fix(sdk): recover crashed writers and preserve JSON diagnostics - #1623
Superseded by #1629, which preserves this branch's complete implementation and PM history, includes the subsequent refusal-accounting correction, and runs the final delivery checks against main.
The combined delivery contains crashed-writer recovery, monotonic lock waits, Windows atomic fixture publication, native merge-receipt proof, structured JSON advisories and Sentry CLI 3.8.2 adoption, together with the extension SDK boundary fixes. Review findings from this PR are addressed in the combined source.
A process killed inside a fenced mutation leaves a cleanup gate younger than
its ten-second stale threshold. Ordinary writers previously exhausted their
three-second wait budget before checking whether that gate owner was dead.
Capture same-host identity on gate owners and retire proven dead generations
immediately into retained nonempty recovery directories. This preserves the
original receipt and prevents competing recoverers from moving a replacement
gate after the first recovery wins.
Use monotonic clocks for acquisition, commit-gate and release deadlines while
retaining wall-clock lease timestamps and existing live or unknown owner
policy. Keep conflict diagnostics in whole milliseconds.
Repair the related Windows generation fixture with the existing atomic writer
and its bounded sharing-violation retries. Handle background handoff rejections
at creation and assert their settled outcomes before temporary-root cleanup.
Add Node/Bun killed-owner, concurrent recovery, evidence failure, live/foreign/
legacy owner, frozen-clock and Windows sharing-violation regression controls.
Focused validation passes 141 tests; isolated baseline controls reproduce the
four intended failures. First-rename SIGKILL probes distinguish the working
2026.10.21 release, failing 2026.10.26 release and repaired checkout.
Record PM ownership, reproduction, public-distribution verification and the
separate pm-presets Sentry release blocker. Full and hosted gates remain part
of reviewed closeout for pm-gh1609 and pm-gh1558.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
Improved lock recovery after a process exits, including during a commit, while preserving recovery evidence and preventing conflicting recovery attempts.
Lock wait limits now remain reliable when the system clock changes.
JSON command output now reports preflight warnings as structured diagnostics while preserving command results and errors.
Changed
JSON output can now be selected with --format json; explicit TOON output still takes precedence.
Documentation
Clarified lock recovery behavior, wait limits, manual recovery, and how preflight warnings appear in JSON and human-readable output.
Tests
Expanded coverage for lock recovery, concurrent access, wait limits, Windows file replacement, and CLI warning output across supported runtimes.
This PR updates cleanup-gate owner recovery and lock timing, changes how CLI preflight warnings appear in JSON output, expands workspace receipt tests, and pins the Sentry CLI development dependency to version 3.8.2. It also updates related documentation, changelog entries, and PM records.
Changes: Lock Recovery
Layer / File(s)
Summary
Owner validation and gate retirement src/core/lock/lock.ts, docs/CONFIGURATION.md, docs/SDK.md
Cleanup-gate recovery distinguishes unreadable owner metadata from missing or invalid metadata. It retries release reads, verifies owner tokens before retiring confirmed-dead generations, and retains gates when ownership cannot be verified. Elapsed wait measurements use a monotonic clock.
Tests cover recovery races, owner-read failures, replacement gates, acquired and fenced phases, and Windows rename retries. Documentation and records describe recovery behavior and validation results.
JSON invocations collect warnings and attach codes and details to error envelopes. Warnings that remain after successful completion are written to stderr. Bootstrap JSON selection supports --format=json; non-JSON warning output remains immediate.

Explanation
The title "fix(sdk): recover crashed writers and preserve JSON diagnostics" matches the two main themes of the changeset. The raw summary shows lock-owner recovery for writers killed inside the commit fence, plus structured JSON preflight diagnostics in the CLI and SDK. The title omits the Sentry CLI pin, the fixture and receipt test work, and the monotonic wait change. Those are secondary to the main changes, so the title is concise and accurate.
Full details: Linked Issues check
Explanation
[#1609] src/core/lock/lock.ts adds generation-preserving recovery for confirmed-dead owners, token revalidation, foreign-host and unreadable-owner safeguards, and monotonic wait timing. The lock tests cover recovery races, replacement gates, PID reuse, frozen-clock bounds, and Node/Bun crash cases. Documentation covers operator recovery. [#1558] tests/unit/core/lock/lock-lease-fencing.spec.ts uses atomic fixture publication, retries Windows EPERM rename failures, settles background operations, and retains fencing assertions. [#1602] src/cli/preflight-warnings.ts, src/cli/main.ts, src/cli/migration-gates.ts, src/cli/error-guidance.ts, and src/sdk/cli-bootstrap.ts implement invocation-scoped diagnostics, JSON failure and success handling, local --format json, and TOON precedence. The related tests cover these paths and preserve exit codes. The reported bounded native Windows results support the shard investigations. The incomplete final-head hosted coverage is a merge gate, but it does not establish an unmet coding requirement.

