fix(sdk): expose mutation completion and refusal audit provenance - #1586
Close receipts now distinguish asserted completion times, preserved values and close-time inference. Policy refusals now disclose their tracked audit append, so a failed mutation no longer leaves an unexplained history change.
Seal portable completion timestamp/source evidence under the item lock, preserve it in compact CLI/MCP receipts and successful bulk rows, and provide correction guidance for inferred dates.
Preserve explicit completion timestamps through the bulk SDK/MCP adapter, including the snake-case alias.
Return tracker-relative refusal side effects and the exact sealed record hash through typed SDK errors and CLI/MCP diagnostics. Retain separate attempt evidence and write-free dry runs.
Document the contracts and update the reviewed SDK surface. Tracker records and changelog are generated through pm and the latest npm pm-changelog package.
Expose durable completion provenance and policy-refusal audit effects across mutation APIs and diagnostics.
New Features:
Expose completion timestamp provenance and correction guidance in close receipts across SDK, CLI, MCP, and bulk APIs.
Expose tracker-relative audit side effects and sealed record hashes for policy-refused mutations across typed and diagnostic error surfaces.
Bug Fixes:
Preserve explicit completion timestamps through bulk SDK and MCP input adapters, including the snake-case alias.
Make repeated policy refusals independently auditable while keeping dry-run refusals write-free.
Enhancements:
Document completion provenance and policy-refusal audit contracts and update the reviewed public SDK surface.
Documentation:
Document completion timestamp sources, correction guidance, and refusal audit side effects.
Tests:
Add regression coverage for completion provenance across SDK, CLI, MCP, and bulk transports, plus policy-refusal audit disclosure and repeated-attempt identity.
Chores:
Update changelog and project-management evidence for the completed delivery and policy-refusal work.
Return asserted, existing or close-time completion provenance from single and
bulk closes, and seal portable timestamp/source evidence in the same locked
history event. Preserve explicit bulk timestamps through canonical and
snake-case SDK/MCP inputs, existing completion values, compact receipts,
reproducible history and established JSON key prefixes.
Disclose successful policy-refusal audit appends through typed side_effects,
tracker-relative history coordinates and the sealed record hash. Explain the
tracked write in human diagnostics while retaining per-attempt evidence,
unchanged item history, write-free previews and audit-failure causes.
Extend transport, history, attribution and output regressions. Make the token
replay consume completion context and account for its measured 156-token cost
without changing unrelated ceilings or the seeded regression control. Update
the public SDK snapshot, authoring documentation and package-owned changelog.
Record live PM ownership, regression evidence, dependency and release inventory,
and independent Sentry/telemetry verification for pm-gh1579 and pm-gh1585.
Closing an item now reports its completion timestamp and whether it was provided, preserved, or inferred from the close time. Bulk close results include the same information.
Policy-refusal errors now disclose any audit records written, including their location and record hash. Dry-run refusals report no side effects.
Documentation
Updated SDK and workflow-policy guidance to describe completion details and refusal audit records.
The pull request adds completion provenance to close results and history, and reports tracked audit writes caused by workflow-policy refusals. It also updates related tests, documentation, public-surface records, and project history.

Close results now include a completion receipt with a timestamp and source: asserted, existing, or close_time. Close history records the timestamp and source. Successful bulk-close rows include the receipt, and completed_at is accepted as a bulk-input alias when completedAt is absent.

Summary
Audit append and error disclosure src/core/shared/errors.ts, src/core/policy/workflow-policy-store.ts, src/cli/error-guidance.ts, sdk/public-surface.json
Non-dry-run refusals include the successful audit append’s tracker-relative path, operation, and record hash in error context. CLI structured guidance carries the side_effects field. Audit-write failures retain their separate error path.
Refusal tests, policy documentation, and issue records tests/integration/workflow-policy-mcp.spec.ts, tests/unit/sdk/policy/workflow-policy-runtime.spec.ts, docs/WORKFLOW_POLICIES.md, CHANGELOG.md, .agents/pm/issues/pm-gh1585.toon, .agents/pm/history/pm-gh1585.jsonl
Tests check side-effect details in CLI and MCP responses, distinct hashes for repeated refusals, and empty side effects for dry-run refusals. Documentation and project records describe the refusal audit behavior and reported validation.

Explanation
The title "fix(sdk): expose mutation completion and refusal audit provenance" matches the main changes. The diff adds completion provenance receipts to close operations (SDK, CLI, MCP, bulk rows) and discloses tracker-relative audit side effects for policy refusals. The title names both linked issues' themes (#1579 and #1585) and follows a conventional-commit style. It is specific and does not use vague terms.
Full details: Description check
Explanation
The description states the two main behaviors: close receipts that distinguish asserted, preserved, and close-time completion timestamps, and policy refusals that disclose their tracked audit append. It also lists bulk-row and snake-case alias handling, documentation and SDK surface updates, the linked issues (#1579, #1585), and validation steps. It is directly related to the changeset.
Full details: Linked Issues check
Explanation
The PR meets the coding requirements for #1579 and #1585. For #1579, close receipts and close history context record timestamp and source values for asserted, existing, and close_time; inferred values include correction guidance. Bulk close rows and both completedAt and completed_at inputs are covered. For #1585, non-dry-run refusals append a policy_refused audit event and expose its tracker-relative path, operation, and sealed record_hash through typed errors and CLI/MCP diagnostics. Dry-run refusals remain write-free. The linked tests and documentation cover these contracts. Optional reason parsing and refusal coalescing are not required objectives.

Full details: Out of Scope Changes check
Explanation
The changes include unrelated project-management records. .agents/pm/history/pm-4ydh.jsonl records a security-advisory inventory, .agents/pm/history/pm-7zs0.jsonl and .agents/pm/reminders/pm-7zs0.toon record release-health activity, and .agents/pm/history/pm-bj7rq0.jsonl and .agents/pm/issues/pm-bj7rq0.toon record a separate test-tracking item. These records do not implement or document #1579 or #1585.
Resolution
Remove the unrelated PM history, issue, and reminder changes, or provide a concrete link between each record and the #1579/#1585 mutation contracts.

The PR adds end-to-end provenance for successful closes and policy refusals: completion timestamps and their source are sealed in history and exposed through all mutation surfaces, while refusal audit writes expose their tracker-relative path and exact record hash without changing dry-run semantics. Documentation, public-surface metadata, generated PM records, and regression coverage are updated accordingly.

Expose completion provenance for close operations across SDK, CLI, MCP, and bulk interfaces.
Seal timestamp and source under the item lock in close history context.
Return asserted, existing, or inferred completion receipts with correction hints for inferred values.
Preserve completion data in compact receipts and successful bulk rows.
Accept both camel-case and snake-case bulk completion timestamp inputs.
Make it possible to distinguish completion timestamps asserted during a close from timestamps inferred from the close time, including preserving existing completion values.
Give users actionable correction guidance when a close infers completion time, and ensure explicit completion timestamps work across close interfaces, including bulk operations.
Prevent repeated identical refusals from accumulating redundant tracked audit records.
❌
The PR explicitly preserves a distinct audit event for every refused attempt, including repeated identical requests, rather than coalescing them.
