feat: workspace rules — proposal card, owner review, approved rules in every session - #1605
Replaces #1438 (lesson sync), which is closed in favour of this redesign. Server side: AltimateAI/altimate-backend#7514 (/datamates/{id}/rules) and AltimateAI/altimate-frontend#3603 (the workspace Rules tab).

What does this PR do?
In a project linked to a workspace, what the person teaches the agent can become a team rule. They confirm it on a card, the workspace owner reviews it, and approved rules reach every teammate's sessions. This replaces #1438's background lesson sync, which sent staged lessons without asking.
Proposal card, built once in the session server (src/altimate/rules/proposal.ts, events.ts). It uses the same ask/reply shape as question: rule.proposal.asked goes out on /event, and the client answers with POST /rule-proposal/:id/reply. The TUI renders it now (plugin/tui/altimate/rules.tsx); the IDE extension can render the same event later with no logic of its own.
Enter sends the rule to the owner's queue; e edits it (re-linted); n ("Not a rule") suppresses the topic for 30 days.
The owner also sees a to approve at once, which becomes "approve anyway" when the rule weakens verification.
An unlinked project gets "Save for this project" instead.
When a card appears (trigger.ts), checked after each turn:
a topic corrected in 2 or more sessions with no rule covering it (from recurrence's unmatched topics);
a correction on the subject of a team rule the session was shown (an edit or retire card, with the current text struck through);
the person asks to remember a rule: training_save with kind: "rule" and project scope now shows the card instead of writing training.
One model call (the reflector, scoped to the triggering corrections) words the rule. If the drafter says the rule already covers it (HELPFUL), no card appears.
Rule or memory (classify.ts). A preference ("I prefer…") or a fact ("our warehouse is…") is memory, saved without review: training_save sends the agent to altimate_memory_write. When the wording says neither, the card asks "Save as: Memory · Team rule · Skip".
Sync (sync.ts, service.ts, store.ts):
Pull happens at session start (bounded wait), every 5 minutes, and on Sync now. It brings the approved rules, the settings, and review notices for the person's own proposals ("approved", or "rejected" with the reason). The notices show as TUI toasts.
Delivery: approved rules are mirrored into a read-only, git-ignored workspace-rules store (.altimate-code/learn/workspace-rules/; the name is reserved, and a store named team stays the person's own), so the existing delivery, recurrence and learn show treat them like any lesson. They reach the prompt's "Team rules" with their applies_to globs as path triggers. They stop being delivered when the project is unlinked, the person signs out, access is refused, or workspaces are turned off.
Outbox: proposals are queued offline-safe, per signed-in user (user:<id>), and sent on the next run under a cross-process push lock. Only an item the server refuses on its own leaves the queue; an account or workspace change drops it with a notice.
Usage: applied (sessions a rule was delivered in) and corrected again (recurrence after exposure) are reported back to the workspace.
Learning from sessions. In order: the learning kill switch, ALTIMATE_LEARN_CAPTURE, a learn.capture set by a config layer above project files (organization policy: managed config, MDM, the account's config, inline config, OPENCODE_CONFIG_DIR or the home config), the person's /rules choice (kept in git-ignored rules state), learn.capture in the project file, then the workspace's "Learn from sessions". startCapture takes force so the rules service can start capture on the workspace's say-so.
Also:
Server routes GET /rule-proposal, GET /altimate/rules/summary, GET /altimate/rules and POST /altimate/rules/sync.
The TUI gets /rules and a sidebar rules: line.
learn sync is new, and learn status shows a rules block with the last proposal check.
Behaviour changes for scripts:learn status text prints Learning from sessions: on|off (source) where it printed Capture:, and existing learn messages say "rule" where they said "lesson". The JSON output is unchanged apart from the added rules block. In a linked project with a client showing cards, training_save and altimate_memory_write can return metadata.actionproposed, memory, redirect or error.
In a linked project, learn bootstrap and learn import-reviews send what they find for review instead of staging a local candidate.
api-client errors now carry the coded detail.
Docs: "Workspace rules" in configure/learn.md.
Not in this PR:
the one-time card offering to bootstrap from history in the background (learn bootstrap is the path today);
helpful/harmful usage counts (sent as 0);
the IDE extension's renderer.
Beyond the backend contract
This PR also adds, beyond syncing rules with the workspace:
the inline proposed-rule card in the TUI (ctrl+x p), and /rules (team rules, your proposals, Sync now, your own learning on/off);
a per-person learning setting (POST /altimate/rules/learning, stored in the git-ignored rules state);
learn bootstrap / learn import-reviews offering their staged rules to the owner's review, with a confirmation that names the workspace (--send-for-review for scripts);
training_save in a linked project proposing a team rule instead of saving project training (only when rules are enabled);
the rule-or-memory classifier for "remember …" requests.
Not in this PR (follow-up): proposals where no client renders cards — altimate-code run, ACP, and the Claude Code / Codex plugins (there the save tools save as before cards). The VS Code extension renders cards through AltimateAI/vscode-altimate-mcp-server#540.
How did you verify your code works?
bun run typecheck.
test/altimate/rules/rules.test.ts (129 tests): classification, applies_to, learning precedence, the team mirror, the summary line, card actions per project, edit lint and the verification flag, dismissal, sync (push, pull, usage, scope changes, locks), card replies and their failure paths, "remember" fallbacks and crash recovery, and the after-turn check.
Existing learn, carry-forward and server suites: 2438 pass. The httpapi-experimental / httpapi-mcp timeouts fail on a clean main too.
End to end, with the TUI in tmux against a local backend with #7514 and the Rules tab from #3603:
"remember this as a team rule" → card → sent → owner approves in the web → the next session receives it, and the agent cited it;
the same correction in 2 sessions → the card appears on its own with "seen in 2 sessions" and the quote;
editing the wording;
rejecting with a reason → a toast with the reason;
owner Approve now, and Approve anyway for "skip dbt tests…";
"I prefer CTEs" → saved to memory, no card;
usage counts visible in the web.
Cost side effects
Drafting: one model call per proposal check that finds a candidate, at most one per turn, and only after a correction (never for turns without one). It runs with the configured learn.model or the session's model.
HTTP: one request to the workspace every 5 minutes per open project, plus one per sent proposal and per usage batch. Sizes are bounded (≤50 proposals and ≤500 usage items per request).
Screenshots / recordings
"Remember this as a team rule" → card (owner view: Send, Approve now, Edit, Not a rule)
Corrected in 2 sessions → card on its own, with the evidence and the correction quote

A rule that weakens verification: flagged, "Approve anyway"; the wording was unclear, so it also offers Memory
Rejection notice with the owner's reason
/rules after approval: team rules, your proposals, Sync now, learning on/off
"I prefer CTEs" → saved to memory, no card

Linked projects can sync workspace rules, submit proposals from corrections or explicit requests, and report rule usage.
Use /rules to browse team rules and proposals, manage personal learning settings, and sync. The learn sync command is also available.
Proposals can be edited, dismissed, or saved locally or as memories. Workspace owners can review and approve proposals.
The workspace sidebar displays rule status and the last sync time.
Imported or bootstrapped rules can be submitted for workspace review with consent.
Improvements
Project-level learning settings are supported. Capture and reflection are off by default; approved workspace rules are still delivered unless learning is disabled.
Learning status shows where its setting comes from, and documentation and CLI terminology now consistently refer to rules.
…ules in every session
A linked project's corrections become proposed team rules that the person confirms on a card and the
workspace owner reviews; approved rules reach every teammate's sessions within minutes.
- `src/altimate/rules/`: client for the backend `/datamates/{id}/rules` API (`api.ts`); local state and a
read-only `team` store that mirrors approved rules so delivery, recurrence and `learn show` treat them
like any lesson (`store.ts`); pull, outbox push, "Approve now" and usage reporting (applied,
corrected again) with a session-start pull and a 5-minute refresh (`sync.ts`, `service.ts`)
- Proposal card built once in the server (`proposal.ts`, `events.ts`): `rule.proposal.asked` on `/event`,
answered with `POST /rule-proposal/:id/reply` (send, approve, edit, dismiss, save_local, save_memory)
- Trigger and drafter (`trigger.ts`): a topic corrected in 2+ sessions, a correction of a shown team rule
(edit or retire card), or an explicit "remember this as a rule" from `training_save`; one model call
words the rule. Each check's outcome is recorded for `learn status`
- Rule or memory (`classify.ts`): preferences and facts go to memory without review; ambiguous wording
shows "Save as: Memory · Team rule · Skip"
- Learning from sessions: the person's setting wins, otherwise the workspace's "Learn from sessions"
(capture can now start for a linked project with no local setting)
- Server routes `/rule-proposal`, `/altimate/rules/{summary,sync}` and `/altimate/rules`; TUI card, review
toasts, `/rules` menu and a sidebar "rules:" line; `learn sync`, rules block in `learn status`;
`learn bootstrap` / `import-reviews` send findings for review in a linked project
- Docs: "Workspace rules" in `configure/learn.md`

Summary
Configuration, API contracts, and rule storage packages/opencode/src/altimate/learn/project-config.ts, packages/opencode/src/altimate/rules/api.ts, packages/opencode/src/altimate/rules/store.ts, packages/opencode/src/altimate/workspace/api-client.ts, packages/opencode/src/altimate/learn/store.ts
Adds project learning configuration access, validated rules API operations, structured workspace API error details, and persisted workspace-rule state with a team-rule lesson mirror. The learn store reserves the team name for workspace rules.
Adds rule-versus-memory classification, automatic and explicit proposal triggers, proposal actions, and history-to-workspace proposals. Project-scoped training and memory writes can return a proposal instead of saving locally. Tests cover classification, settings, storage, synchronization, and proposal actions.
Adds proposal push, rule and review-notice pull, usage reporting, and service lifecycle hooks. Session start and service startup trigger synchronization; the service gates, starts, and stops capture based on learning settings.

When a user picks Memory on a rule-or-memory choice card, the saved memory can land in a different block than requested and an expiring memory becomes permanent. Several earlier concerns also remain open: outbox scope handling, rejected-batch retries, staged-candidate upload, and TUI error feedback. Fix the memory-write argument loss and the open sync issues before merging.

Explanation
The title "feat: workspace rules — proposal card, owner review, approved rules in every session" matches the main change. The changeset adds a proposal card flow, owner review through the workspace, and delivery of approved rules to teammates' sessions. The title is specific and identifies the primary feature. It is 84 characters, which is slightly long, but it stays within a readable single sentence.

Explanation
The pull request description is complete and follows the required structure. It identifies the replaced issue and related dependencies, marks the change types, explains the implementation and scope, documents verification and cost side effects, includes UI screenshots, and completes the checklist. The issue section uses “Replaces #1438” instead of “Closes #”, but this is appropriate because the issue is described as closed in favor of the redesign.

Kept as a string. The statuses the server sends are active, rejected and retired, not approved, and it can add more. Clients map the ones they know and show any other as is, so a strict enum would make a newer server's notice fail validation.

Fixed by changing the behaviour. Sync now and the learning toggle close the menu and report with a toast, so there is no open menu left with an old list.
Use the tmpdir() fixture with await using for the temporary project roots.
The tmp helper creates directories with fs.mkdtemp under os.tmpdir() and never deletes them. Every run leaves rules-* trees behind, each with a learn lock directory and state files.
Import tmpdir from test/fixture/fixture.ts. Then use await using dir = await tmpdir() in each test so that cleanup is automatic.
Based on learnings: "For brand-new test files added under packages/opencode/test/altimate/, follow the documented tracing-test temp-dir convention: import tmpdir from fixture/fixture.ts and use await using tmp = await tmpdir()." As per coding guidelines: "Use await using for automatic cleanup when the variable goes out of scope".

Use one shared project-config writer instead of a copy of the learn.ts writer.
projectConfigFile and writeProjectLearning copy most of writeProjectLearning in packages/opencode/src/cli/cmd/learn.ts (lines 88-158). The copied parts are:
the candidate discovery;
the SafeFS.assertSafePath checks;
the JSONC formatting detection;
the temp-file-and-rename write.
The two copies already differ. The CLI copy writes capture, auto_reflect and auto_promote. This copy writes only capture. A later fix to discovery precedence or to the safe-write path must then be made in both places.
Make the CLI command call this module with a list of edits, such as (root, directory, edits: [path, value][]).

CRITICAL:slice(0, 48) after stripping hyphens can leave a trailing -, causing MemoryStore.read to silently reject the saved memory
MemoryBlockSchema (packages/opencode/src/memory/types.ts:14) requires each ID segment to end with an alphanumeric character (/[a-z0-9](?:[a-z0-9_-]*[a-z0-9])?/). Because .slice(0, 48) runs after .replace(/^-+|-+$/g, ""), any rule text whose 48th slugified character is - produces an ID like notes/foo-bar-. MemoryStore.write writes the file without schema validation, but MemoryStore.read and MemoryStore.list fail MemoryBlockSchema.safeParse and silently skip the block. The same issue applies to name in save_local on line 146.
The reason will be displayed to describe this comment to others. Learn more.
Fixed. Names come from slug(): a 40-character prefix with leading and trailing dashes trimmed, plus an 8-character hash of the whole text. The id always ends in an alphanumeric, and two rules that start alike do not collide.
The reason will be displayed to describe this comment to others. Learn more.
CRITICAL: Workspace privacy setting include_quotes is ignored when uploading correction excerpts
Settings (packages/opencode/src/altimate/rules/api.ts:14) defines include_quotes: z.boolean(), which is persisted in RulesStore.State.settings during pull(). However, state.settings?.include_quotes is never checked before attaching card.evidence.correction.excerpt to the outbox payload sent to /proposals. When a workspace disables include_quotes, user correction quotes are still uploaded to the server.

CRITICAL: Passing process.cwd() instead of Instance.directory breaks --dir and can reset the target project's rules store
root is resolved from Instance.worktree / Instance.directory via projectRoot(), but process.cwd() is passed as directory here (and in BootstrapCommand at line 521 and ImportReviewsCommand at line 590). When invoked with altimate-code --dir <project> learn sync from an unlinked working directory, RulesSync.context(directory, learn) checks the binding of process.cwd(), gets status: "unbound", and RulesSync.run calls RulesStore.reset(root) (sync.ts:271), wiping the target project's rules state and mirror.

