Summary

Two concurrency defects in the SDK lock layer, fixed together because they share the lock acquisition path.

GH-1549 — lost update after lease takeover (pm-gh1549)

  • New acquireLockLease() returns { release, fence }. fence(commit) verifies the lease token under the per-lock stale-cleanup gate (the same gate stale removal and lock publication hold) and keeps it for the whole commit, so a takeover cannot interleave with a fenced commit.
  • A lost lease refuses with lock_lost (exit 4, context.lease + context.current_owner) and writes neither item nor history; the newer value is kept.
  • mutateItem / deleteItem additionally refuse with stale_revision when the stored item bytes changed after the locked read (unlocked edits, extension lock overrides). restore fences its write with the lease.
  • acquireLock is now a thin wrapper, so all other callers keep their exact contract. acquireLockLease / LockLease are public SDK so packages (e.g. pm-csv) can fence their own state.

GH-1516 — queued writers fail at a fixed deadline (pm-iop1pg)

  • locks.wait_ms / PM_LOCK_WAIT_MS is now a no-progress window: every observed owner handoff restarts it, capped at 120 s total. A stalled single owner still refuses after the budget; explicit maxWaitMs ceilings and 0 fail-fast are unchanged.
  • lock_conflict recovery reports waited_ms and owner_handoffs.
  • Per-parent hierarchy lock scope and lock-free appends stay on pm-lvyd.

Evidence

  • tests/unit/core/lock/lock-lease-fencing.spec.ts: takeover → lock_lost with commit never run; released lease; the issue's same-process SDK repro now keeps the newer title and history; stale_revision; handoff-restarted wait; stalled owner; maxWaitMs ceiling.
  • Multi-process conformance: 10 comment appends + 10 parented creates on one plain-directory tracker under a 1 s window lose zero writes; the same test fails on the previous build (2/3 red vs 4/4 green).
  • Regenerated: error catalog, contract snapshot, SDK public surface (acknowledged additive changes), docs (SDK, CONFIGURATION, SDK_STORAGE_LIFECYCLE), CHANGELOG via pm-changelog.

Also files pm-0trehk: the Release Reliability monitor is red since #1548 re-sharded the nightly matrix (new legs at 0 attempts → insufficient_completed_attempts).

Closes #1549

Summary by Sourcery

Harden SDK lock acquisition and commits against lease takeovers while waiting for active lock progress instead of a fixed deadline.

New Features:

  • Expose fenceable SDK lock leases through the public API for protecting long-running commits from lease takeovers.

Bug Fixes:

  • Prevent expired or superseded lock holders from overwriting newer item, history, workspace, or extension-backed writes.
  • Reject item mutations when the stored revision changed or was removed after it was read.
  • Allow configured progress-aware lock users to continue waiting while lock ownership changes hands instead of failing at a fixed deadline.
  • Protect workspace lock renewal and snapshot activation from publishing stale lock state after ownership changes.

Enhancements:

  • Add structured lock-loss and contention diagnostics, including lease ownership, wait duration, and owner handoff counts.
  • Keep the existing acquireLock contract while routing lock acquisition through the new lease implementation.

Documentation:

  • Document lease fencing, stale revision conflicts, progress-aware waiting, and extension-provider responsibilities in SDK and configuration guidance.

Tests:

  • Add unit, integration, and multi-process coverage for lease fencing, stale revisions, lock-gate contention, progress-aware waiting, diagnostics, workspace recovery, and concurrent writer conformance.

Chores:

  • Regenerate error catalogs, contract snapshots, SDK surface metadata, generated documentation, and changelog entries.

Summary by cubic

Fixes two concurrency defects in the SDK lock layer: a lease takeover can no longer overwrite newer commits, and contended writers wait on lock progress instead of a fixed deadline.

