Two security fixes from an audit sweep of this repository's concurrency, money, and verification paths. Both were verified against the code and both ship with regression tests confirmed to fail against the unfixed source.

1. The unauthenticated envelope could choose the verification verdict

verify_bundle() read alg and canonicalization from the envelope and short-circuited to UNSUPPORTED before the signature was ever checked. One edited byte therefore converted a genuine, fully-verifiable receipt into "your verifier is too old":

BEFORE
genuine, untouched              -> verified     tampered=False
envelope alg -> RS256           -> unsupported  tampered=False   <-- attacker picks the status
envelope canonicalization -> v2 -> unsupported  tampered=False   <-- attacker picks the status
signing_input tampered          -> invalid      tampered=True

This is the exact integrity/capability inversion the six-state taxonomy exists to prevent — the verifier's own docstring says the non-INVALID statuses "must not be reported as evidence of tampering." It also reaches the unauthenticated MCP tool, where an LLM is handed "CANNOT VERIFY (unsupported) — a statement about the verifier's situation, not evidence of tampering." It was evidence of tampering. And it hands anyone in the transport path a deniability primitive: silently turn a VERIFIED receipt into something an operator resolves by upgrading rather than escalating.

Fix — four changes that had to land together:

  1. Key selection and the capability gates now read the signed payload. If the signature verifies, that kid is authenticated retroactively, so a relabelled envelope cannot steer key selection at all. This also closes the previously-documented kid gap, which docs/ip/ carried as [not implemented].
  2. Envelope values are cross-checked after verification, and disagreement is MISMATCH — a finding about the bundle — never UNSUPPORTED.
  3. A new is_rejected property covers MISMATCH as well as INVALID. is_tampered deliberately stays narrow — INVALID only — because MISMATCH also covers a caller-supplied expected_issuer disagreement, which is not a claim about tampering. Callers that need "do not trust this bundle" should read is_rejected.
  4. The CLI maps is_rejected to exit 1, not exit 2 ("could not determine").

3 and 4 are not extras. The fix moves the envelope-kid attack out of INVALID and into MISMATCH; since MISMATCH did not previously feed any rejection signal and the CLI routed it to "couldn't tell," shipping 1–2 alone would have made that attack's caller-visible signal worse — a caught forgery reported as an outage.

AFTER
genuine             -> verified     is_rejected=False  is_tampered=False
envelope alg flip   -> mismatch     is_rejected=True   is_tampered=False
envelope relabel id -> mismatch     is_rejected=True   is_tampered=False
real tampering      -> invalid      is_rejected=True   is_tampered=True
key absent          -> unknown_key  is_rejected=False  is_tampered=False

Two existing tests failed, and they were wrong. Both asserted that an envelope-only alg/canonicalization override yields UNSUPPORTED, on bundles with a deliberately bogus signature, under the comment "None of these are evidence of tampering." That encoded the defect. They are replaced with cases where the signed payload declares the unsupported value — genuine UNSUPPORTED, still resolving before any signature check so the file's no-cryptography constraint holds.

The new tests asserting the envelope specifically cannot downgrade the status need a real Ed25519 signer, so they live in tests/test_receipt_portability.py, not in the SDK's bare-install test file whose whole premise is that it runs without the verify extra. That premise is now verified directly: the SDK suite passes in a venv with no cryptography installed (56 passed, 3 skipped).

Exposure: this verifier has never been released

An earlier revision of this description claimed the published b2a_sdk 0.4.0 sdist carried this defect. That was wrong, and the correction matters because it lowers the real-world exposure. The published python-sdk-v0.4.0 tag (released 2026-08-07) contains no receipt_verifier.py at all — git ls-tree -r python-sdk-v0.4.0 -- b2a_sdk/src/b2a_sdk/ lists eight modules and the verifier is not among them. The whole verifier, CLI, and verify extra sit under ## Unreleased in b2a_sdk/CHANGELOG.md. No user has ever been able to pip install the defective verifier; it is fixed before its first release.

