docs(design): record P-D1, P-D2 and P-D4 as decided - #48

Mark the three plan decisions left open since 2026-10-23 as decided in 07 §5.2, docs/design/README and ADR-0007 / ADR-0009: the approval matrix in 03 §12 is the single source, material A/B/C lives on srm_category, and the core keeps Chinese as source language with full English translations (ADR-0007 result note explains why and how to switch later).
