for either the CAS checkpoint or task completion, with a timeout.

Update the CAS-wait loop in the spawned begin test to wait for either cas_calls
to advance or the begin task to complete, and bound the wait with a timeout. If
begin completes before the CAS checkpoint, fail promptly instead of continuing
to yield indefinitely.