2. The governed AWI HTTP charge had no operation_key

awi_http_governance.py called money.charge() with no operation_key, so it had neither the uq_ledger_wallet_operation_key constraint nor the adopt-the-existing-debit recovery the governed MCP path relies on. Unlike the read-modify-write races elsewhere in this codebase, this one does not depend on a row lock being a no-op — it is exposed on PostgreSQL too.

The failure branch is what makes it bite. If the charge commits but its acknowledgement is lost, the except handler releases the permit budget and completes the idempotency record as charge_failed with a 500. The caller's key now replays a permanent error, so it retries under a fresh key and is debited a second time for one logical action. Because response_json is non-NULL, both reconciler branches skip the record, so nothing ever flags the orphan. Six governed endpoints route through this path.

Fix: thread the idempotency record id onto AwiHttpGovernedContext (begin → begin_with_record) and pass it as operation_key, so a retry adopts the first durable debit instead of creating one.

The regression test drives the real route and asserts the debit carries the request's record id, and that no governed AWI charge lands with a NULL operation_key. An earlier draft tested money.charge() in isolation and would have passed even with the wiring missing — the mechanism was never the broken part, this caller simply did not use it. Against the unfixed service the route test fails with assert 0 == 1.

Type of Change

  • fix

Validation

Every CI job was reproduced locally, including the ones that need PostgreSQL or a booted server.

  • pytest -q tests b2a_sdk/tests — 1771 passed, 32 skipped
  • both fixes confirmed to fail against the unfixed source — reverting the service/verifier, not the test
  • ruff check . clean; mypy app/ clean across 172 source files (mypy 2.3.1, the pinned floor)
  • python scripts/check_doc_references.py — 266 refs across 242 files resolve
  • PostgreSQL 16: alembic upgrade head clean; test_permit_postgres_concurrency 11 passed; test_mcp_postgres_multiprocess 13 passed (two-process kill-at-checkpoint crash recovery)
  • python_sdk job: wheel built and installed into a clean venv; SDK suite green against the wheel with no cryptography present; SDK ruff + mypy clean
  • invariant_attack_proof: verdict HELD, killed mid-storm at 12 committed debits, zero failing checks
  • trust_release_gate, trust_coverage_gate (82.6% trust scope), production_trust (6 passed), quickstart-check (6 passed)
  • demo_trust_plane, dogfood_trust_plane, red_team_trust_plane, agent_ops_war_room all --assert clean
  • gitleaks working-tree scan clean, and the known-bad canary is still caught
  • python scripts/export_openapi.py --check — matches app.openapi()
  • no new claim contradicts SECURITY_LIMITATIONS.md

Risk

  • medium

Two behavioural changes callers can observe. A bundle whose envelope disagrees with its signed content now returns MISMATCH with is_rejected=True and CLI exit 1, where it previously returned UNSUPPORTED/INVALID with exit 2 — scripts branching on exit codes will see caught forgeries move from "undetermined" to "rejected", which is the intended correction. Because the verifier is unreleased, no published consumer is affected. And a retried AWI charge now adopts the existing debit rather than creating one. No migration, no API surface change.

Not addressed here

The sweep produced fourteen verified findings; these are two of them. Still open, and not fixed by this PR:

  • Engine-independent: local governed calls have a crash window (operation_kind='local', debit committed, ledger_entry_id IS NULL) matching neither reconciler branch — the record sticks forever and needs_review stays 0.
  • Exposed wherever FOR UPDATE is a no-op, and nothing forbids SQLite in production: the wallet balance debited by read-modify-write (no sa_update touches WalletModel anywhere); reclaim_child_wallet able to credit a parent twice; release_dispatch_budget_once not once-only; reserve_budget missing the expiry predicate; refund reconciliation clobbering concurrent reservations; velocity counters under-counting, which silently disables the spend cap and anomaly freeze.

Note

High Risk
Changes authentication-adjacent verification semantics and ledger charge deduplication on six AWI routes; CLI exit-code branching and caller-visible MISMATCH vs UNSUPPORTED behavior shift, though the SDK verifier is unreleased.