WARNING:RulesSync.runShared can join an in-flight sync that already finished its push() phase, leaving the newly enqueued proposal unsent and skipping notice events
RulesSync.run executes push() -> pull() -> reportUsage(). If a background refresh is already in flight inside pull() or reportUsage() when reply() calls enqueue() followed by runShared(), runShared() returns the existing promise whose push() phase already completed. When it resolves:
The newly enqueued item was not pushed (nor approved if the owner clicked Approve now).
report?.push?.sent.find(...) is undefined, so reply() reports "Queued: ... when the workspace is reachable" even when online.
Calling RulesSync.runShared directly instead of RulesService.syncNow skips publishNotices, dropping rule.proposal.reviewed events for any notices returned by that pull.
The reason will be displayed to describe this comment to others. Learn more.
Fixed. A card reply uses runFresh, which waits out the run in flight and starts a new one, so the newly queued item is pushed. The card's outcome is looked up by outbox id, including outcomes another run recorded. Since 0d4bfd4, review notices are announced by the run that pulled them, whoever started it.

WARNING: Post-turn rule drafting calls providerGenerate without recording token/cost usage
Unlike auto.ts:139 and session-reflect.ts:105, deps.generate invokes providerGenerate without an onUsage callback and defaults to source.model (the full session model) when learn.model is unset. Per REVIEW.md Focus Area 12, background model calls triggered automatically on session.idle should use a small/cheap default model when unconfigured and track their token/cost usage.
The reason will be displayed to describe this comment to others. Learn more.
Not changed in this PR. The after-turn draft is one model call per idle turn with fresh corrections, and only while learning is on and a client showing cards is attached. It uses learn.model / ALTIMATE_LEARN_MODEL when set and otherwise the session's model, the same as automatic reflection. Recording its token and cost usage is a follow-up.

WARNING:showMenu reads static api.state.config.learn instead of on-disk readProjectLearning, so toggling learning never updates summary.learning in the current session
writeProjectLearning (line 259) writes learn.capture to the project config file on disk, and readProjectLearning in packages/opencode/src/altimate/learn/project-config.ts:50 was added specifically so /rules can read the updated setting from disk immediately. However, readProjectLearning is never called here: learnConfig(api) reads api.state.config.learn, which stays stale for the rest of the session, causing subsequent /rules openings and toggles to see the old summary.learning state.

Lines 74–81 (// altimate_change start — workspace rules... through // altimate_change end) are nested inside the outer // altimate_change start - status lines block (lines 72–82). Non-depth-aware marker parsers (findMarkers and auditBranding in script/upstream/analyze.ts, extractMarkerBlocks in script/upstream/bridge-merge.ts) close the outer block at the first altimate_change end (line 81), leaving line 82 orphaned (REVIEW.md Focus Area 4a).

WARNING:POST /rule-proposal/:id/reply does not scope or bind Instance to the proposal's project directory
RuleProposal stores pending cards in a process-global Map across all open projects (proposal.ts:46). While GET /rule-proposal filters by Instance.directory, POST /rule-proposal/:id/reply does not verify that the card belongs to Instance.directory, and RuleProposal.reply's save_memory and save_local branches call MemoryStore.write / TrainingStore.save using the ambient HTTP request's Instance.directory rather than entry.directory. In addition, GET /altimate/rules/summary (line 1290) and GET /altimate/rules (line 1298) lack try/catch around Config.get().
The reason will be displayed to describe this comment to others. Learn more.
Fixed. POST /rule-proposal/:id/reply passes Instance.directory, and reply answers not_open for a card of another project, so saves run in the card's own project. /altimate/rules/summary and /altimate/rules have try/catch.
The reason will be displayed to describe this comment to others. Learn more.
WARNING: Unclosed proposals in Symbol.for("altimate.rules.proposal.pending") and unremoved temp directories leak across tests
RuleProposal.ask adds entries to the process-global Symbol.for("altimate.rules.proposal.pending") map (proposal.ts:46). This test (and the edit test at line 141) never closes the created cards, leaving them in the global map for the life of the bun test worker where RuleProposal.list() (used unfiltered in trigger.ts:139) sees them.
const tmp = () => fs.mkdtemp(path.join(os.tmpdir(), "rules-")) (line 14) creates 6 temporary directories across the suite without afterEach cleanup or await using tmp = await tmpdir().
The reason will be displayed to describe this comment to others. Learn more.
Partly. Cards are scoped by directory now: list(directory) is used everywhere, including the after-turn check, and tests drop the cards they create. The temporary directories under the OS temp dir are left for the OS to clean, like other tests in this package.

Incremental review of 2758e4ae..3f2f127e (3 files / ~90 changed lines). The new commit makes the discard decision in sendStaged conditional on the candidate file still reading exactly what stagedRules listed, and adds Store.reject({ ifCandidate }) plus a test for the unchanged and concurrently-reflected cases. The mechanism is sound: reject reads the candidate inside the cross-process learn lock (withLearnLock), all candidate writers serialize through Lessons.canonical under the same lock, readCandidate's migrate is a no-op when approved.json/candidate.json exist, and Lessons.parse behaves exactly like the previous loadCandidateLessons, so the guard cannot silently misfire on a normal run.
The one new finding is a message-accuracy edge case on the discard fallback (below). The five remaining findings are the previous round's unresolved items, each re-checked against current HEAD and still present.

Incremental review of 8d83a1a5..2758e4ae (2 files / 9 changed lines). The new commit makes the workspace's "Learn from sessions" decide nothing while ALTIMATE_DISABLE_WORKSPACE is set, which is the correct fix for the shown gap: RulesService.start no longer early-returns on rulesEnabled(), so without this gate followLearning would still read the saved workspace setting and start capture with force even though workspaces are off. The config and per-person choices are still checked before the workspace (so learn.capture and personal_learning keep working with workspaces off), and the new test covers workspace-on, kill-switch, and config-overrides-kill-switch. No new defects in the changed lines.
The five findings below are the previous round's unresolved items, each re-checked against current HEAD and still present.

Incremental review of f5a35a9..8d83a1a5 (round seven, 10 files / ~527 changed lines). The previous round's two warnings are fixed and re-verified: reportUsage now skips sessions older than USAGE_COUNT_MS while remembering their pairs for USAGE_PAIR_MS, so an aged snapshot is no longer re-reported on every sync; and the request/card lifecycle now guarantees the "remember" request is on disk before a card is shown, ends in the same write as the enqueue or dismissal, and is re-kept on a refusal. readShownSnapshot now performs a single read, and recoverRequests retries this process's leftovers and renews live records so a long-lived card is not taken as ownerless.

A requested card is only shown after persistRequest succeeds, and the dedupe branch returns no card (so the save tool writes as before) when the request cannot be kept or the card closed during the write.
The enqueue, dismissal and re-keep paths end the kept request in the same state write (enqueueMany's forget, the dismiss filter, entry.enqueued), so a crash cannot turn a queued or dismissed proposal into a local save.
A failed send re-keeps the request only when the proposal is not queued (refusal, or a landed-then-threw enqueue detected by the record's absence); a queued card clears its fallback so closing later saves no second copy.
recoverRequests claims in one locked write, renews this process's open cards every 6 h, retries its own non-open records, and removes (never saves) answered ids; settling covers the delete-to-forget window within a process.
reportUsage counts only snapshots written within USAGE_COUNT_MS, keeps pairs for 3x that window so nothing countable is forgotten, and refuses to send or acknowledge a batch belonging to another scope.
delivery.ts rebuilds a cached session section when the mirror no longer holds a shown team rule (workspaces off, signed out, unlinked, other workspace, retired) and deliberately keeps it when the mirror cannot be read.

Incremental review of 9ef7382..f5a35a9 (review round six). Fourteen earlier findings from the previous summary are fixed and re-verified in current code: the MAX_USAGE_PAIRS concern is now addressed by age-based pruning (but see the re-count issue below), the recovered "remember" request is kept on transient failures via SaveRefusedError, the push-lock error is only swallowed for the exact busy timeout (PUSH_LOCK_BUSY), from-history decides outbox room inside enqueueMany (upTo), service.ts reuses RulesRemember.subagentSession, the process-boot id fixes PID reuse in recoverRequests, pending_usage.scope prevents sending/acking another scope's batch, the daily TTL is enforced on reply, training_save memory content is written instead of redirected, pulled rules are linted and non-conforming review rows skipped, readShownSnapshot replaces the full-snapshot read on the after-turn path, and prompt.ts reuses canonicalDirectory with an inode/ctime-stamped cache.

Pairs are pruned once their snapshot mtime passes USAGE_PAIR_MS, but the applied-count loop still reads the same old snapshot (no age filter in readShownSnapshots for a project with ≤200 sessions), so every refresh re-reports the same sessions (up to MAX_USAGE_DELTA per rule per run), inflating the workspace's applied counts
packages/opencode/src/altimate/learn/capture.ts

running.add() happens before the Bus subscriptions; if subscribeWhenReady throws, the catch leaves the directory marked running, so service.ts never retries startCapture and correction capture stays dead for the process

packages/opencode/src/altimate/rules/sync.ts
641
reportUsage scans .sessions twice per report (snapshotFiles, then readShownSnapshots): readdir plus an lstat per file each time
enqueueMany(..., { upTo: true }) computes room and slices items/ids inside the same write; the change callback runs once, so ids stay aligned with queued items, and held is reported without rethrowing a full queue as a CLI failure.
push swallows only /^Timed out waiting for lock: rules-push$/; acquire, learn-lock and send failures are rethrown (a card reply still degrades to "Queued" because act catches the failed run).
pending_usage.scope plus ownedBy/sameScope guards prevent sending or acking another account/workspace's batch; cleared() still drops pending_usage on a scope change and keeps it on a same-scope relink.
reply closes (and, for a requested card, saves) a card answered after a day instead of answering it; expire() continues to close them on ask/list/drop.
persistRequest never evicts a live request (refuses at MAX_REMEMBER_REQUESTS), proposeFor falls back to a cardless save at the same limit, and recoverRequests distinguishes this boot's records by PROCESS_BOOT, so a reused PID cannot hide a dead card's request.
settle keeps the request after a transient save failure and drops it only for SaveRefusedError; recovery removes it after a successful or refused save and logs instead of aborting on a failed clear.
sync() filters rules that fail lint or are not normalized text, skips unreadable review rows, counts them as skipped, and the revision stays unset so the next pull retries.
readShownSnapshot validates the file name against sha256(session) and only a session's own snapshot is read; a resumed session's team rules are rebuilt out while workspaces are off, with the session ledger preserved for attribution.

Incremental review of bddd5ed7..9ef7382 (one commit on top of the previous round). All fourteen findings from the previous summary are fixed and verified in current code: the capture_policy precedence (it is now derived only from config layers above project files and deleted from the merged result), learn status using the effective config, the push/learn lock separation (entered), the record-retry restricted to an unacquired learn lock, the usage_reported pair trim (replaced by pruning to snapshot files), the read-time scope_drop expiry, the O(n²) usage lookup (now a Set), the from-history round bound, the prompt.ts bindings-cache memoization, the drop() close promise, the training_save test teardown, the TUI "proposals stay queued" toast ordering, onUsage for drafting, and the rules.json empty-string parse. Five new findings come from this round's request persistence, lock handling and usage retention; one test-isolation nit is carried.

Removing the MAX_USAGE_PAIRS cap on the claim that reportUsage prunes to live snapshots leaves usage_reported unbounded: .sessions/*.json files are never deleted, and the prune is skipped entirely once state.rules is empty. Every RulesStore.update rewrites the growing rules.json under the learn lock and every usage report builds a Set over it
packages/opencode/src/altimate/rules/remember.ts
147
A recovered "remember" request is deleted even when saveAsBefore failed for a transient reason (lock, I/O, store error), not only for the documented "memory or training is full" case, so the request that survived the crash is lost; the removal update is also unguarded and can abort recovery of the remaining claimed records

entered treats every failure to acquire the push lock (EACCES/ENOSPC on the lock dir, mkdir failure, the Flock "possible compromise" errors) as "another sync is still sending proposals", swallowing the real error and leaving proposals stalled; match the push-lock timeout message and rethrow the rest
room is read outside the enqueueMany lock, so a concurrent sync can fill the outbox and the MAX_OUTBOX refusal is rethrown as a CLI failure instead of being reported as held items
packages/opencode/src/altimate/rules/service.ts
103
The new subagent() helper duplicates the lazy subagent lookup in remember.ts (same imports, same "unknown counts as not" fallback); one shared helper keeps them from drifting
adopt/ownedBy gate both the push record and the pull answer/failure, so an answer that arrives after another process adopted a different scope (or a newer revision) is not applied; the per-write check is inside the lock.
pending_approvals is written before any "Approve now" and reconciled in a second record; a retry answered with invalid_status/status: "active" is settled as approved, and a retryable failure keeps exactly one attempt.
record retries only Timed out waiting for lock: learn-state (the exact Flock message for the 5 s RulesStore lock) and never re-runs a change that may already have been written.
enqueue/enqueueMany check the card's scope in the same write, reply settles a ScopeChangedError card (saving a requested one) without sending it, and close now waits for the running reply via done.
Requests kept on disk survive cleared/reset; recovery claims with pid so two starting processes save once, and memory/training saves are idempotent by id/name if a crash lands between save and forget.
effectiveLearn deletes a spoofed capture_policy, sets it only when a config layer above project files set capture, and learningDecision reads it before the personal choice; the TUI says "organization policy" and learn status now reports the same decision.
reportUsage runs before the pull (usage is counted against the versions sessions were shown), only when the adopted state still has a workspace, and push failure/queue reporting is unchanged.
drafted telemetry carries the accounted tokens/cost on both success and NoObjectGeneratedError, and the after-turn check re-checks the renderer lease before showing a card.
The new tests clean up instances before await using removes the temp dir, restore spies and env vars in finally, and the lock/routing tests assert the renewed behaviors.

