docs: changelog for the round-5 bug-hunt fixes

Summary

Adds [Unreleased] CHANGELOG entries (Added / Changed / Fixed) for the round-5 bug-hunt fixes merged so far:

#351, #352, #353, #354, #356, #357, #358, #360, #361, #362, #363, #364, #365, #367, #368, #369, #370, #371, #372, #374, #375, #377, #378, #379.

New entries are appended after the existing bullets in each subsection; no existing entry is changed. #350 is already covered by the #345 bullet, and #366/#376 (CI and test tooling) are intentionally omitted. PRs from this round that merge later (#373, #380–#391 and the CSV leading-zeros PR) will get a follow-up changelog PR.

Verification

• git diff --stat: CHANGELOG.md only, 199 insertions, no deletions.
• Every issue closed or referenced by the listed PRs appears in the new text.