Overview
Two security fixes: offline receipt verification and governed AWI HTTP billing.

Receipt verifier (b2a_sdk.receipt_verifier) no longer trusts the unauthenticated envelope for kid, alg, or canonicalization. Those gates and key selection now read the signed signing_input; envelope fields are cross-checked only after a successful signature, with disagreement reported as MISMATCH (not UNSUPPORTED or INVALID). Adds is_rejected (INVALID + MISMATCH) for “do not trust”; is_tampered stays INVALID-only. The b2a-verify-receipt CLI maps is_rejected to exit 1 instead of 2. Public verify_receipt MCP tool narrates MISMATCH before is_tampered so envelope inconsistency is not described as a bad signature.

Governed AWI HTTP (awi_http_governance) switches idempotency to begin_with_record, stores record_id on context, and passes operation_key=ctx.record_id into money.charge()—matching governed MCP deduplication so a committed debit with a lost ack can be adopted on retry instead of double-charging under a new idempotency key.

claude added 3 commits August 17, 2026 06:02
The offline verifier's core property is that everything deciding an outcome
comes from the SIGNED bytes. Three fields broke it, and one edited byte was
enough to exploit any of them.

`alg` and `canonicalization` were read from the envelope and short-circuited
to UNSUPPORTED *before* the signature was ever checked. So flipping
`"alg": "Ed25519"` to `"RS256"` on a genuine, fully verifiable receipt
produced:

    genuine, untouched              -> verified     tampered=False
    envelope alg -> RS256           -> unsupported  tampered=False
    envelope canonicalization -> v2 -> unsupported  tampered=False

UNSUPPORTED is a statement about the verifier's own capability. The attacker
was choosing it. That is the integrity/capability inversion the six-state
taxonomy exists to prevent, and it reached the public MCP tool, where the
model is told verbatim that the result is "not evidence of tampering."

`kid` had the same shape, already documented: key SELECTION came from the
envelope, so a relabelled bundle resolved to INVALID (or, worse, to
UNKNOWN_KEY -- a genuine receipt made to look like a key-distribution
problem).

Fix: parse the signed payload first, select the key by its `kid`, and gate
capability on its `alg`/`canonicalization`. If the signature then verifies,
that kid is authenticated retroactively. Envelope values are still checked,
but afterwards, and disagreement is MISMATCH -- a finding about the bundle --
never UNSUPPORTED.

Payload-first selection lost one detection, so it is restored explicitly:
when the payload names a kid we do not hold, we now check whether the
envelope names one we do that actually signs those bytes. If so the bundle is
provably self-contradicting and that is a verdict (MISMATCH), not a gap.
Without this a forger could name a nonexistent kid to downgrade a detectable
inconsistency into "refresh your key set and try again".

Caller-visible signal, which had to change with it. Moving attacks out of
INVALID and into MISMATCH would have made them *quieter*, since MISMATCH set
no tampering flag and the CLI mapped it to exit 2, "could not determine" --
a caught forgery reported as an outage. But widening `is_tampered` to cover
MISMATCH was wrong too: MISMATCH also carries caller-expectation failures
like `expected_issuer`, and calling those forgery is the same category error
in the opposite direction. It also made the MCP endpoint render "its
signature does not hold" for a check that runs before verification.

So `is_tampered` stays narrow (INVALID only, a claim that cryptography
failed), a new `is_rejected` covers INVALID+MISMATCH for the accept/reject
decision most callers actually want, the CLI branches on `is_rejected`, and
the endpoint checks MISMATCH before is_tampered so it keeps its accurate
wording.

## Tests that encoded the defect

Five failed. None was fixed by weakening it.