…pture and the TUI card queue
Sync (`rules/sync.ts`):
- Check the saved scope before `push`: an outbox queued for another account, workspace or remote is dropped, never sent with the new credentials (`adopt`).
- A batch the server refuses is retried one item at a time; a refused item leaves the outbox with its error, so it cannot block later proposals. Transient failures and items with no result stay queued.
- Usage: a rule reports at most 50 sessions per run and the rest wait for the next run; the "corrected again" baseline follows the recurrence window down and only advances by what was sent.
- Key in-flight runs by the real path, so the service and the session loop share one run. `runFresh` waits out an in-flight run before sending a newly queued proposal.
- A failed or skipped run is not waited on again by session start within `REFRESH_MS`.
Proposals (`rules/proposal.ts`, `server.ts`):
- Honor the workspace's `include_quotes`: with it off, correction excerpts are not uploaded.
- Keep a card open until its action succeeds; a second reply while one runs is refused.
- `POST /rule-proposal/:id/reply` answers only cards of the request's project.
- Memory and training names: a readable prefix plus a text hash; no trailing hyphen, no collisions.
- Re-publish an already open identical card so the person actually sees it; "Dismissed" wording for cards without a topic.
Store (`rules/store.ts`):
- Only a missing `rules.json` reads as empty; other read errors propagate, and a file this version cannot parse is kept as `rules.json.unreadable-*` before a write.
- Create the directory and `.gitignore` under the learn lock with `SafeFS`, and rewrite a team `.gitignore` that is not `*`.
- Cap only settled sent proposals, never pending ones.
Post-turn check (`rules/trigger.ts`):
- Mark corrections processed only after the check completes; a failed check is retried. Processed ids are pruned to signals still on disk.
- A HELPFUL verdict on the shown team rule ends the check (no second drafting call). The recurrence path only takes an ADD. Dismissed edit topics are skipped before drafting.
Service and capture (`rules/service.ts`, `learn/capture.ts`):
- Read the learn config (with the project file as on disk) on each refresh and check, so `learn disable` takes effect without a restart.
- `startCapture` returns a stop function; capture the service started stops when learning turns off, retries after a failed start, and is not started after disposal.
Elsewhere:
- `session/prompt.ts`: only a project with rules state awaits the session-start pull.
- `learn sync` / `bootstrap` / `import-reviews` use the instance directory, not `process.cwd()`. Bootstrap and import send only the lessons this run staged, and keep a candidate that existed before; the epilogs describe linked projects.
- TUI card queue: programmatic dialog changes no longer advance the queue, a reply acts only if its card is still showing, open proposals load at startup, edits refresh queued copies, and a card queued behind another dialog shows when it closes.
Tests: outbox retry and scope drop against a local stand-in server, `include_quotes`, failed actions keeping the card, cross-project replies, slugs, unreadable state, `.gitignore`, and the sent cap.

CRITICAL:raw! is undefined when rules.json contains an empty string (""), throwing a TypeError in Store.writeAtomic and breaking all subsequent store updates
When rules.json is missing (ENOENT), readRaw returns undefined and parse(undefined) returns EMPTY. However, if rules.json exists on disk as an empty file (""), readRaw returns "", if (!raw) in parse (line 134) treats "" as falsy and returns EMPTY — wait: if rules.json contains whitespace or invalid JSON (or if readRaw is ever empty/unparseable), wait — let's check both cases: if raw is whitespace like "\n", raw is truthy; if raw is "", parse("") returns EMPTY. Wait — what if raw is undefined? parse(undefined) returns EMPTY, so !current is false. What if State.parse({ version: 1 }) or structuredClone fails? Wait: why is raw! non-null asserted here? Because parse(undefined) returns structuredClone(EMPTY). However, in ensureDir (line 177) and here (line 195), Store.writeAtomic calls assertLearnLock(root) (learn/store.ts:101), which keys its AsyncLocalStorage map by await fs.realpath(root) (learn/lock.ts:33,44), whereas SafeFS.open inside writeAtomic validates expectedDirectory(root, file) using path.resolve(root). Even more importantly, read(root) (line 143) calls readRaw(root) using raw fs.readFile without SafeFS symlink validation, unlike Store.read (learn/store.ts:56).

Not a bug as far as I can tell. parse(raw) returns EMPTY for undefined and for "" (!raw), so current is undefined, and raw! is reached, only when raw is non-empty text that fails to parse.

… or approve now (owner)
An owner saw both "Send for review" and "Approve now". Sending their own proposal to their own review queue adds nothing, so the owner's card offers "Approve now" ("Approve anyway" when flagged) on Enter, and a member's card "Send for review". The `a` shortcut is gone: Enter is the team action. Docs updated.

Ran a multi-reviewer peer review (lead + 2 independent reviewers). This is a substantial, mostly well-structured client: clean rules/proposal/sync layering, per-scope ActAs credentials, scope adoption that drops state/outbox when account/workspace/remote changes, a lock-protected atomic store, a git-ignored team mirror, and a card ask/reply contract with the CSRF guard present on the new routes. Good work.
Scope note: AI-9764 names the backend API + SaaS review screen as in-scope and the altimate-code sync as a sibling ticket, so this client is reviewed on its merits as sibling work. The PR should call out the beyond-ticket additions (the /rules learning toggle, learn bootstrap/import-reviews rerouting, the training_save interception, the rule-vs-memory classifier) in the description.
No CRITICAL issues. The six MAJORs below are blocking — they are correctness/data-loss gaps, several with strong 2-reviewer consensus. The rest are MINOR/NIT. Inline comments follow; nits consolidated separately.

MAJOR (bug/data).classify maps every WorkspaceApiError whose status isn't undefined/408/429/>=500 to rejected, and in push a rejected item is added to done and removed from the outbox permanently. So an expired/revoked key (401, which api-client.req returns as a plain WorkspaceApiError), a proxy 413/414, a gateway 400, or a ZodError from contract drift (after the server already stored the proposals) all cause the user's proposals to be silently dropped with only a transient rejected line. Conversely conflict(409)/forbidden(403)/unsupported never leave the outbox and are not retried per-item, so one poison item at the head of a 50-item batch blocks everything behind it forever; attempts is incremented but never used — no max-attempts, no age expiry, no enqueue cap, so rules.json grows unbounded. Fix: treat only item-scoped refusals (422/400 with a server code) as rejected; map 401/407/413/unknown to a retained kind; treat a parse failure as transient; cap attempts/age/outbox length. Add tests for 401 and a malformed 200 body. (Consensus: both reviewers, MAJOR.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed. 401/407 classify as auth and other uncoded 4xx as transient, and both keep the outbox. Only an item refused on its own (rejected) leaves it. There is no attempts cap that drops anything, and the outbox is bounded by MAX_OUTBOX (a new proposal is refused once it is full).
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (bug/API contract). In a linked project, training_save(kind=rule, scope=project) no longer saves anything locally — it only publishes an in-memory card (the pending Map, not persisted) and tells the model "Follow the rule… do not save it again". With no client able to answer the card — altimate-code run/headless, ACP/IDE clients that don't render cards, a TUI that closes, or a process exit — the rule is lost permanently. proposeTeamRule also doesn't check rulesEnabled/learningEnabled, so with learn.enabled=false or ALTIMATE_DISABLE_WORKSPACE (a stale state.scope survives because the service never runs and nothing resets it) the card is still offered, "Send" enqueues, and runFresh returns off — yet the card says "Queued… when the workspace is reachable" even though nothing will ever drain it. Fix: gate on RulesSync.rulesEnabled(learn) and a connected card-capable client (or persist pending cards under rules.json); otherwise fall back to the existing local training save; make the off/skipped message accurate. (Consensus: both reviewers, MAJOR.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed.
A card is raised only when rules are enabled (rulesEnabled), the project is linked, and a client showing that project's cards holds a renderer lease. Otherwise training_save saves as before.
A card the person asked for that closes unanswered is saved as before cards.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (bug/data).TEAM_STORE = "team" is an ordinary playbook name accepted by Playbook.validateName, and nothing reserves it. The header claims "reflection and promotion never target this store", but every learn promote/reflect/reject --name team writes .altimate-code/learn/team/approved.json, which the next RulesStore.update silently overwrites via writeMirror. Conversely, a user who already has a playbook called team gets its approved.json overwritten on the first pull with no backup, and the forced * .gitignore then hides their playbook from Git. The "read-only team mirror" is a convention, not an enforced property. Fix: reserve the name (reject team in validateName, or use an internal name that fails NAME_RE like _workspace), refuse writes to it in Store mutators, and migrate/refuse a pre-existing team store. Add a learn promote --name team test. (Consensus: both reviewers, MAJOR.)
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (design/API contract). The header says "a thin renderer… No rule logic lives here", and the server exposes /altimate/rules/summary, /altimate/rules and /altimate/rules/sync for clients. But the TUI imports RulesSummary/writeProjectLearning directly and reads .altimate-code/learn/team/rules.json + writes the project config from the TUI process's own filesystem. Under attach/remote/sandboxed server (or the promised IDE client), root(api) is the server's path, so the sidebar, /rules list, notice replay and toggle read/write the wrong machine. The GET summary/list routes then have no in-repo consumer and go untested in practice. Fix: call the server routes from the TUI (as already done for sync and card reply) and keep file access server-side. (Consensus: both reviewers, MAJOR.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed. The TUI uses the server routes: /altimate/rules/summary, /altimate/rules, /altimate/rules/sync and /altimate/rules/learning. File access stays on the server.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (bug)./rules computes summary.learning from api.state.config (the config as loaded at session start) but the toggle writes the project file on disk. After one toggle the in-TUI config is unchanged, so reopening /rules shows the old state and a second "Turn learning on/off" writes the same value again — it looks like the toggle does nothing. The toast says "Applies from the next session" while the server (service.learnConfig) actually re-reads the file and applies on the next idle/refresh, so the user-facing contract is wrong in both directions. The sidebar readRules has the same stale-config input. Fix: drive the summary and the toggle through the server route so the file write and the follow-learning reaction happen where disk state is merged; fix the toast. (Reviewer r2, MAJOR; corroborated by r1.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed. The toggle and the summary both go through the server. The toast says the choice applies in this project for you, and the server reacts at once (a changed event starts or stops capture).
The reason will be displayed to describe this comment to others. Learn more.
MINOR (bug). The pending-card registry is process-global with no TTL. Unanswered cards (Escape, other sessions, a disposed instance) accumulate and are lost silently on server restart — even though training_save told the model "The person sees a card". Dedup keys on root+lowercased text but the card keeps the original sessionID, so "Replied" events for a second session carry the wrong session. (Relatedly, in rules.tsx the 1s drain setInterval, the 4s setTimeout and the on(api) subscriptions are never disposed, leaking timers/handlers on reload, and first-run notice replay can toast up to 10 historical reviews at once.) Fix: drop a project's cards in the instance disposer, add an age cap, register timer/subscription disposers, and seed the notice-seen set silently on first run. (Consensus: both reviewers.)
The reason will be displayed to describe this comment to others. Learn more.
MINOR (API contract).reply failures all return HTTP 409, including "no longer open" (should be 404) and failed edit-lint (should be 422), so clients can't distinguish them without parsing the message, and there's no machine-readable code alongside error. text on edit also has no length bound at the route layer (relies on lint). Return 404/409/422 appropriately with a coded body. (Reviewer r1.)
The reason will be displayed to describe this comment to others. Learn more.
MINOR (logic). The rule/memory regexes run over the person's whole last message (said) and are loose/asymmetric: \bi\s+(?:prefer|like|want|…) is extremely common in ordinary prompts ("I want staging models in models/staging") and with no rule cue yields memory; memoryScope treats any \bmy\s+ as global memory ("never touch my_schema" → global). A genuine team convention is thus silently turned into a personal memory with no card, and an incidental "I prefer …" anywhere in a long message flips the whole kind. Fix: classify the agent-saved content for memory cues, use said only as a tiebreaker for explicit phrases ("as a team rule", "for me"), and prefer ambiguous (the choose card) over memory when the only signal is a generic first-person verb. (Consensus: both reviewers.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed. A generic first-person verb (I want/I like) only makes the text ambiguous, which gives the choose card. memoryScope matches only specific my … phrases. "As a rule" and "just for me" decide on their own.
The reason will be displayed to describe this comment to others. Learn more.
MINOR (bug).edit is allowed for retire cards: it changes card.text while change/replaces stay retire, so the TUI shows edited struck-through text and sends a retire for a text that isn't the rule's. The a key maps straight to approve_anyway for a flagged card with no confirmation. And the server's weakens_verification in ProposalResult is parsed but ignored, so a rule the server flags but the client's verificationWarning does not will fail on approve with no client path to "approve anyway". Fix: disallow edit for retire; require a confirm for approve_anyway; on a verification_flagged response surface the server reason and offer approve-anyway. (Reviewer r2.)
The reason will be displayed to describe this comment to others. Learn more.
MINOR (performance/bounded-size). Bootstrap/import enqueue applies_to from lesson.trigger?.paths with no cap on count or glob length (the interactive card limits paths to 4; this path doesn't). Usage items is likewise one-per-rule with no chunk cap. enqueue also does one locked read-modify-write + mirror rewrite per lesson (N sequential full rewrites). Fix: clamp applies_to count/length, chunk usage items, and add an enqueueMany. (Reviewer r2.)
The reason will be displayed to describe this comment to others. Learn more.
Fixed. applies_to is clamped (MAX_GLOB). Usage batches hold at most 500 rules, and the rest go in the next report. Bootstrap uses enqueueMany (one write).
Comment hygiene (NIT): a few header comments overstate guarantees the code doesn't enforce — store.ts "reflection and promotion never target this store" (see the team-store MAJOR), proposal.ts reply doc "the card stays open until the chosen action succeeds" (see the misleading-outcomes MINOR), rules.tsx "No rule logic lives here" (see the TUI-bypass MAJOR). Align the comments with behaviour or enforce them. Also: project-config.ts mentions "/learn" where the command is learn enable; rules.tsx "Enter on the first row sends" is wrong for the choose card (Memory is first). No Jira/GitHub IDs or prompt leakage found.
Dead state (NIT):store.tsbootstrapped_at is declared ("Set once learn bootstrap proposed rules") but never written or read — remove it, or set it in proposeStaged and use it to avoid re-proposing.
Naming (NIT):sync.tsrun/runShared/runFresh/push/pull/send are only distinguishable by reading their bodies (syncOnce/syncDeduped/syncAfterQueued read better); the inner item field of OutboxItem collides with the loop item (rename to proposal); proposal.tsact → applyAction; trigger.tsdraft() returns object | "covered" | undefined — a discriminated {kind:"covered"} beats the magic string; store.tsreset/update → resetForScopeChange/updateState; Summary.learning_source: "you" is overloaded (person/user clearer).
Approve-anyway confirmation (NIT): the single a keypress approves (and approve-anyway if flagged) for the whole workspace with no confirm — low risk (owner-only, labelled) but a confirm step for the approve_anyway case would be safer.
…input, without blocking typing
The card was a modal dialog. It now sits above the session's input (the `session_prompt` slot, which renders the regular input below it), like the permission prompt's place but without taking the input away:
- Typing and sending messages continue while a card waits.
- `<leader>p` (ctrl+x p) or a click focuses the card: ←/→ (or tab) select an action, Enter confirms, `e` edits the wording, `n` is "Not a rule", Escape goes back to typing and leaves the card. While focused, a high-priority key layer takes those keys and the input is blurred.
- One card per session; "+N more" when others wait. Permission and question prompts hide it, as they hide the input.
- A card answered in another client (`rule.proposal.replied`) disappears; an edit (`rule.proposal.updated`) updates it in place.
- Editing the wording still opens a one-line prompt dialog.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… (Enter in "Edit the rule" kept the old text)
While the card was focused, its key layer (priority 100: enter, e, n, h, l, tab, arrows, escape) stayed active under any dialog. In "Edit the rule", Enter re-ran the card's selected action — "Edit wording" — which reopened the dialog with the old text, and typed e/n/h/l never reached the field. The same happened in the palette.
The layer is now released whenever a dialog is open (explicitly when the edit dialog opens, and by following the dialog's open state for any other dialog) and bound again when it closes while the card is still focused.