Bug Fixes

  • acquireLockLease() returns { release, fence }; fence(commit) verifies the lease token under the stale-cleanup gate for the whole commit. A lost lease refuses with lock_lost (exit 4); fence_gate_unavailable is used when the gate never frees, with retry and pm gc --scope locks --dry-run advice. Neither item nor history is written.
  • mutateItem/deleteItem refuse with stale_revision when item bytes changed after the locked read or the file is removed or unreadable at commit time; restore uses lock_lost or its own changed-while-waiting conflict.
  • item_store_write and non-dry-run item_store_delete run inside the fence, so storage overrides only persist after lease and revision verification.
  • Workspace snapshot activation and heartbeat renewal now run under the lease fence: the lock copy plus root swap refuse with lock_lost when ownership was lost, and renewals run under the stale-cleanup gate so they can never overwrite a taken lock.
  • Extension lock overrides own their lifecycle, so fence() runs the commit unfenced there; packages must fence in their provider or compare-and-write atomically at the storage boundary.
  • locks.wait_ms stays a fixed total budget by default; waitPolicy: "progress" restarts it on each observed owner handoff (120 s cap, 0 fail-fast unchanged), and the item-store lease, restore, and hierarchy lock opt in.
  • lock_lost and lock_conflict diagnostics carry lease, current_owner, item_id, waited_ms, and owner_handoffs through JSON, text, and compact CLI output.
  • acquireLockLease and LockLease are public SDK; acquireLock stays a thin wrapper with the default contract.
  • New tests cover gate contention, never-free gates, overrides reached only under the fence, removed or unreadable item files, snapshots and renewals after takeover, wait budgets, and a conformance run of 10 appends plus 10 parented creates losing zero writes.

Written for commit bbb9f09. Summary will update on new commits.

…ixed deadline

A mutation whose callback outlived its lock TTL still committed, silently
overwriting a newer write committed by the process that took over the expired
lock (GH-1549). acquireLockLease now returns { release, fence }: fence(commit)
verifies the lease token under the per-lock stale-cleanup gate, the same gate
stale removal and lock publication hold, and keeps it for the whole commit, so a
takeover cannot interleave. A lost lease refuses with lock_lost (exit 4) carrying
the lease and current owner, and writes neither item nor history. mutateItem and
deleteItem also refuse with stale_revision when the stored item bytes changed
after the locked read (unlocked edits, extension lock overrides); restore fences
its write with the lease. acquireLock is a thin wrapper, so every other caller
keeps its exact contract, and acquireLockLease/LockLease are public SDK so
packages can fence their own state.

Concurrent writers on one plain-directory tracker failed with lock_conflict at
a fixed 3 s budget even while the lock was continuously changing hands
(GH-1516). The wait budget is now a no-progress window that restarts on every
observed owner handoff, capped at 120 s total; explicit maxWaitMs ceilings and
fail-fast 0 are unchanged, and lock_conflict recovery reports waited_ms and
owner_handoffs. A multi-process conformance test (10 comment appends + 10
parented creates under a 1 s window) loses zero writes and fails on the
previous build.

Docs (SDK, configuration, storage lifecycle), the settings description, error
catalog, contract snapshot and SDK surface are regenerated. Closes pm-gh1549
and pm-iop1pg; remaining GH-1516 proposals stay on pm-lvyd. Files pm-0trehk for
the nightly reliability monitor staying red after matrix re-sharding.

Merge Risk: 🔵 Low · up to bbb9f

The change adds lease fencing and progress-aware waiting for the main contended write paths. The review found no new blocking defects. One narrow gap remains: during a workspace snapshot restore, a takeover could still slip in while the root is being swapped. This is rare, and the owner should track it before treating the fencing guarantee as complete for that path.

Full details: Title check

Explanation

The title names the two main changes: lease-fenced lock commits (fence(commit) refusing stale lease holders) and progress-based lock waiting in place of a fixed deadline. Both match the objectives and the changeset summaries, and the scope is specific enough for a teammate scanning history.

Full details: Description check

Explanation

The description covers the GH-1549 lease-fencing work and the GH-1516 progress-aware wait change, with the affected APIs, error codes, config behavior, and test evidence. It also notes the related pm-0trehk reliability issue. It is closely tied to the changeset.

Full details: Linked Issues check

Explanation

[#1549] Directly linked requirement: refuse a commit after lease takeover, keep the newer value, and check lock generation or item revision before writing.

Evidence in the PR:

  • acquireLockLease() returns LockLease. fence(commit) verifies the owner token under the per-lock stale-cleanup gate and runs the commit only while the lease is current. Otherwise it throws lock_lost with lease and current_owner context and does not run the commit.
  • mutateItem and deleteItem reread the item inside the fence. Changed or removed item bytes refuse with stale_revision, and the commit callback does not run.
  • The author's independent reproduction at head 6bb2cfd reports that the stale SDK mutation refused with lock_lost (exit 4), and the final title and history kept the newer CLI write.
  • tests/unit/core/lock/lock-lease-fencing.spec.ts covers takeover, release, changed or removed items, and same-process concurrent writes.

