feat: ship CLI quota release

Summary

• ship CLI-first quota table release as codex-switch@1.0.7
• add GitHub Actions CI on Ubuntu and Windows
• remove dashboard package, stale output artifacts, and unused history/session code
• keep npm package focused on root CLI bins plus compiled CLI/shared dist

Validation

• pnpm typecheck
• pnpm test
• pnpm lint
• pnpm build
• npm pack --dry-run
• npm publish --dry-run
• docs validation

Notes

• npm registry currently has codex-switch@1.0.6; this branch bumps root package to 1.0.7.
• live npm publish still requires npm auth in the publishing environment.