…e the menu; the toggle reads the setting from disk
- Sync now and "Turn learning on/off for me" are actions: the menu closes and a toast reports the result (Sync now used to reopen the menu). A failed sync request or config write is reported instead of thrown.
- The menu took "learning on/off" from the config loaded at startup, so after a toggle it kept offering the same direction and wrote the same value again. It now reads the project config's `learn` as on disk, like the rules service does.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…n's messages and docs
"Learn" is the feature (learning from sessions, the `learn` commands, the learning on/off setting); what it produces are rules, as in the workspace's Rules tab. The learn engine still called them "lessons", so in a linked project `learn status` said "Lessons: 2 approved" right under "Workspace rules: 2 team rules".
- `learn` command help, epilogs and output, bootstrap / import-reviews summaries, automatic-promotion outcome lines and hold-back reasons, store errors, the recurrence report and the generated learn `.gitignore` comment: lesson(s) → rule(s). User-facing text only: identifiers, store paths, JSON keys and config keys (`learn.core_lessons`, …) are unchanged.
- `learn status`: one "Learning from sessions: on/off (your setting / workspace setting / default)" line for every project, replacing "Capture: …" (your setting only) and the linked-only "Learning from sessions" line, which showed the same switch twice and could disagree. `--json` keeps `capture`.
- `docs/configure/learn.md`: lesson(s) → rule(s) throughout; the status section describes the new line.
- Tests: expected strings updated.

Consensus review: REQUEST CHANGES
Five independent reviewers each checked the CLI side against the heads of AltimateAI/altimate-backend#7514 and AltimateAI/altimate-frontend#3603, then agreed on this review over two rounds. Every finding below is on its diff line.
Major (blocking)
learn bootstrap / learn import-reviews send model-written rules to the owner's review queue with no card and no confirmation that names the workspace (from-history.ts:41).
A 401 is classified rejected, so confirmed proposals are permanently dropped from the outbox (api.ts:142).
training_save rules in a linked project are lost when no client renders the card (run, the IDE extension) (training-save.ts:125).
Minor (non-blocking): the usage batch_id isn't reused on retry; usage isn't chunked at 500 items; the "for me" learning toggle writes the shared project config; team isn't a reserved store name; why ignores include_quotes; the env parsing in learningEnabled diverges; the first session after linking skips the bounded pull.
Checked and fine
Wire shapes match the backend: sync cursor, proposal items and results, review source: "cli", usage batch_id, rules_not_available 404, repo_not_bound 422, and the 409 codes.
The client verificationWarning agrees with the backend fixture on 103/103 cases, and the edit lint agrees with lint_cases.json on 523/523.
Delivery re-lints every pulled rule and applies_to glob, and the team mirror is git-ignored.
The session-start pull is bounded at 2 s with a 5-minute backoff.
The precedence order puts the person's setting ahead of the workspace's.
The new routes use the existing workspaceRouteRefusal guard.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (consent). In a linked project, every lesson this run staged is enqueued here and pushed to the owner's review queue, with no card and no prompt that names the workspace. The only confirmation is "Send these redacted past-session excerpts to the displayed model?" (learn.ts:502 / :576), and with --yes there is none at all. The texts are reflection output the person never saw. Anything the push can't send stays in the outbox, and the 5-minute background refresh sends it later. That breaks the invariant this PR sets: the outbox holds only proposals the person confirmed, which is why #1438 was closed. The texts also skip the local lint before they leave the machine.
Fix: list the staged rules and ask "Send N rules to 's review in ?" (default no). --yes for the model step should not imply it; add a separate --send-for-review. Alternatively, keep them as local candidates the person sends one by one. Run lint before enqueue.
The reason will be displayed to describe this comment to others. Learn more.
Fixed. After bootstrap/import-reviews the staged rules are listed, and the person is asked "Send these N rules to 's review in ?" (default no). --yes does not imply it; outside a terminal it needs --send-for-review. Staged texts go through lint first, and invalid ones stay local.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (data loss).api-client has no 401 branch, so an expired or revoked key arrives as WorkspaceApiError(status 401) and falls through to rejected here. push (sync.ts:181) treats rejected as done and removes the entry. Example: the person presses Enter on a card while their key has expired. They see "Not sent: Request failed with status 401", the card closes, and the proposal is gone. A background push drops queued items the same way, with no message.
Fix: classify 401 (and a 403 with no code) as transient or auth so the entry stays queued. Drop only a per-item invalid result or a 422.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (regression). In a linked project, training_save(kind: "rule", scope: "project") now only creates an in-memory card (the pending map in proposal.ts) and tells the model "The person sees a card… Do not save it again". No client renders the card in altimate-code run or in the IDE extension (the PR leaves its renderer for later), so the rule is lost. Before this PR, the same call saved project training.
Fix: intercept only when a card-rendering client is subscribed. Otherwise fall back to the existing training save, or persist the card so the next /rules or TUI session shows it.
The reason will be displayed to describe this comment to others. Learn more.
Agreed, and deferred to a follow-up that covers every client without the TUI card: the IDE extension (rendering the same rule.proposal.* events and routes) and headless runs / the Claude Code and Codex plugins (persisting open proposals and a learn proposals command to answer them). In this PR the interception now only happens when rules are enabled (RulesSync.rulesEnabled), so with workspaces or learning off the rule is saved as project training as before.
The reason will be displayed to describe this comment to others. Learn more.
MINOR.batch_id is a fresh UUID on every attempt, which defeats the backend's idempotency. If the POST commits but the response is lost (the 15 s abort, or a reset), usage_sessions and corrected_reported don't advance. The next run then sends the same counts under a new id, and the server counts them twice. Persist {batch_id, items, sessions, corrected} before sending, and retry with the same id.
Blocker: no (edge input; advisory counters).
The reason will be displayed to describe this comment to others. Learn more.
MINOR.items has one entry per rule and is not chunked. The backend caps it at 500 (MAX_USAGE_ITEMS) but allows 1000 active rules. If more than 500 distinct rules have unreported deltas, for example after a long offline stretch in a large workspace, every report gets a 422 and usage never advances. Send chunks of 500 and advance the state per chunk.
Blocker: no (edge input; advisory counters).
The reason will be displayed to describe this comment to others. Learn more.
MINOR. "Turn learning on/off for me" writes learn.capture into the project config (.altimate-code/altimate-code.json / opencode.json), and that file is normally committed. Once one person's toggle is committed, every teammate gets it as their "own setting", which then overrides the workspace's "Learn from sessions" for all of them. Write to a per-user, git-ignored location, or label the toggle as a project setting.
Blocker: no (needs a commit to spread; safe to do in a follow-up).
The reason will be displayed to describe this comment to others. Learn more.
MINOR. Nothing reserves team. learn promote/reflect/reject --name team writes .altimate-code/learn/team/approved.json, which is delivered as team rules until the next state write replaces it. In a repo that already has a committed store named team, the first sync overwrites that store with the mirror. Reserve the name for writers in Playbook.validateName or in the learn CLI.
Blocker: no (edge input).
The reason will be displayed to describe this comment to others. Learn more.
MINOR.why is sent whatever include_quotes says. For session cards it embeds the drafter's free-text reason, which can paraphrase or quote the correction, and it doesn't go through redactSecrets. It is shown on the card, so nothing hidden leaves the machine, but it defeats the workspace's "no quotes" setting. Redact it, and use a fixed string when include_quotes is false.
Blocker: no (shown on the card the person confirms; safe to do in a follow-up).
The reason will be displayed to describe this comment to others. Learn more.
MINOR. The env handling here diverges from learn/config.ts. ALTIMATE_LEARN_CAPTURE=yes (or any value other than 0/1/true/false) forces learning off here, while captureEnabled ignores it and follows config, so capture can run with no cards. ALTIMATE_LEARN=1 together with learn.enabled: false turns learning on in learnEnabled but off here, at line 44. Reuse booleanFlag and learnEnabled.
Blocker: no (edge input).
The reason will be displayed to describe this comment to others. Learn more.
MINOR. The bounded session-start pull is gated on team/rules.json already existing. In a freshly linked project the first prompt doesn't wait, and it can run before the service's initial background pull, so approved rules may be missing from the first turn. Gate on "rules enabled and the project is bound" instead (still bounded by SESSION_WAIT_MS).
Blocker: no (first turn only; safe to do in a follow-up).
The reason will be displayed to describe this comment to others. Learn more.
Fixed. The session-start pull runs when the project has rules state or a cached workspace binding for this directory (mayHaveWorkspaceRules), still bounded by SESSION_WAIT_MS.
Consensus review: nits and test gaps (non-blocking)
Nits
sync.ts:315: unlinking resets the state and silently drops confirmed proposals that were never sent. Tell the person on the next /rules or learn status.
server.ts:1287: every reply failure is HTTP 409. A closed card fits 404 and a failed edit lint fits 422; a machine-readable code would help the IDE renderer.
proposal.ts:48: the pending-card registry never expires, so unanswered cards stay for the life of the server.
rules.tsx:300: the toggle toast says "Applies from the next session", but the service re-reads the file on each check, so it applies at once. Meanwhile the menu reads api.state.config, so it still shows the old state after a toggle.
Missing tests (each would fail without the behaviour it covers)
pull: notices, the reviewed_since cursor, unchanged, and the reset on 404 workspace_not_found / 422 repo_not_bound.
reportUsage: session dedup, the per-rule cap, the corrected-again baseline, and retrying with the same batch_id.
sessionStart against a hanging backend (returns within waitMs).
proposeStaged, plus its consent gate once added.
afterTurn: the edit/retire card on a shown team rule, and the recurrence card.
The training_save interception (linked, unlinked, and no renderer).

Review summary
Solid design overall — the ask/reply card mirrors question, the outbox is offline-safe, and the team mirror reuses the existing lesson delivery. Two issues need fixing before merge:
MAJOR (security):POST /altimate/rules/sync returns the Altimate API key in its response body.
MAJOR: in a linked project, training_save rules become TUI-only cards, so headless/serve/IDE/ACP sessions lose them.
Plus 10 minor findings inline (usage idempotency and 500-item cap, outbox loss on reset, card lifecycle, the "for me" toggle writing committed config, notice publishing, classifier precedence, TUI reading local state, test coverage).

project-config.ts:17-44 — projectConfigFile/writeProjectLearning duplicate the config discovery and JSONC writer in cli/cmd/learn.ts:87-150; two copies with the same name will drift. Have learn.ts use the new module.
project-config.ts:18-96 — Comment hygiene: nested // altimate_change start/end markers in a file that is entirely new; keep only the single altimate_change - new file header.
prompt.ts:771-776 — The session-start pull runs on the prompt path, so it can add up to 2 s to a prompt whenever a refresh is due (every 5 minutes), not only at session start as the comment says. Gate it on the session's first turn or rely on the service timer.
server.ts:1295-1300 — The summary/rules routes use the loaded config's learn, while the service merges the on-disk project learn (altimate/rules/service.ts:30-36), so right after /rules toggles learning the sidebar can disagree with what the service enforces. Share the service's learnConfig helper.
RunReport.context for a ready project is { status: "ready", scope, actAs }, and actAs is { url, instance, apiKey } (altimate/rules/sync.ts:34, :60). Spreading the report into the response means every "Sync now" from the TUI (and later the IDE extension) returns the Altimate API key in the HTTP response body, where clients may log or cache it. learn sync --json already strips it with a replacer (cli/cmd/learn.ts:454); this route doesn't.
Suggested fix: Return a redacted report, e.g. const { context, ...rest } = report; return c.json({ ok: true, ...rest, context: context.status === "ready" ? { status: "ready" } : context }), or keep actAs out of RunReport entirely and pass it internally. Add a route test asserting the response has no apiKey.
In any linked project (proposeTeamRule only checks state.scope && state.workspace, lines 33-34), a project-scoped rule now only creates an in-memory card and tells the agent it was proposed. The only renderer of rule.proposal.asked is the TUI plugin (plugin/tui/altimate/rules.tsx), so in headless altimate-code run, serve + IDE extension, or ACP nothing is saved and nothing is sent — before this PR the rule was saved as project training. The gate also ignores RulesSync.rulesEnabled(): with ALTIMATE_DISABLE_WORKSPACE=1 the TUI rules plugin and the service don't start, yet a stale rules.json with a scope keeps diverting rules into cards nobody can see.
Suggested fix: Only intercept when rulesEnabled(learn) is true and a card-capable client is attached; otherwise fall back to the previous save path, or enqueue straight to the outbox (as from-history does) when no client can answer.
The server makes /usage idempotent per batch_id, but a fresh id is minted on every attempt. If the server commits and the response is lost, usage_sessions / corrected_reported don't advance and the next run re-sends the same counts under a new id, so applied and corrected-again get double counted. items is also never capped: the server rejects more than 500 items, so a project with >500 rules gets a 422 on every run and usage reporting stalls permanently.
Suggested fix: Persist the pending batch (batch_id, items, sessions, corrected) in rules.json before sending and resend the same id until acknowledged; send at most 500 items per batch.
When the owner picks "Approve now" and the proposal is stored but the review call fails (e.g. 409 verification_flagged, or a transient error), push sets row.error = "sent for review; approving failed: …" (sync.ts:210-212). The card then says Not sent: sent for review; approving failed: … — contradictory, and the proposal is actually pending.
Suggested fix: Carry a separate approve_error and, when status === "pending", word it "Sent for review; approving failed: …".

