• Doctor alignment with beads snapshots

12 commits. Supersedes #112 (codex/reality-oss-pr) which is a subset.

Test plan

• cd sdp-plugin && go test ./... -count=1
• cd sdp-plugin && go vet ./...
• sdp reality emit-oss produces valid artifacts
• bd ready works without local beads database

The original branch mixed reality, beads drift, hooks fixes, prompts, and committed runtime artifacts. The replacements split those concerns into mergeable lines.