The description directly relates to the changeset. It identifies the superseding pull request and summarizes crashed-writer recovery, lock waits, Windows fixture handling, receipt validation, JSON diagnostics, and Sentry CLI 3.8.2 adoption.
Full details: Out of Scope Changes check
Explanation
The pull request includes changes not connected to directly linked issues #1609, #1558, or #1602. package.json pins @sentry/cli to 3.8.2, and CHANGELOG.md records that dependency change. The pull request also adds or updates release-health, telemetry, dependency-cooldown, review-readiness, startup-budget, dispatch-selection, DeepScan, and unrelated open-issue records, including pm-gh1622, pm-gh1624, and pm-gh1625. These changes belong to separate PM work. The linked lock, Windows, receipt, and JSON-diagnostic implementation and test changes have a direct connection and are not out of scope.
Resolution
Remove the unrelated Sentry CLI upgrade and its changelog entry from this pull request. Remove unrelated release-health, telemetry, dependency, review, startup-budget, dispatch-selection, analysis, and unrelated issue records. Keep the implementation, tests, documentation, and focused PM records that support #1609, #1558, and #1602.

The PR fixes dead commit-gate recovery by validating same-host process death and atomically retaining retired generations as recovery evidence, while converting lock wait deadlines to monotonic timing. It also strengthens Node/Bun crash and fencing coverage and makes Windows generation fixtures use production atomic-write retries with deterministic background-task settlement.

Do not remove a cleanup gate after its owner read fails.
readStaleCleanupGateOwner returns null for every read error. removeExpiredStaleCleanupGate then treats the gate as ownerless and removes it once it is older than STALE_CLEANUP_GATE_STALE_MS. That removal can vacate a gate whose live owner is still using it. A contender can then create a replacement gate at the same path. If an observer that read a dead owner calls retireDeadCleanupGate after that point, the rename moves the replacement into recoveries. The replacement holder then runs its fenced commit without the gate.
This sequence also exists at the merge base. There, the same fallback rm deletes the replacement instead of renaming it. The PR changes the operation from deletion to a rename, but does not change the trigger or the loss of exclusion. Treat this as a pre-existing hazard that the PR does not fix.
The destination check in retireDeadCleanupGate does not block this sequence. The fallback removal creates no recovery destination. Renaming the gate first and then checking its token does not work either. The rename vacates gatePath, so another contender can acquire it before the check runs. A failed restore leaves the moved gate in recoveries while its commit continues.
Suggested direction: treat an owner read error other than a missing file as an unknown owner. Return false before the age fallback. Keep the fallback for missing or malformed owner files, so gates left by a crash still recover. readStaleCleanupGateOwner is also called by releaseStaleCleanupGate, so keep that caller's release behavior unchanged. This is a prose direction only. The inspected code does not establish a safe patch.
Note: this behaviour predates this PR (already present at merge base 5f1540b56b31); the change keeps it reachable.

A cleanup gate may be owned by a long-lived SDK or MCP process. A single
EBUSY or EACCES read during release previously left the gate held until that
process exited, because recovery correctly retains uncertain live owners.
Retry an inconclusive owner read at most three times using the existing
short jittered delay. Continue to require the original ownership token and
retain the gate when metadata remains unreadable or ownership changes.
Add independent EACCES and EBUSY regressions that fail before the change
and verify the next writer can acquire the gate after release. The linked

Keep the unclaimed-item advisory structured in JSON mode.
When pm update <id> --status in_progress --output-format=json targets an unclaimed item, bootstrap selects JSON but getGlobalOptions().json remains false. Line 351 therefore reaches this direct printError call and writes human text to stderr. The new warning scope cannot attach that advisory to structured diagnostics. Use effective JSON selection for this gate, or route this advisory through emitPreflightWarning.

The --output-format=json selector chose a JSON invocation while the mutation
guard still saw json=false. Updating unclaimed work to in_progress therefore
wrote a human advisory to stderr outside the structured warning scope.
Resolve the current invocation renderer before the legacy boolean fallback.
Represent an explicit human scope separately from a missing scope so TOON
selection also wins over an inherited json=true flag. Direct guard callers
retain their existing JSON fallback and human advisory behavior.
Extend the existing real tracker test with renderer precedence and both
canonical JSON option spellings. The new regression fails before the change;
all five linked commands pass afterward, including 78 CLI integration tests,
180 warning/error tests, 143 selector tests, 47 guard/scope tests, and all
TypeScript projects. Full exact-head hosted checks remain mandatory.
Record the outside-diff review under pm-gh1602, retain immutable closure and
recurrence evidence, regenerate and check the package-owned changelog, and
verify terminal defect evidence without weakening gates.