constreport=awaitRulesSync.runFresh(entry.root,entry.directory,entry.learn).catch(()=>undefined)constsent=report?.push?.sent.find((s)=>s.text===card.text)constowner=card.workspace?.owner_name??"the workspace owner"if(!sent)message=`Queued: it will be sent to ${owner}'s review queue when the workspace is reachable.`
runFresh waits for the run in flight and then starts a new one (sync.ts:348-351). If the in-flight run (background refresh, session start, another card's reply) read the outbox after this enqueue, it already sent this item; the new run's push.sent doesn't contain it, so the person is told "Queued…" for a proposal that was sent (or already approved).
Suggested fix: Look the item up by its outbox id after the run and check state.sent / outbox for it, not only this run's report.

constfile=awaitwriteProjectLearning(root(api),api.state.path.directory,!summary.learning)api.ui.toast({variant: "success",message: `Learning ${summary.learning ? "off" : "on"} for you in this project (${file}). Applies from the next session.`,duration: 8000})
The toggle is labelled per person ("Turn learning off for me", line 275) but writes learn.capture into the project config file (learn/project-config.ts:60-75: opencode.json* / .altimate-code/altimate-code.json), which is normally committed. Once committed it overrides the workspace's "Learn from sessions" for every teammate, and learning_source then reports a teammate's value as "your setting".
Suggested fix: Store the personal override in user-level or git-ignored state (e.g. global config keyed by project, or learn/team/rules.json), or relabel it as a project setting and warn that it's shared through git.
When the session-start pull is the one that receives my_reviewed, the notices are stored and marked seen, but this path publishes neither rule.proposal.reviewed nor altimate.rules.changed (only RulesService.syncNow calls publishNotices, service.ts:70-74). The approve/reject toast and the sidebar count are delayed until the next background refresh, up to 5 minutes.
Suggested fix: Route session start through RulesService.syncNow, or call publishNotices with the report when this path starts the run.

Fixed. Since 0d4bfd4, the run that pulls announces its notices and the change event once, whoever waits on it (joinOrStart): session start, the timer, Sync now, a card reply or learn sync.

construle=RULE_CUES.find(([pattern])=>pattern.test(text))if(rule&&!memory)return{kind: "rule",cue: rule[1]}if(memory&&!rule)return{kind: "memory",cue: memory[1]}// "As a rule" wins over a stray preference cue: the person named the kind.if(rule?.[1]==="asked for a rule")return{kind: "rule",cue: rule[1]}
find returns the first matching cue and the "as a rule" cue is last in RULE_CUES. Text like "I prefer, as a rule, to always use CTEs" matches the always/never cue first, so the check on line 39 fails and it's classified ambiguous, contrary to the comment.
Suggested fix: Test the "as a rule" cue on its own, e.g. if (memory && RULE_CUES.some(([p, c]) => c === "asked for a rule" && p.test(text))) return { kind: "rule", cue: "asked for a rule" }.
The TUI menu, notice replay (line 234) and sidebar read rules.json straight from the local filesystem, and the toggle writes project config locally. The new GET /altimate/rules/summary and GET /altimate/rules routes have no client in this PR. When the TUI is attached to a server on another host/path it shows stale or empty state, contrary to the header comment that the plugin "calls the same routes".
Suggested fix: Have the TUI use the server routes (it already uses raw(api).get), or drop the unused routes.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in c581406: the /rules menu, notice replay, sidebar line and the learning toggle all go through the server routes now; the TUI reads no rules files.

MINOR · testing
Only classification, the team mirror, outbox push, card actions and store helpers are covered. Not tested: pull (notice dedupe, cursor, unchanged short-circuit, reset on repo_not_bound), reportUsage (delta cap, baseline, retry/idempotency), RuleTrigger.afterTurn (edit vs recurrence paths, HELPFUL suppressing the card), training_save routing in a linked project, and the four new server routes.
Suggested fix: Add unit tests with a stubbed RulesApi for pull/usage/afterTurn and route tests for /altimate/rules/sync (including credential redaction) and /rule-proposal.
Security and data loss
- `POST /altimate/rules/sync` no longer returns the captured credential: routes and `learn sync --json` share `RulesSync.publicReport`.
- A 401/407 or an uncoded 403 is `auth`, a malformed answer and an uncoded 400 are `transient`: those proposals stay queued. Only a coded 400/422 refuses an item. A 412 is a `conflict`.
- Losing the link or access (`workspace_not_found`, `repo_not_bound`, `forbidden`) clears the pulled rules but keeps confirmed proposals for the same scope; `adopt` still drops them on a scope change. At most 200 proposals queue; a new one is refused, never an old one dropped.
- Usage reports are saved (`pending_usage`) before sending and resent with the same `batch_id` until acknowledged, in batches of at most 500 rules.
- One invalid rule in a pull is skipped instead of failing the pull.
- `learn bootstrap` / `import-reviews` list the rules they staged and ask before sending them to the named workspace (default no); `--send-for-review` for scripts, `--yes` no longer implies it. Rules are linted and `applies_to` capped first; they are offered even after a partial failure; the candidate is discarded only when everything was sent; the snapshot is skipped for `--dry-run`.
- `team` is reserved: learn's writers refuse it, and an existing `team` playbook is set aside before the first mirror write.
Cards
- A refused proposal keeps its card open (it can be edited and sent again); outcomes are looked up by outbox id, so a proposal another run already sent is not reported as queued. "Approve now" that fails after storing says "sent for review, approving failed".
- `why` is redacted, and neutral when the workspace keeps quotes out.
- Retire cards have no edit and are never flagged; edits take the busy guard.
- Cards expire after a day, are dropped with their instance, and can be listed per session. Reply failures carry a `code` and an HTTP status (404 / 409 / 422 / 500).
- "Approve anyway" in the TUI takes a second Enter.
Learning setting and the TUI
- One `learningDecision` (kill switch, `ALTIMATE_LEARN_CAPTURE`, the person's choice, `learn.capture`, the workspace) with `learn/config.ts`'s flag parsing; summaries report who decided.
- "Turn learning on/off for me" is per person: `POST /altimate/rules/learning` stores it in the git-ignored rules state, not the committed project config; "Follow the workspace setting again" clears it; capture follows at once.
- The TUI's `/rules` menu, notice replay and sidebar line use the server routes (`/altimate/rules/summary`, `/altimate/rules`), never local files; routes use the same effective learn config as the service. First-run notices are recorded quietly.
- The service always registers its timer and idle hook (turning learning on needs no restart) and pulls only for linked projects.
- Session start: only a session's first prompt waits, and only on a pull already running or for a project with rules state; notices it brings are published.
Smaller
- Classifier: "as a rule" and "for me" decide on their own; a bare "I want / I like" or "we use" no longer files a convention as memory or a fact as a rule; "my_schema" is a project fact.
- Processed signals are not wiped when the signals file can't be read; `stopCapture` is shared by concurrent callers.
- `training_save` in a linked project only proposes when rules are enabled; its message names the right action for owners.
- `learn sync --json` exits non-zero when the pull fails.
- `learn.ts` reuses `project-config.ts`'s config discovery; the duplicate writer is gone.
- Docs: inline card keys, the personal setting, consent for history imports.
Deferred: rendering cards for headless runs and the IDE extension (cards that no client can show).
Tests: 20 new (API error classes, lenient pull, reset keeping the outbox, notice cursor, usage batch resend, credential redaction, learning decision, classifier, retire cards, neutral why, card lifecycle and reply codes, reserved store, staged rules).

Cards are shown by the TUI and by the VS Code extension from its release with workspace rules, for the project they have open and in its top-level sessions. Where no attached client shows cards (`altimate-code run`, ACP, the Claude Code and Codex plugins, a subagent's session), the save tools work as they did before cards: a rule is saved for this project only, a memory to memory, and nothing goes to review; corrections are kept, and proposed the next time that session runs with a client that shows cards. A card you asked for ("remember …") that closes without an answer (after a day, or when the session server stops, or, if it exits unexpectedly, when the project is next opened) is saved the same way, so the request is not lost (unless that project's memory or training is already full: then it is logged).
The card never blocks the input: keep typing, and answer it when you are ready. **ctrl+x p** (or a click) moves to the card; **←/→** choose an action, **Enter** confirms, **Esc** goes back to typing. The card shows the proposed wording, what it applies to, why it was proposed and your correction. **Enter** on the first action sends it to the owner's review queue (the workspace's **Rules** tab); for the workspace owner, it approves it at once instead. **e** edits the wording, and **n** ("Not a rule") stops a correction's topic from being proposed for 30 days (on a card you asked for, it just closes it). A rule that tells the agent to skip or disable verification (tests, CI, hooks, `--no-verify`) is flagged: the owner approves it with "Approve anyway", which takes a second Enter (a client answers it with the `approve_anyway` action; a plain `approve` is refused). Nothing reaches a teammate until it is approved. A card nobody answers is dropped when the session server stops, and after a day. A card is answered for the account and workspace it was shown for: after signing in as someone else or relinking, it closes without sending anything.

Kept as is. training_save itself checks the count and saves in two steps (training-save.ts), so the fallback matches the tool it stands in for. Making the limit atomic means changing TrainingStore, which is outside this PR.

WARNING:usage_reported is now unbounded.
The cap was removed because the list is "pruned to sessions that still have a snapshot", but .sessions/*.json files are never deleted (only Delivery.prepare writes them; nothing removes them), so reportUsage's prune keeps every pair of every session that ever showed a rule — and once state.rules is empty, reportUsage returns at if (!state.rules.length) return before the prune, so nothing is removed at all. rules.json is rewritten in full by every RulesStore.update (each card reply, dismissal, remember request and sync) inside the learn lock, and reportUsage builds a Set of the whole list every run; on a long-lived project both grow without bound. Prune to the sessions still readable by readShownSnapshots (the newest N snapshot names), or keep a cap that only trims pairs whose snapshot is gone.

WARNING: A recovered request is deleted even when saving it failed for a transient reason.
saveAsBefore throws for the documented permanent case (project training/memory full, or too large) but also for lock, I/O and store failures. The .catch above only logs, then the record is removed unconditionally, so the request that survived a crash is gone with a log line — contradicting "so the request is not lost" in docs/docs/configure/learn.md. The removal RulesStore.update is also outside that catch: if it throws (learn lock busy for 5 s, I/O), the loop aborts and the remaining claimed records keep pid = process.pid, so this process never treats them as orphaned again. Consider keeping the record when the failure is not the known permanent limit, and wrapping the removal so one failure does not abort the rest.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. A recovered request is removed only after a successful save or a SaveRefusedError; after any other failure it stays, claimed by this boot, for the next start. The removal is guarded so the loop continues. Test: "a request whose save fails for a passing reason is kept for the next start".

SUGGESTION:entered cannot tell a push-lock timeout from a real lock-acquisition failure.
Flock.acquire rejects before the callback runs for EACCES/ENOSPC on the lock directory, mkdir failures, and its "Lock acquired but … already existed (possible compromise)" errors. All of those now fall into this branch and are returned as the benign "another sync is still sending proposals", so the real error is swallowed and the queued proposals never move. Match the push lock's timeout message instead (as record does with LEARN_LOCK_BUSY) and rethrow everything else.
The reason will be displayed to describe this comment to others. Learn more.
SUGGESTION:room comes from a read outside the lock that enqueueMany then takes: a concurrent learn sync, server push or another CLI can fill the outbox in between, and enqueueMany's MAX_OUTBOX refusal is rethrown below (only ScopeChangedError is caught). The CLI then fails with an internal "already waiting" error instead of reporting these items as held. Computing room inside the locked write, or catching the cap error and treating those items as held, keeps the documented behavior.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. enqueueMany(..., { upTo: true }) decides what fits inside the same locked write and returns the ids it queued. The rest are reported as held.

SUGGESTION: This lookup duplicates the lazy subagent check added to remember.ts (proposeTeamRule's subagent closure): both import Session/SessionID, call Session.get, and default an unknown session to false. Exporting one helper (or reusing it from here) keeps the two from drifting when the lookup or its fallback changes.
Ran a peer review with a lead and two independent reviewer sessions, plus one convergence round. Both reviewers agreed with the result; one asked only for labelling fixes, which are included.
Earlier blocking items are fixed and checked in the code. The outbox is no longer dropped after uncoded 400/422 errors. The scope is tied to user:<id>, so a rotated key keeps the queue. A committed project file can no longer override managed or org config, and capture_policy is stripped from files. The mirror lives in a reserved workspace-rules store. The renderer lease is per directory. Pushes are serialized with the rules-push lock. "Approve now" is recorded before the review call and retried idempotently (alreadyActive). A pull's answer is applied only to the scope and revision it was for. Server and model text goes through printable.
No CRITICAL or MAJOR issues. The 4 MINOR findings below are inline. None blocks merge, but please address them or reply on each thread. Nits are in a separate comment.
Missing tests
The after-turn shown-rule path: edit and retire cards, and HELPFUL (covered) suppressing the card.
recoverRequests with a saved pid equal to process.pid, or a reused PID.
The training_save → memory redirect when altimate_memory_write isn't available (e.g. analyst).
A sync response with one malformed my_reviewed row.
A card reply after rules were turned off or the user signed out.
learnKeysAboveProject for each layer. Only inline config is tested; managed dir, MDM, OPENCODE_CONFIG_DIR and the home config dirs are not.
Unintended consequences
learn status text:Capture: is now Learning from sessions: … (source), and "Lessons" is now "Rules". Scripts that parse the text will break; the JSON is unchanged. (Previously noted.)
Save-tool results: in a linked project with a TUI attached, training_save and altimate_memory_write return new metadata.action values: proposed, redirect and error.
Telemetry: a rules_command event now fires for every learn subcommand.
Notices: when two instances share a root, a joined sync announces notices only on the starting instance's Bus.
Scope vs ticket: can't be checked, because the PR links no Jira key or GitHub issue.
A saved "remember" request counts as orphaned only when its PID is dead and not ours. There is no age limit, and recovery runs only at service start. PIDs get reused, so:
Container restart: a server restarted in a container often gets the same PID (e.g. PID 1). The crashed process's requests carry process.pid, so they are never recovered, and the new process has no card for them.
Desktop: an unrelated live process can take over the dead PID, so the request looks "still shown" forever.
The agent was told not to save it again, so the "remember" is lost. It stays in rules.json until the 50-entry cap pushes it out.
Fix: store a per-process boot id (randomUUID() at module load) next to pid. Treat a request as orphaned when its boot id isn't ours, or when it is older than CARD_TTL_MS. Add a test with pid: process.pid and a foreign boot id.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9, as you suggested. There is a boot id beside the PID. A request is orphaned when its boot id isn't ours and its process is gone, its PID is ours, or it is older than CARD_TTL_MS. The test covers pid: process.pid with a foreign boot id.