- Two SDK cases asserted envelope-only alg/canonicalization -> UNSUPPORTED
  with the comment "None of these are evidence of tampering", on bundles
  with a deliberately bogus signature. Replaced with the same values
  declared in the SIGNED payload (still resolving before any signature
  check, so the file's no-cryptography constraint holds), plus a new test
  asserting the envelope specifically cannot downgrade the status.
- `test_unknown_key_is_not_reported_as_tampering` relabelled only the
  envelope, so the plane did hold the key and the receipt was genuine. Now
  names an unpublished kid inside the signed bytes -- the real unknown-key
  case -- and a new test pins the relabelled-envelope case to MISMATCH.
- `test_preflight_failures_do_not_touch_the_key_store` used envelope alg to
  stand in for a cheap rejection. An envelope-declared value is no longer
  cheaply rejectable, by design; the fixtures now declare it in the payload.
- `test_signature_under_a_key_the_payload_does_not_name_is_rejected` caught
  the real regression described above, and is passing on its original
  assertions.

## Validation

- pytest 1732 passed, 19 skipped (app + b2a_sdk)
- ruff clean; mypy clean across 172 source files
- taxonomy re-checked end to end with a live Ed25519 signer:
  genuine -> verified; envelope alg/canonicalization/kid/receipt_id edits ->
  mismatch (is_rejected); payload tampering -> invalid; absent key ->
  unknown_key (not rejected)

Found by an audit sweep of envelope-vs-signed-payload trust boundaries.

The governed AWI HTTP path called money.charge() with no operation_key, so
it had neither the uq_ledger_wallet_operation_key constraint nor the
adopt-the-existing-debit recovery on IntegrityError that the governed MCP
path relies on. Unlike the read-modify-write races elsewhere in this
codebase, this one does not depend on a row lock being a no-op -- it is
exposed on PostgreSQL too.

The failure branch is what makes it bite. If the charge commits but its
acknowledgement is lost -- a cancelled coroutine, a connection drop -- the
`except Exception` handler releases the permit budget and calls
abort_awi_http_governed, which *completes* the idempotency record with
{"error": "charge_failed"} and a 500. The caller's key now replays a
permanent error, so it retries under a fresh key and is debited a second
time for one logical action. Because response_json is non-NULL, both
reconciler branches skip the record, so nothing ever flags the orphan.

Six governed endpoints route through this path.

The fix threads the idempotency record id onto AwiHttpGovernedContext
(begin -> begin_with_record) and passes it as operation_key, so a retry
adopts the first durable debit instead of creating one -- the same
database-evaluated guarantee the MCP path already had.

## The regression test fails without the fix

test_awi_route_keys_its_debit_to_the_idempotency_record drives the real
route and asserts the debit carries the request's record id, and that no
governed AWI charge lands with a NULL operation_key.

Asserted on the route rather than on money.charge directly, and that
distinction matters: an earlier draft of this test exercised the charge
mechanism in isolation and would have passed even with the wiring missing,
because the mechanism was never the broken part -- this caller simply did
not use it. Against the unfixed service the route test fails with
`assert 0 == 1`.

## Validation

- pytest 1733 passed, 19 skipped (app + b2a_sdk)
- test confirmed to fail against the pre-fix service (output above)
- ruff clean; mypy clean across 172 source files

Found by an audit sweep of the money and metering paths.

📝 Walkthrough

Summary by CodeRabbit

  • Bug Fixes
    • Prevented duplicate governed HTTP charges when acknowledgements are lost or requests are retried.
    • Improved receipt verification to distinguish envelope/content mismatches from invalid signatures.
    • Ensured signed receipt metadata takes precedence during verification.
    • Updated verification results and CLI exit codes so definitive rejections are reported consistently.
  • Documentation
    • Clarified receipt verification outcomes, mismatch handling, and CLI behavior in the changelogs.

Walkthrough

The PR keys governed AWI ledger debits to durable idempotency records. It also updates receipt verification to use signed metadata, classify envelope conflicts as MISMATCH, and map determined rejections to invalid CLI and MCP outcomes.

