Fix GitHub release creation after publish

Summary

• Add an explicit GitHub release creation step after Changesets publishes npm packages.
• Use docs/releases/vX.Y.Z.md when present, otherwise fall back to generated notes.
• Keep the step idempotent if the release already exists.

Validation

• YAML parsed locally with Ruby YAML loader.