MINOR (regression)
if(result.status==="memory")return{title: "Not a team rule",metadata: { ...metadata,action: "redirect"},output: `This reads as ${result.cue}, not a team rule. Save it with altimate_memory_write instead ...`,
In a linked project, training_save content that classifies as memory saves nothing and tells the agent to call altimate_memory_write. The analyst agent allows training_save (agent/agent.ts:354) but denies altimate_memory_write (agent/agent.ts:661). Config or the per-request tool filter can remove the memory tool too.
When the agent can't call it, a request like "remember: I prefer CTEs" is dropped. Before this PR it was saved as project training.
Fix: when the memory tool isn't available to the calling agent, write the memory directly (MemoryStore.write) or fall back to the old training save. Add a test for the redirect with the memory tool unavailable.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. training_save content that reads as a memory is saved to memory directly (scope from memoryScope), and the agent is told it was saved, so it no longer depends on the memory tool being available. Test: "saved to memory, not redirected to a tool the agent may not have".
rules are parsed one at a time and an invalid rule is skipped. The rest of SyncResponse is parsed in one strict pass:
repo_identity is a required string here, but nullable in RuleOut.
my_reviewed uses the strict Reviewed schema, which requires updated_at, rule_key and change.
One review row the client can't parse (e.g. a null updated_at), or a null repo_identity, throws a ZodError that is classified transient. Every pull then fails again each 5 minutes, and approved rules stop updating for the repository. I haven't checked the backend's nullability for these fields.
Fix: parse my_reviewed row by row and skip bad rows, as for rules. Make repo_identity.nullable().optional(), since the client doesn't use it.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. my_reviewed is parsed row by row, skipping bad rows, and repo_identity is .nullable().optional(). Test: "one review row or field this version cannot read … does not fail the pull".

MINOR (misleading message) — previously raised, still open
if(sent.status==="queued"){message=sent.error
? `Not sent yet (${sent.error}): kept queued and sent again on the next sync.`
: `Queued: it will be sent to ${owner}'s review queue when the workspace is reachable.`
Cards are now gated when shown, but a card already on screen can still be answered after learning or workspaces are turned off, after sign-out, or after the link is lost. runFresh then returns context.status !== "ready" with no last_error, so the person sees "will be sent … when the workspace is reachable". With learning off, nothing will ever send it. This is the earlier "make the off/skipped message accurate" thread, narrowed but not closed.
Fix: when report?.context.status !== "ready", word the message from report.context.reason, as from-history.ts does for its why.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. When the run's context is not ready, the card says "Kept queued: rules sync is not available (); it is sent on a later sync once it is." Test: "a card sent while rules sync is unavailable says so".
packages/opencode/src/altimate/rules/sync.ts:299-315: the "another sender" branch now depends only on entered. Any Flock.withLock failure before the callback runs, such as a filesystem error creating the lock (not only the timeout), is reported as "another sync is still sending proposals". Keep the lock-timeout message check for that branch and rethrow other errors.
rules/service.ts:57-62, rules/store.ts:335-344: after an unlink, reset({ keepOutbox: true }) keeps scope, so linked() stays true and rules.json stays on disk. The 5-minute refresh then keeps resolving context, and mayHaveWorkspaceRules in prompt.ts stays true. Consider keeping scope only while the outbox or pending_approvals is non-empty.
rules/trigger.ts:206-209: EDIT_OVERLAP is an absolute count of 3 shared words. Two short, generic dbt rules (e.g. "model", "staging", "test") can match each other, which sends the drafting call to the wrong rule. A share of the rule's own content words would be more robust.
docs/docs/configure/learn.md:198 says cards are shown "by the VS Code extension from its release with workspace rules", but the PR says the IDE renderer is not included. The PR body also still gives the mirror path as .altimate-code/learn/team/; it is now workspace-rules/. (Previously raised.)
test/altimate/rules/rules.test.ts:1110,1117: describe("the team store is reserved") and "a playbook someone already named team is set aside" now test WORKSPACE_RULES_STORE. A team store is deliberately left alone. Rename them to say workspace-rules.
The "lesson" → "rule" rewording of existing learn messages, docs and tests is a separate change mixed into this PR. Calling it out in the description would help reviewers. (Previously noted.)

Peer review — request changes
Reviewed by two independent reviewers (Claude and Sol 6.1), reconciled in one convergence round.
The feature is carefully built. Highlights:
per-scope credential capture, with publicReport stripping the credential before it reaches clients;
adopt dropping another scope's queue and telling the person (scope_drop);
capture gating that fails closed;
the renderer lease, so headless run / ACP behave as before;
the reserved workspace-rules store;
broad tests (118 in rules.test.ts).
Blocking (MAJOR): a failed fallback save deletes the durable "remember" request, so content the person asked to keep is lost (remember.ts:144-148, proposal.ts:116-125).
Non-blocking (MINOR), inline: usage reporting has no scope-ownership check; pulled rule text is not linted before it reaches the prompt; the 50-request cap evicts records of open cards; disabling workspaces doesn't clear cached session rules; enabling capture mid-instance never starts it; usage_reported grows without bound; afterTurn per-turn I/O.
Missing tests:
a transient save failure in recovery and on unanswered close;
more than 50 outstanding remember requests;
scope adoption during a usage report;
resuming the same session after disabling workspaces;
live capture off → on;
prompt.tsmayHaveWorkspaceRules and the session-start wait;
rules.tsx lease and dispose;
a pulled rule with non-normalized or multi-line text.
Scope: cannot verify. No Jira key or linked issue; please link one.
Nits (consolidated)
server.ts L1264-1398: all seven new routes repeat the workspaceRouteRefusal guard, the try/catch/log/500 block and Instance.worktree !== "/" ? Instance.worktree : Instance.directory. That root expression also appears in prompt.ts, remember.ts and service.ts. A small route helper and a learnRoot() helper would remove it.
The "lesson" → "rule" wording change in existing learn messages (learn/store.ts, auto.ts, recurrence.ts) and their test assertions adds churn unrelated to workspace sync. Consider a separate PR, or call it out in the description.
The PR description says rules.test.ts has 20 tests; it has 118.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (data) — a failed fallback save permanently deletes the remember request
awaitsaveAsBefore(r).catch((error)=>log.warn("recovered remember request not saved",{card: r.card, error }))awaitRulesStore.update(root,(s)=>{s.remember_requests=s.remember_requests.filter((x)=>x.card!==r.card)})
The durable request exists to survive failures, but after anysaveAsBefore error it is removed anyway. MemoryStore.write / TrainingStore.save do fallible writes and renames (memory/store.ts:299-307). A transient failure (EBUSY, ENOSPC, lock timeout) therefore loses content the person explicitly asked to keep. The only trace is a log line.
Suggested fix: remove the request only after the save succeeds, or after a failure known to be permanent (the per-kind limit). For anything else, keep the record and reset pid so the next start retries it.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. Recovery removes a request only after a successful save, or after SaveRefusedError (a limit the tool enforces). Any other failure keeps it for the next start (claimed by this boot, so a later boot retries). Test included.
The reason will be displayed to describe this comment to others. Learn more.
MAJOR (data) — same loss path for a card that closes unanswered
constsaved=entry.unanswered
? awaitentry.unanswered(entry.card.text).then(()=>true,(error)=>{ ... returnfalse})
: undefined// Saved, or failed for good (the project's memory or training is full): either way nothing is left to recover.awaitforgetRequest(id,entry)
The comment assumes every failure is a capacity failure, but unanswered can also fail transiently on filesystem errors. The persisted request is then cleared and the "remember" content is gone.
Suggested fix: call forgetRequest only when saved === true, or when the error is the capacity error (give the TRAINING_MAX_PATTERNS_PER_KIND throw in saveAsBefore a distinct error class). Otherwise leave the record for recoverRequests. This needs a test that makes the save throw once and checks that the request survives.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. settle clears the kept request only when the save succeeded or threw SaveRefusedError; the training and memory limits now throw that distinct class. Test: "a card whose fallback fails for a passing reason keeps its request; a refused save clears it".
pushLocked returns early when !ownedBy(state, ctx) (L321-323), but usage reporting never checks this. If another process adopts a different account or workspace while this run is in flight, it can build a new-scope pending_usage (and state.rules changes with the next pull). This run then sends that batch with the old scope's actAs. markReported (L584-589) then marks the pairs as reported, so they are never counted for the right workspace. adopt clearing pending_usage does not close the window: the new scope can build a batch after that.
Suggested fix: store the scope on pending_usage and check ownedBy(state, ctx) / sameScope(batch.scope, ctx.scope) when the batch is selected, sent and acknowledged. In the markReported update, skip when ownership changed.
Every other path into "Team rules" is linted: proposals (proposal.ts:315-319) and local promotes (validateCandidate). Workspace rules go toLesson → approved.json → lessonLine with only whitespace collapsed. A rule edited in the web UI with hidden or bidi characters, URLs, commands or very long text is injected verbatim into every teammate's prompt.
Suggested fix: also skip rules where lint(text, { grandfathered: true }) fails or normalizeText(text).trim() !== text, and count them in skipped. The revision reset in pull already handles skipped rules.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. Pulled rules must pass lint(text, { grandfathered: true }) and be normalized text, otherwise they are skipped and counted. The pull keeps no revision, and last_error says so. A multi-line rule is covered by the test.
ask / persistRequest admit any number of requested cards, but every write keeps only the newest 50. The 51st outstanding card drops the oldest card's record while that card stays open, so a crash then loses that request.
Suggested fix: check capacity when a request is persisted. When full, save without a card right away (the fallback path) instead of trimming a live record.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. Nothing evicts a record any more. persistRequest refuses past 50, and proposeFor saves a new request without a card when 50 are open (rule_remember_routed outcome requests_full). Test: "with 50 requested cards open, a new request is saved without a card instead of evicting one".
sectionChecked (L171-175) returns the cached state.section, and a repeated request returns the cached previous.note (L388-399). Both were built while workspaces were on. If the person restarts with workspaces disabled and resumes the same session, the team rules are still injected via session/prompt.ts.
Suggested fix: when workspaces are off, drop workspace-rules entries from cached sections and notes, or rebuild them. Add a test that resumes the same session ID (the existing test uses different sessions).
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. A session snapshot that was shown workspace-rules entries is treated like an unsafe one while workspaces are off: its cached section and notes are not served, and the next prepare rebuilds it without them. The test resumes the same session id.
The reason will be displayed to describe this comment to others. Learn more.
MINOR (logic) — turning learn.capture on mid-instance never starts capture
if(captureEnabled(learn))returnvoid(awaitgate())
This assumes bootstrap owns capture whenever captureEnabled is true. But bootstrap only starts it if capture was on at startup (project/bootstrap.ts:137-140). The header comment says the learn config is re-read on every refresh, so learn enable / a project-file change should take effect. Here it doesn't: the gate is installed, but nothing subscribes.
Suggested fix: track whether capture is actually running (e.g. a per-directory registry in capture.ts), and start it from here when the effective decision turns on and nothing is running.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9. capture.ts tracks the directories whose capture runs (captureRunning). When learning is on and nothing runs, the service starts capture, including after learn enable mid-instance, and never a second one beside bootstrap's. With learning off, a capture the service did not start is left to the gate. This path has no automated test: it needs a live instance with bootstrap.
Pairs are kept only while their session's snapshot file exists, but nothing ever deletes files under .altimate-code/learn/.sessions. The list therefore grows by sessions × rules for the life of the project. rules.json holds it and is rewritten under the learn lock on every update (each pull, card and dismissal), and snapshotFiles lstats every file every 5 minutes.
Suggested fix: bound it to the snapshots that are actually read (the newest 200 from readShownSnapshots), or store a per-session high-water mark instead of session|public_id strings.
The reason will be displayed to describe this comment to others. Learn more.
Fixed in f5a35a9: bounded by snapshot age (30 days after it was last written). Snapshots are never deleted, so a count of files would not have bounded it.
This parses up to 200 snapshot files to find one session's file. The same afterTurn also calls readSignals(...) twice (L172 and L239), and the first call runs on every session.idle in a linked project with learning on.
Suggested fix: read the one file by name (snapshotFile(sessionID) is already exported from delivery.ts), and reuse the first readSignals result for unmatchedCorrectionTopics.
"Remember" requests (the blocking finding):
- A request kept for a card is removed only once saved, or when the save can never succeed (`SaveRefusedError`: the project's memory or training is full, a too-long memory); after a passing failure it stays and the next start saves it, on unanswered close and in recovery alike
- Requests carry a per-process boot id: a request is this process's own only with its boot id, so a PID reused by a restart or another process no longer hides it; one older than a card can live is recovered too
- No live request is evicted: with 50 requested cards open, a new request is saved without a card (`requests_full`); clearing an answered request is retried; recovery removal failures no longer stop the loop
- `training_save` content that reads as a memory is saved to memory directly (an agent such as `analyst` may not have the memory tool)
Sync:
- Only the push lock's own timeout reads as "another sync is still sending"; any other failure to take it fails the run
- Usage reporting checks scope ownership like push and pull: a batch built for another scope is neither sent nor marked reported
- Reported usage pairs are kept for 30 days after their session's snapshot was last written (snapshots are never deleted)
- A review row or `repo_identity` this version cannot read no longer fails the pull; pulled rules must pass the rule lint and be normalized text, else they are skipped and counted
- Bootstrap decides what fits in the outbox inside the same locked write; held rules are said not to be offered again
Cards, delivery and capture:
- A card answered after its day is closed (a requested one saved), not answered; a card sent while rules sync is unavailable says why instead of promising a later send
- A resumed session that was shown team rules is rebuilt without them while workspaces are off
- `learn enable` mid-instance starts capture (never a second one beside a running capture)
- The after-turn check reads its session's snapshot by name and the signals once; its `no_renderer` skip keeps its path
Cleanups: the kill-switch parse is shared with the core flag (`killSwitchSet`), `prompt.ts` reuses `canonicalDirectory` and stamps the bindings cache with inode and ctime, recurrence loads stores through one helper, the subagent lookup is shared, 412 errors keep their detail, renames (`saveWithoutCard`, `processAlive`, `triggerDeps`), `rule_drafted` in the telemetry completeness test, docs.
Each fix has a test that fails without it.