Estimated code review effort: 4 (Complex) | ~45 minutes

Merge Risk: 🟡 Moderate · up to 148b1

The change correctly authenticates receipt metadata and keys governed AWI charges for retry deduplication, but merge readiness remains moderate because the post-commit charge-recovery path is not covered, direct context construction can still allow an unkeyed charge, and CLI/test follow-up is needed to preserve accurate rejection semantics.

…rify

The three cases I added to b2a_sdk/tests/test_receipt_verifier.py asserted
`status is INVALID`, which requires actually verifying a signature. That
file is explicitly for cases that resolve *before* any signature check, so
it runs against a bare SDK install without the `verify` extra -- its
docstring says so. The python_sdk CI matrix installs the bare wheel, so all
four legs failed with "Ed25519 verification requires the 'cryptography'
package". They passed locally only because cryptography is installed here.

Moved to tests/test_receipt_portability.py, which has a real signing helper
and always has cryptography. That also makes the tests stronger than what
they replaced: instead of asserting "not UNSUPPORTED" on a bundle with a
deliberately bogus signature, they now build a genuinely VERIFIED bundle,
assert the baseline verifies untouched, and then show one edited envelope
byte turns it into MISMATCH rather than an attacker-chosen UNSUPPORTED.

Two fixture bugs found and fixed while writing them:

- `_signed_bundle` takes `kid` as a keyword-only parameter, so passing it
  again through **overrides raises TypeError. The bundle is now built and
  then mutated.
- `_local_signer()` hardcodes its kid, so calling it twice yields one kid
  with the second key silently overwriting the first in a merged key set.
  The relabelling test was therefore failing because the wrong key was
  selected, not because of relabelling -- proving nothing. It now mints a
  second key under a genuinely distinct kid.

The assertions use `is_rejected`, not `is_tampered`. MISMATCH deliberately
does not set `is_tampered`, because it also covers caller-expectation
failures such as `expected_issuer`, and calling those forgery would be the
same category error as reporting a missing key as tampering.

Validation: pytest 1751 passed, 19 skipped; ruff clean; the SDK test file
re-checked with cryptography blocked, and none of the remaining failures
are in it.

Two corrections to my own description above, and a CI fix.

1. The description of is_tampered is wrong. Point 3 of the fix list says "is_tampered now covers MISMATCH as well as INVALID." It does not, and the committed code is right where my description was wrong.

is_tampered is deliberately narrow — it asserts a signature failed. MISMATCH also covers caller-expectation failures such as expected_issuer, and reporting "you asked for a different issuer" as forgery would be the same category error as reporting a missing key as tampering, just in the opposite direction. The accept/reject decision is is_rejected, which covers INVALID and MISMATCH, and that is what the CLI branches on to return exit 1.

So the corrected version of point 3 is: is_rejected covers INVALID and MISMATCH; is_tampered stays narrow. The observable behaviour claimed in the PR body — that a caught forgery now exits 1 rather than 2 — is unchanged and correct.

2. python_sdk failed on all four Python versions, and it was my mistake. The three cases I added to b2a_sdk/tests/test_receipt_verifier.py asserted status is INVALID, which requires actually verifying a signature. That file exists specifically for cases resolving before any signature check so it can run against a bare SDK install without the verify extra — its own docstring says so, and the python_sdk matrix installs the bare wheel. They passed locally only because cryptography is present in my environment.

Fixed in fd1a32e by moving them to tests/test_receipt_portability.py, which has a real signing helper. That made them stronger rather than merely relocated: instead of asserting "not UNSUPPORTED" against a deliberately bogus signature, they now build a genuinely VERIFIED bundle, assert the baseline verifies untouched, then show one edited envelope byte turning it into MISMATCH.

Two fixture bugs surfaced while writing them, both of which would have made a test that passes while proving nothing:

  • _signed_bundle takes kid as keyword-only, so passing it again via **overrides raises TypeError.
  • _local_signer() hardcodes its kid, so calling it twice gives one kid with the second key silently overwriting the first. The relabelling test was failing because the wrong key was selected, not because of relabelling.

