Why

Make missing test and review evidence visible during a bug fix. Keep successful evidence tied to the code snapshot that produced it.

What changed

• Add a separate, opt-in Claude Mods adapter for reproduction, fixes, verification, and two independent Thermos reviews.
• Capture actual process exits and invalidate evidence after code changes, interruptions, or stale callbacks. Late tool failures cannot block a replacement pipeline.
• Show the current stage, missing evidence, and failures; generate and validate the package from fork-owned inputs.

Scope

Bug-fix workflows on Claude Code 2.1.289 and Unix hosts. Existing pstack, Thermos, and Codex packages remain unchanged from current main. The adapter does not merge, deploy, or enable itself in an existing user session.

Tradeoffs

Require a clean committed reproduction baseline and reject unsupported Git state. Mods and bounded Stop reminders are workflow checks, not a universal security or completion guarantee.

Blast Radius

Only users who install the separate plugin and explicitly start a recipe enter the pipeline. CI gains pinned native Mods checks.

Verification

• 131 Python tests and 89 official native Mods tests passed, including new current/stale reviewer Read and parent Edit failure regressions.
• Strict native validation, generated TypeScript 5.9.3 checks, no-inference load, and both-runtime installer/source parity passed.
• Fresh independent correctness and quality reviews found no remaining actionable issues. The remote tree and modes match the reviewed local tree; current main was merged without rewriting existing commits.

Authenticated real-model execution remains unverified. Native reviewer identities and results are stubbed in the test harness. See the original verification evidence and limits.