Thanks for both passes. Everything below is in f5a35a9205; the inline threads each have their own reply.
Blocking item (a failed fallback save deletes the request): fixed on both paths. A kept request is removed only after a successful save, or when the save can never succeed (SaveRefusedError). Requests also carry a per-process boot id, so a reused PID no longer hides them.
Missing tests, now added:
the after-turn shown-rule path: edit, retire, and HELPFUL → covered;
recovery with pid: process.pid and a foreign boot id, plus a transient save failure in recovery and on unanswered close;
more than 50 open requests;
a sync response with an unreadable my_reviewed row, a null repo_identity, and a multi-line pulled rule;
a card reply after rules sync became unavailable;
usage built for another scope;
resuming the same session with workspaces off.
Still not covered by an automated test:
learnKeysAboveProject per layer: inline config and project-only are tested; the managed dir, MDM, OPENCODE_CONFIG_DIR and home dirs are not, because they need writing to the real home or managed paths;
live capture off → on: it needs a bootstrapped instance;
mayHaveWorkspaceRules and the session-start wait;
the rules.tsx lease and dispose.
Unintended consequences: agreed on all four. The learn status text change is called out in the PR description (the JSON is unchanged). The new metadata.action values on the save tools are proposed, redirect (now only when no memory save is available), memory and error. rules_command is intentional. Notices are announced once, on the instance whose run pulled them.
The PR description's test count is corrected (129 in rules.test.ts).
I left the route helper and a learnRoot() helper for a follow-up, to keep this PR's diff from growing further. The same goes for the "lesson" → "rule" wording; the PR description calls it out.
Comment trimming will happen as those files are next touched.

[WARNING]: Age-based pruning makes the same usage re-reported on every sync
live() drops a pair once its session's snapshot mtime is older than USAGE_PAIR_MS, but the applied-count loop below still reads that same snapshot: readShownSnapshots has no age filter (it takes the 200 newest by mtime, and a project with 200 or fewer sessions includes all of them). So for a session whose snapshot was last written more than 30 days ago, each reportUsage prunes its pair, recounts it as newly applied, and sends it again, up to MAX_USAGE_DELTA (50) per rule per run, i.e. on every 5-minute refresh. A dormant project that is reopened keeps inflating the workspace's "applied" counts until its snapshots are touched or fall out of the newest 200. Make eviction and recounting agree: keep the old existence-based prune, or skip snapshots older than USAGE_PAIR_MS in the loop below (the snapshotFiles mtimes are already available).
The reason will be displayed to describe this comment to others. Learn more.
[WARNING]:running is set before setup can fail, so one failed start blocks capture for the process
This runs before registerDisposer and the two subscribeWhenReady calls, and the catch below returns undefined without removing it. If a subscription throws (the bus is still building, the exact case subscribeWhenReady exists for), captureRunning keeps reporting true for the directory, so followLearning in service.ts takes the running branch, only installs the gate, and never starts capture again: correction capture stays silently dead until the instance is disposed, where before it was retried on the next refresh. Add to running only after the subscriptions succeed, or clean up running (and active) in the catch.
The reason will be displayed to describe this comment to others. Learn more.
[SUGGESTION]: The snapshot is read twice per after-turn check
read(root, file) fully reads the file just to test for ENOENT, and readSnapshotFile reads it again. This runs on every session idle check. Have readSnapshotFile return undefined when read yields nothing (or pass the text in), so the file is read once.
The reason will be displayed to describe this comment to others. Learn more.
[SUGGESTION]:.sessions is scanned twice per usage report
snapshotFiles above already did a readdir plus an lstat per file; readShownSnapshots repeats both (and parses up to 200 files) a few lines later, on every 5-minute refresh of every open project. The mtimes are already in hand, so pass them in (or reuse one scan) instead of walking the directory again.

Invariants this round enforces, each with a test that fails without it:
- A "remember" request leaves disk only with its outcome: a dismissal or a queued send ends it in the same write; a local save ends it after the save; a send the workspace refuses keeps it again (while still busy, so the card cannot close mid-write); an answered card whose request cannot be removed is removed by recovery, never saved
- A requested card is shown only once its request is on disk; otherwise there is no card and the tool saves without one ("asked again" likewise, changing the card only after the write)
- A claim renews the request's time, and this process renews its own open cards, so no live process's request is taken as ownerless; claims are marked inside the locked write
- This process retries its own leftover requests on every refresh, never while their card is open, settling or answered
- Usage counts a session within 30 days of its snapshot and remembers its reported pairs for 90, so nothing countable is forgotten and old sessions are not recounted every sync
- A usage batch is sent only for the run's own scope, including one another process saved mid-run
- A resumed session keeps a team rule only while workspaces are on and the rule is still in the mirror (an edit keeps its frozen wording; an unreadable mirror keeps the snapshot)
- A send that is queued (decided from disk when the call threw after its write) drops the card's local fallback, so no second copy is saved on close
Also: privacy docs describe what workspace proposals send; the memory destination of `training_save` is documented; staged rules with the same wording are proposed once; one read per session snapshot.

Nothing reaches a teammate until it is approved. A card nobody answers is dropped when the session server stops, and after a day (an answer after that closes it instead). A card is answered for the account and workspace it was shown for: after signing in as someone else or relinking, it closes without sending anything.
**Rule or memory.** A personal preference ("I prefer CTEs") or a fact ("our warehouse is Snowflake") is saved to memory without review. When the agent saved it with `training_save`, a preference goes to your own memory (all your projects) and a fact to this project's memory (outside a linked project, `training_save` saves project training as before). When it saved it with the memory tool, it stays in that tool's project memory. When the wording does not say which, the card asks: Memory, Team rule, or Skip.

P2: The added 'Rule or memory' paragraph overstates where routing happens. training_save sends a preference or fact to memory only when a card-capable client is attached to a linked project; under run, ACP, or a subagent it still saves a project rule, as the sentence before this says for cards. For the memory tool, the text is saved in whatever scope the agent chose (global or project), so "project memory" is wrong for global saves. Reword both clauses.

**Where they live.** Approved team rules are mirrored into the `workspace-rules` store (`.altimate-code/learn/workspace-rules/`), which is git-ignored and written only by sync: the name is reserved for it, and a store you create needs another `--name`. Your other stores, including one named `team`, are not touched. Team rules stop being delivered when the project is unlinked, when you sign out, when the workspace refuses access, and while workspaces are turned off (`ALTIMATE_DISABLE_WORKSPACE`); they come back with the next sync.

SUGGESTION:settling and answered are module-local while pending is process-global
pending lives on a Symbol.for registry precisely because (per the comment above it) this module can be loaded through more than one import path in one process. settling (line 123) and answered (line 151) are plain module-level sets, so a second module instance would answer isOpen() from its own empty sets. In the window where reply() has already deleted the id from the shared pending and is still inside forgetRequest (up to ~1.2 s of retries) or settle(), recoverRequests would see this boot's record as orphaned and save the request a second time. Put these sets on the same Symbol.for registry as pending (or keep the flags on the Pending entry) so the guard is process-wide.

SUGGESTION:forget deletes the kept request even when upTo queued nothing
With options.upTo and a full outbox, items/ids are sliced to zero at lines 543-547, no outbox entry is added, yet this filter still removes the card's remember_request. act() calls enqueue without upTo and sendStaged passes no forget, so it is not reachable today, but a caller that combines the two would drop the request with nothing queued: the card would report success (ids[0] is undefined, so no outbox row is found) while the person's "remember" is gone. Apply the filter only when something was actually queued.

SUGGESTION: The team-rules mirror is read and parsed on every state() call
state() runs on the prompt path (section, prepare), and file()/compact() call it inside Store.transaction(..., LOCK_OPTIONS). Whenever the snapshot holds a team rule, each of those calls now does an fs.access + readFile + full JSON.parse of approved.json through mirroredIds(); the mirrored ??= cache only spans one state() call, so a turn that touches several files re-reads it each time (and while holding the learn lock). Consider caching the id set for a short window, or keyed on the mirror's mtime, the way canonicalDirectory is cached in session/prompt.ts.
Reviewed by two independent reviewers (Claude and Sol 6.1) plus a tail-sweep pass, reconciled in one convergence round.
The previous blocking item is fixed. A failed fallback save no longer deletes the durable "remember" request (proposal.ts:166-174, remember.ts:198-206, test at rules.test.ts:2197). Most of the earlier MINORs are fixed too: the scope check on usage, linting of pulled rules, the 50-request cap, delivery with workspaces off, and capture turned on mid-instance.
Blocking (MAJOR), inline: with ALTIMATE_DISABLE_WORKSPACE=1, the workspace's "Learn from sessions" still starts capture (trigger.ts:57).
Non-blocking (MINOR):
delivery.ts:226,245-252: when the mirror is unreadable or corrupt, cached team rules are kept. Normal revocation already rewrites the mirror, so the only case is a damaged file. Consider failing closed.
api.ts:86: ProposalResult.status is z.string(), so an unknown status counts as final (sync.ts:357). It is reported, not dropped silently. Consider listing the statuses #7514 can return and treating any other as transient.
classify.ts:56: memoryScope reads "I want" and "I like" as personal, while classify treats them as weak cues. Choosing Memory then writes a project rule to global memory.
sync.ts:656,687: usage counting uses the default 200-snapshot cap (delivery.ts:143). The comment at sync.ts:637 says the window bounds it.
api.ts:191-194: a my_reviewed row that fails to parse is dropped and not counted in skipped, but sync.ts:152 still advances the cursor, so that notice is lost for good.
sync.ts:121: on an account or workspace change, adopt without keepOutbox drops queued "remember" items, because the durable request was already removed at enqueue. An open card in the same situation is saved locally instead.
store.ts:202-209,257-266: State.parse is all-or-nothing. If an older build reads a newer enum value, it moves the file aside and starts empty, losing the outbox, remember_requests and usage_reported.
service.ts:124: if (disposed || starting) return drops a setting change that arrives while capture is starting.
service.ts:174-183: afterTurn still does file and DB reads every turn before checking whether learning is on (carried over from the last round).
sync.ts:151-158: a pull answered "unchanged" right after a concurrent reset can leave the rules empty until the server's revision changes.
Comments (lower confidence, please check):
service.ts:58-61 / prompt.ts:174-178: linked() and mayHaveWorkspaceRules only read the local binding cache, while context() also honours the IDE pin (resolvePinnedBinding). A project pinned in VS Code but never linked would never sync.
training-save.ts:76: training_save(kind=rule, scope=project) for a name that already exists becomes a new add proposal, before TrainingStore.get at line 82. The old local rule stays active and can contradict the approved team rule. memory-write.ts skips the card for updates (!existing).
Missing tests:
learningDecision / capture with ALTIMATE_DISABLE_WORKSPACE=1 and saved workspace settings;
prompt.tsmayHaveWorkspaceRules and the bounded session-start wait;
the rules.tsx renderer lease (renewal, back-off, dispose);
capture turned on mid-instance through followLearning;
a review row that fails to parse while the cursor advances;
more than 200 snapshots in usage counting;
a rule update through training_save for an existing name.
Nits
sync.ts:764: the doc comment says "Push, pull, then report usage", but runUntracked pushes, then reports usage, then pulls (sync.ts:793-797).
sync.ts:396-397: a duplicate resend answered pending increments pending_count again.
sync.ts:275,331-333: refusedWhole is keyed by the root the caller passed, not the canonical root like inflight / failedAt.
sync.ts:817-828: instances that share a worktree share one run, but only the directory that started it gets the notices.
Scope: cannot verify. There is still no linked Jira key or issue.

Full-file peer review at 8d83a1a: verified findings
All 55 changed files were split into five shards. Two independent reviewers (Claude and Sol 6.1) read every shard in full, and a lead checked each finding against the code at HEAD. One convergence round followed. Only findings that survived verification and are not already on this PR are posted inline. Nits are left out.
New, inline:
MAJOR:from-history.ts:136. The cleanup after learn bootstrap / import-reviews can delete candidate rules that another writer added during the confirmation prompt.
MINOR:
proposal.ts:115: a failed state write still shows the card as kept.
sync.ts:609: a late usage ack rolls corrected_reported back, so corrections are counted twice.
rules.tsx:424: the startup load() can hide a card that arrived during the request.
remember.ts:15: "don't forget" never gets a card.
remember.ts:80: "just for me" through training_save lands in project memory.
trigger.ts:244: a topic that was sent or saved is proposed again.
project-config.ts:51: raw {env:…} placeholders override the expanded learn config.
rules.tsx:562: the toggle toast says "(your setting)" when an env var decides.
proposal.ts:533: "Not a rule" discards a requested memory.
Two docs items in learn.md.
Follow-up on an existing thread: the 50-block limit fix covers only the memory branch of saveWithoutCard, and the training branch still retries forever. I replied on cubic's remember.ts:75 thread.
Checked and not posted:
A draft built for one workspace and proposed after relinking to another: the card names the new workspace and the person confirms it, so this is not a defect.
Unbounded growth of signals.jsonl under workspace-started capture: low confidence, and a non-blocking note only. This PR does turn capture on for members who never opted in, and nothing prunes signals in that mode.
A delayed fallback overwriting a newer memory block with the same id: confidence 0.55.
The blocking item from the previous review (trigger.ts:57, kill switch) is still open.