3. Minor: the fd1a32e commit message says "1751 passed". The actual figure is 1749 — removing three parametrized SDK cases and adding three portability cases nets to zero, so the count was unchanged from the previous run. Not worth a force-push to amend; recording it here instead.

Generated by Claude Code

The verifier work changes behaviour the SDK changelog already documents:
the Unreleased section promised `b2a-verify-receipt` exit codes "0
verified, 1 forged, 2 undetermined", which no longer describes what the
CLI does now that MISMATCH is a rejection. Amend that entry, record the
signed-payload authority rule and the new `is_rejected` property, and
note that MISMATCH is a third outcome family alongside the integrity and
capability ones.

The app-side changes were undocumented entirely. Add the governed AWI
`operation_key` fix and the public MCP `verify_receipt` wording change
to the root changelog's Unreleased security section.

No behaviour change; documentation only.

success state; also cover the relevant failure/negative path without permitting
a second debit.

In `@tests/test_public_mcp_endpoint.py`:
- Around line 746-752: Update the unknown-key fixture built in _signed_bundle so
its modified signing_input is re-signed with a generated Ed25519 private key
whose kid is absent from the published key set; keep the matching unknown kid in
both the signed payload and fixture metadata, and preserve the existing
unknown_key assertion.

Reject a missing record_id before charging.

All current route callers obtain gov from begin_awi_http_governed, but direct context construction can still pass operation_key=None and create an unkeyed debit. Make record_id required or reject it before money.charge. Add a negative-path test.

Verify known-key bundles before returning UNSUPPORTED.

signing_input is still attacker-controlled until its signature verifies. An attacker can edit alg or canonicalization in a genuine bundle while retaining its original signature. Lines 376-395 then return UNSUPPORTED before the available Ed25519 key verifies the exact bytes. This converts a detectable INVALID receipt into an undetermined capability result.

Keep MISMATCH messaging and CLI coverage aligned with rejection semantics.

Add a regression test for the post-commit charge recovery path.

This test verifies only a normal successful debit. It does not force the path where the ledger debit commits but money.charge() loses its acknowledgement. The change must adopt that existing debit instead of aborting the request and allowing a later charge to create another debit.

Add a route-level test that injects this failure mode. Assert one ledger debit, the original ledger_entry_id, and correct idempotency completion after recovery.

As per coding guidelines, include “relevant negative paths.”

Sign the unknown-key fixture with an unpublished key.

This fixture changes signing_input after _signed_bundle() signs it. The signature is therefore invalid, not a genuine receipt from an unpublished key. Generate an Ed25519 key that is absent from the published key set, sign the modified signing_input, and retain the unknown_key assertion.

…ath; make dispatch-budget release once-only without a row lock; refuse SQLite in production (#304)

Three fixes from the audit findings #302 listed as not addressed. Each was
verified against current main and each ships a regression test confirmed to
fail against the unfixed source.

1. reserve_budget spent against expired permits.
   #298 added the expiry predicate to authorize_and_reserve's guarded update,
   but reserve_budget is a second reservation path and did not get it. An
   expired permit keeps status="active" in storage (expiry is a dynamic check,
   not a stored state), so the WHERE clause matched and budget moved. Adds the
   same expires_at > now term, plus precise denial reasons (permit_expired /
   permit_not_active) instead of reporting every miss as budget_exceeded.
   Engine-independent: this was wrong on PostgreSQL too.

   Test: an active permit whose expires_at is moved into the past is refused
   with permit_expired and spent_credits stays 0. Against the unfixed source
   the reservation succeeds (DID NOT RAISE).

2. release_dispatch_budget_once was not once-only.
   It read budget_released_at under with_for_update=True and then decremented
   the permit. SQLAlchemy silently drops FOR UPDATE on engines that do not
   support it, so two concurrent callers both observed NULL and both released
   — the permit was credited back twice for one dispatch. The claim is now a
   guarded UPDATE (NULL -> now, rowcount == 1 wins); only the winner
   decrements. The decrement itself was already atomic; it was the claim that
   was not.