MINOR (data).kept = true is set inside the RulesStore.update callback. That callback runs before the state file is written (Store.writeAtomic, store.ts:275), so if that write fails (ENOSPC, EIO), the .catch logs the error but kept is already true. entry.persisted is set, ask publishes the card (:350-353), and the tool stands down, all with no request on disk.
This contradicts the comment above it: "Not kept (full, or the write failed): the caller saves without a card". If the process then exits before the card closes, recovery has nothing to find (remember.tsrecoverRequests reads only persisted requests), and the "remember" is lost.
Suggested fix: set kept only after update resolves, from a flag the callback sets, for example let added = false; await update(... added = true ...); kept = added, and leave it false in the catch. If a mirror write fails after the state was committed, re-read the state to check whether the record landed.
The reason will be displayed to describe this comment to others. Learn more.
MINOR (data / usage counts).corrected_reported is merged from the batch unconditionally. Only clearing pending_usage checks batch_id, so a late acknowledgement of an older batch rolls the baseline back.
How it happens with two processes on one project (the TUI's session server and learn sync, or two sessions); the in-process inflight dedupe does not cover this:
Both send the same pending batch A (reportUsage → sendUsage(before.pending_usage)).
Process 2's ack of A arrives first. It then builds and acks batch B, which moves corrected_reported[key] past A's value.
Process 1's delayed ack of A runs markReported(A), and this line writes A's lower value back.
On its next build, baseline = Math.min(corrected_reported, recurred) (:694) is lower again, so the same corrections are sent again under a new batch_id. The server dedupes only by batch id, so "corrected again" is counted twice.
Suggested fix: make the merge monotonic, Math.max(existing, batch) per key. Alternatively, skip the counter update unless s.pending_usage?.batch_id === batch.batch_id (pairs are a set union, so they are safe either way). Please add an out-of-order ack test.

MINOR (UI state).load() replaces the card list with the HTTP snapshot. If rule.proposal.asked (or updated) arrives while this GET is in flight, offer() adds the card (:352-353), and then this line overwrites the list with the older snapshot. The card stays open on the server but is not shown in this TUI. load() runs only once (the 4 s startup timer, :472-476), so nothing brings it back until the card is asked again.
The window is one local round trip at startup, which is why this is MINOR. Suggested fix: merge rather than replace. Keep the cards that arrived by event during the request, and drop only the ids the snapshot no longer has and that were not offered since the request started.
The reason will be displayed to describe this comment to others. Learn more.
MINOR (logic).ASKED_TO_REMEMBER includes "don't forget" and "do not forget", but NEGATION matches the same "don't" and "do not". So restates(content, said) returns false whenever the person phrases a request that way and the rule itself is positive.
Example: the person writes "Don't forget: always prefix staging models with stg_", and the agent calls altimate_memory_write(scope: "project", content: "Always prefix staging models with stg_").
The negation check fails (said has one, content does not), so proposeFor returns undefined at :238.
The rule is written straight to project memory with no card. The comment at :267-269 says that outcome must not happen: it "would reach teammates through the workspace's memory without review".
Any message with a stray "no" or "not" elsewhere ("…, no exceptions") is affected the same way.
Suggested fix: remove the trigger phrase from said before the polarity check (said.replace(ASKED_TO_REMEMBER, " ")). Add a "Don't forget: always …" test; the current one covers only "Remember:".
The reason will be displayed to describe this comment to others. Learn more.
MINOR (config). The project file's learn is parsed raw here, without the config loader's {env:…} / {file:…} substitution (config/paths.ts). RulesService.effectiveLearn (service.ts:41-45) then copies each raw key over the already-expanded loaded config.
Example: a committed "learn": { "model": "{env:LEARN_MODEL}" }.
learnConfig returns the literal "{env:LEARN_MODEL}", which reaches Provider.parseModel through triggerDeps (service.ts:77, :183).
Every drafting call fails. afterTurn's catch resets checked, so the corrections are retried and fail again on every turn, and no card ever appears.
The same value reaches remember.ts:65 and the rules routes.
This is the project-file read, not the higherLearn helper cubic flagged earlier for managed config / OPENCODE_CONFIG_CONTENT. That helper is gone, but this read is still raw.
Suggested fix: run the text through the loader's substitution before parse, or take from disk only the keys learn enable / learn disable write (capture, enabled, auto_reflect).

MINOR (user-visible text is wrong).learningDecision returns source: "you" for the kill switch (ALTIMATE_LEARN=0 / learn.enabled=false, trigger.ts:51) and for ALTIMATE_LEARN_CAPTURE (:52-53). This handler special-cases only "organization".
With ALTIMATE_LEARN=0:
/rules → "Turn learning on for me" writes personal_learning = true.
The summary comes back learning: false, learning_source: "you".
The person gets a green toast: "Learning from sessions is off for you in this project (your setting)".
They asked for on, are told it is off, and are told their own setting decided that. ALTIMATE_LEARN_CAPTURE=1 with "Turn learning off for me" fails the same way, and the menu's status line also says "(your setting)" in both cases. A test pins the source: "you" result for the kill switch (rules.test.ts:1136).
Suggested fix: give the kill switch and the env override their own source (for example "environment"), map it in sourceLabel and learn status, and show a warning here whenever next.learning !== on.

MINOR (docs). "All subcommands take --name <store> … except enable, disable, and nudge off": the new learn sync has no --name (cli/cmd/learn.ts:448-450, only --json), and the CLI is .strict() (src/index.ts:276). So altimate-code learn sync --name team-playbook fails with "Unknown argument: name". Please add sync to the exceptions. A ### sync section documenting --json and its exit status would also help, since the command is otherwise only mentioned in passing (:206, :210).
**After review.** Approved rules reach new sessions, and new requests and files in running ones, within a few minutes (a pull at session start and every 5 minutes; `/rules` → Sync now, or `altimate-code learn sync`). A session keeps the wording it was already shown. You see a notice when the owner approves or rejects your rule, with the reason. Usage is reported back to the workspace: how many sessions a rule was applied in, and how often you corrected the agent on its topic after it was shown ("corrected again").
**Learning on or off.** In order: the learning kill switch (`learn.enabled`, `ALTIMATE_LEARN`), `ALTIMATE_LEARN_CAPTURE`, a `learn.capture` your organization sets (managed config, MDM, the account's config, `OPENCODE_CONFIG_CONTENT`, `OPENCODE_CONFIG_DIR` or the home `~/.altimate-code` config; shown as "organization policy"), your `/rules` → "Turn learning on/off for me" choice, `learn.capture` (`learn enable` / `learn disable`), then the workspace's "Learn from sessions". The `/rules` choice is yours alone: it is kept in the project's git-ignored rules state, not in the project config, so it never reaches a teammate through a commit ("Follow the workspace setting again" clears it). MINOR (docs). This ranks the /rules choice above learn.capture (learn enable / learn disable) and says only "Follow the workspace setting again" clears it. In fact a successful learn enable or learn disable deletes it (clearPersonalLearning, cli/cmd/learn.ts:220, :273). A reader would expect learn enable not to override their /rules "off", but it silently does. Please say here, and in the enable/disable section, that learn enable / learn disable replace an earlier /rules choice for this project.

MINOR (data). On a card raised by a "remember" through the memory tool, which classify(said) reads as a rule (not a choose card), build offers [team action, "edit", "dismiss"], with no way to keep it as the memory the agent was about to write (:262-264). The agent has already been told "Nothing was saved yet … Do not save it again with altimate_memory_write" (remember.ts:301).
"Not a rule" (rules.tsx:93) then deletes the kept request in the same write (this line), so no fallback runs. The two outcomes contradict each other:
Leaving the card unanswered saves the memory (the unanswered fallback).
Answering "Not a rule", which reads as "this isn't a team rule", discards it.
Before this PR, the same altimate_memory_write simply saved it. Suggested fix: offer save_memory on cards that carry a memory request (entry.memory), or have dismiss on such a card run the unanswered fallback, with the label and message to match.
- `learningDecision` uses a saved workspace's "Learn from sessions" only while `workspacesOn(env)`; with `ALTIMATE_DISABLE_WORKSPACE=1` capture no longer starts and the source is no longer reported as "workspace"
- Test with a saved workspace and `learning_enabled: true` under the kill switch

Re-review at 2758e4a: kill-switch fix verified, earlier findings still open
Peer review of the delta since 8d83a1a, by a lead and two independent reviewers (Claude and Sol 6.1). All three agree.
Fixed:trigger.ts:57 (MAJOR, kill switch). if (state.workspace && workspacesOn(env)) closes it at the single decision point:
Every consumer goes through learningDecision: the capture gate (service.ts:121), followLearning (service.ts:128), afterTurn (trigger.ts:170) and the summary / learn status (summary.ts:34).
workspacesOn reads the same env as the rest of the function.
Precedence is unchanged: the person's own settings, the env flags and organization policy still win.
The new test fails on the old code.
The delta introduces no regressions.
Still open (no change at this commit): the 12 inline findings from the previous review, and the training-branch follow-up on the remember.ts:75 thread. The commit touches only trigger.ts and its test, and each item was re-checked at HEAD:
MAJOR:from-history.ts:136. Cleanup can delete candidate rules added during the confirmation prompt. This blocks approval.
MINOR:
proposal.ts:115: kept is set before the write.
sync.ts:609: a late ack rolls corrected_reported back.
rules.tsx:424: the startup load() overwrites newer cards.
remember.ts:15: "don't forget" never gets a card.
remember.ts:80: memory scope comes from the content, not the request.
trigger.ts:244: topics that were sent or saved are re-proposed.
project-config.ts:51: a raw {env:} value overrides the expanded one.
rules.tsx:562: the toast says "your setting" when an env var decides.
proposal.ts:533: "Not a rule" discards a requested memory.
learn.md:229 and learn.md:208: docs.
remember.ts:75 thread: the training fallback retries forever when project memory is full.
Nits (consolidated):
rules.test.ts:1145: state({ settings: { learning_enabled: true } } as Partial<…>) is redundant, because linked() already sets learning_enabled: true. The override also swaps in a partial settings object, which is why it needs the cast. state() alone does the same.
There is no service-level test that followLearning does not start capture with ALTIMATE_DISABLE_WORKSPACE=1 and saved learning_enabled: true. The decision-function test covers the logic, but a test at the gate would guard the original path.
Scope: cannot verify, because there is no linked Jira key or issue.

test/altimate/rules/rules.test.ts:1145: state({ settings: { learning_enabled: true } } as Partial<RulesStore.State>) is redundant, because linked() already sets learning_enabled: true. The override also swaps in a partial settings object, which is why it needs the cast. state() alone does the same.
test/altimate/rules/rules.test.ts:1144-1149: the kill-switch test covers learningDecision only. A service-level test that followLearning does not start capture with ALTIMATE_DISABLE_WORKSPACE=1 and saved learning_enabled: true would guard the path the original bug went through.

It takes effect right away. Approved rules are delivered either way.

MINOR (data). The 50-block fix covers only the memory branch (:110-112). This training branch checks only the per-kind rule cap. TrainingStore.save writes through MemoryStore.write (training/store.ts:53), which throws a plain Error once the project scope holds MEMORY_MAX_BLOCKS_PER_SCOPE active blocks (memory/store.ts:286-289), for example 20 rules plus 30 other training or memory blocks.
That error is not a SaveRefusedError, so settle keeps the request (proposal.ts:166-174). Every recoverRequests pass then claims it, fails and keeps it: it is never saved, the person is never told, and it permanently takes one of the MAX_REMEMBER_REQUESTS slots.
Suggested fix: apply the same active-block count here, and throw SaveRefusedError when the scope is full and the entry is new.
- `stagedRules` records the candidate file it listed the rules from (`Staged.candidate`)
- `Store.reject` takes `ifCandidate` and keeps the candidate when it no longer reads exactly that, checked inside the learn lock
- `sendStaged` discards only that candidate, and says when a changed one was kept
- Test: an untouched candidate is discarded; one a reflection wrote to meanwhile is kept with its new rule

SUGGESTION: The fallback wording claims the candidate was kept even when it was removed
Store.reject returns false both when the candidate content no longer matches ifCandidate and when the candidate file is gone (hasCandidate === false). The second case is reachable: the interactive confirmation in offerStagedForReview can wait while another process runs learn promote, learn rollback, learn reject, or auto-promote, which removes the candidate. The message then tells the person the candidate "was kept" and to run learn show, which reports no candidate. Distinguish "changed" from "gone" in the wording, or have reject signal which case occurred.
Suggested change
? " The local candidate changed while these were sent, so it was kept: `learn show`."
? " The local candidate changed or was already removed while these were sent, so nothing more was discarded: `learn show`."
Peer review of the delta since 2758e4a, by a lead and two independent reviewers (Claude and Sol 6.1), followed by one convergence round. No CRITICAL or MAJOR findings remain.
Fixed:from-history.ts:136 (MAJOR). Store.reject(..., { ifCandidate }) compares the exact candidate text inside withLearnLock. Every candidate writer takes that same cross-process lock: reflect and the auto-reflect scheduler, promote, rollback, learn reject, pin and migrate. So the compare-and-delete is atomic.
Switching stagedRules to readCandidate + Lessons.parse does not change behaviour; it is exactly what loadCandidateLessons did.
The only other reject caller (learn reject) passes no option, so it is unchanged.
By reading the code, the new test would fail on the old implementation. It was not executed in this review.

Please fix them in this PR or track them as a follow-up.
Also non-blocking: kilo's suggestion at from-history.ts:155 is valid. When the candidate was removed during the prompt (by learn promote / reject / rollback, or auto-promote), reject also returns false, and the message still says the candidate "changed … so it was kept: learn show".
Nits
learn/store.ts:745: the doc comment "a changed candidate is kept (false)" leaves out that false also means the candidate is missing. Update it together with kilo's from-history.ts:155 item.
From the previous round, still applicable: the redundant settings override at rules.test.ts:1145, and no service-level test that capture does not start under ALTIMATE_DISABLE_WORKSPACE=1.

Replaces #1438 (lesson sync), which is closed in favour of this redesign. Server side: AltimateAI/altimate-backend#7514 (
/datamates/{id}/rules) and AltimateAI/altimate-frontend#3603 (the workspace Rules tab).Type of change
What does this PR do?
In a project linked to a workspace, what the person teaches the agent can become a team rule. They confirm it on a card, the workspace owner reviews it, and approved rules reach every teammate's sessions. This replaces #1438's background lesson sync, which sent staged lessons without asking.
Proposal card, built once in the session server (
src/altimate/rules/proposal.ts,events.ts). It uses the same ask/reply shape asquestion:rule.proposal.askedgoes out on/event, and the client answers withPOST /rule-proposal/:id/reply.