   Test: two concurrent releases must yield exactly one True and exactly one
   decrement. Against the unfixed source both return True.

3. SQLite was a permitted production durable-state backend.
   STATE_BACKEND=memory was refused in production-like environments but
   sqlite was not, and auto-resolution would select it from a SQLite
   DATABASE_URL — the ordinary local-development setting. That is the
   deployment-level enabler for every remaining FOR UPDATE-is-a-no-op finding
   in the money path. Both the explicit and the auto path now fail closed.

   Tests: production-like + STATE_BACKEND=sqlite with SQLITE_URL set, and
   auto-resolution from a sqlite DATABASE_URL, both raise
   DurableStateConfigError.

   test_sqlite_without_url_refuses_production_like previously asserted the
   refusal reason was the missing SQLITE_URL, which implied a configured URL
   would be accepted. Its assertion is updated to the stricter reason; the
   behaviour it guards (production refuses this config) is unchanged.

Also: the docs sweep in test_onboarding_contract.py walked .claude/worktrees/,
a gitignored path holding full checkouts, so a copy of an already-excluded
file failed the sweep for anyone using a git worktree there. Excluded .claude/.

Tests: 1711 passed, 32 skipped; production_trust 6 passed; ruff clean; mypy
clean across 172 source files; demo and red-team trust-plane gates --assert
clean.

Not addressed here: the remaining read-modify-write findings (wallet balance,
reclaim_child_wallet double credit, refund reconciliation clobbering
reservations, velocity counters) and the local-governed reconciler crash
window. Fix 3 removes their production reachability; converting them to
guarded updates is separate work.

…he debit and its link (#306)

* fix(idempotency): reconcile the local governed crash window between the debit and its link

The engine-independent finding from #302's 'Not addressed here' list. A local
governed call charges with operation_key=<record_id> and only afterwards
attaches that ledger entry to its idempotency record. A crash in that gap left
response_json IS NULL and ledger_entry_id IS NULL, which matched neither
reconciler pass:

  - the unstarted pass is scoped to operation_kind == 'upstream_mcp'
  - the stuck pass requires ledger_entry_id IS NOT NULL

So the record stuck forever, replays kept raising in-progress, and
needs_review stayed 0 — real money had moved and nothing surfaced it.

Adds a pass that runs before the stuck pass and adopts the orphan by the key
the charge already carries: if a ledger entry exists with
operation_key == record_id, link it, and the existing stuck logic then repairs
the record from its receipt or counts it for manual review. No new repair
semantics.

Deliberately NOT deleting the no-debit case. Local tools run their side effect
before the debit exists, so expiring the identity would let a retry execute it
a second time — the invariant asserted by
test_stale_local_identity_is_not_deleted_without_compensation_proof, which
caught exactly that mistake in an earlier revision of this change. Without a
committed debit as compensation proof the record is left untouched.

The session is created with autoflush=False, so the newly written links are
flushed explicitly before the stuck pass selects them.

Tests (both confirmed to fail against the unfixed source):
  - debit + receipt orphaned -> (repaired, needs_review) == (1, 0)
  - debit, receipt missing   -> (repaired, needs_review) == (0, 1)
Unfixed, both report (0, 0) — the record is invisible to reconciliation.

Tests: 1709 passed, 32 skipped; production_trust 6 passed; ruff clean; mypy
clean across 172 source files; demo and red-team trust-plane gates --assert
clean.

* fix(idempotency): accept only a debit as compensation proof when linking

The linking pass adopted any ledger entry carrying the record's
operation_key. The wallet/operation-key uniqueness constraint already makes
a same-key credit impossible alongside the debit, so this was not reachable
today -- but the pass's whole premise is "money left this wallet", and that
direction should be stated in the query rather than inferred from a
constraint elsewhere.

Adds a negative test that plants a same-key credit and asserts
reconciliation reports (0, 0) and leaves ledger_entry_id and response_json
untouched. Confirmed to fail against the unguarded query (reports (1, 0):
the credit is adopted as proof of a charge).