The requirement is met by the reviewed source and tests. The excluded generated files do not affect this conclusion.

Unresolved uncertainty: the author reports local whole-suite invocations that exited 1 without diagnostics, and a 20-process conformance case that was skipped locally. These reports do not name a failing #1549 requirement, so they do not change this verdict. The issue's heartbeat or deadline-exposure option is not required, because the issue says 'or'.

Extension lock_acquire overrides run commits unfenced. This is documented as the extension provider's responsibility. It is not a built-in #1549 requirement failure.

Full details: Out of Scope Changes check

Explanation

The PR contains changes not required by directly linked issue #1549.

  1. Progress-aware lock waiting ([#1516] scope, not a #1549 requirement): waitPolicy: "progress" in src/core/lock/lock.ts; opt-in in src/sdk/lifecycle/hierarchy-mutation-lock.ts; progress opt-in in item-store, restore, and workspace paths; the locks_wait_ms descriptor in src/core/config/nested-settings.ts; the progress and 120-second cap documentation in docs/CONFIGURATION.md, docs/SDK.md, and docs/SDK_STORAGE_LIFECYCLE.md; the waited_ms and owner_handoffs diagnostic fields in src/core/shared/errors.ts, src/cli/error-guidance.ts, and src/sdk/cli-contracts/agent-output-contracts.ts; the related public-surface entries in sdk/public-surface.json; the GH-1516 changelog entry; and the tests and records that cover them.
  2. Separate tracker records: .agents/pm/issues/pm-0trehk.toon, .agents/pm/history/pm-0trehk.jsonl, .agents/pm/issues/pm-iop1pg.toon, .agents/pm/history/pm-iop1pg.jsonl, and .agents/pm/tasks/pm-lvyd.toon with its history. These concern a CI nightly-matrix reliability issue, a separate lock-wait issue, and a separate hierarchy task. None is a linked issue for #1549.

The PR description links these changes by stating that the two defects 'share the lock acquisition path.' Shared code is not a requirement of #1549. #1549 names #1516 only as related. The #1549 fix (lease fencing and stale-revision checks) does not depend on progress-aware waiting.

The lease-fencing changes remain in scope. These include acquireLockLease, LockLease, the lock_lost and stale_revision errors, the item-store and restore fences, the workspace fencing, the related tests, the SDK lease documentation, and the error catalog and contract entries for lock_lost and stale_revision.

Resolution

Move the #1516 progress-aware waiting work to a separate pull request. This includes the waitPolicy option and its opt-in call sites, the locks_wait_ms descriptor change, the wait-policy documentation, the waited_ms and owner_handoffs fields and their projections, the related public-surface entries, the GH-1516 changelog entry, and the tests that cover them. Restore the default fixed total wait budget in the fencing paths. Assessment against linked issues

Issue Objective Addressed Explanation
#1549 Prevent SDK item mutations that outlive their lock TTL from committing after another writer takes over the expired lease, so the newer write and its history are preserved and the stale writer receives a typed refusal. ✅
#1549 Reject item mutations when the stored revision has changed since the mutation read it, preventing stale overwrites even when a lock check alone would not detect the change. ✅
#1549 Apply lease fencing to other item commit paths, including deletion and restore, so an expired or taken-over lease cannot commit stale item state. ✅

…restore's refusal

Extension lock_acquire overrides own their lifecycle, so fence() runs commit
unfenced there; packages must fence in their provider or compare the protected
resource's revision. mutateItem/deleteItem stale_revision still covers
override-held locks. restore refuses with lock_lost or its own changed-while-
waiting conflict, not stale_revision. The progress-wait test publishes lock
generations by rename so a waiter never reads an empty lock file. Sharpens the
warm-up/lineage declaration contract on pm-0trehk.

Independent source verification at immutable head 6bb2cfd: the earlier public-SDK/CLI reproduction for #1549 now preserves the newer write. In an isolated plain-directory tracker with locks.ttl_seconds=1, the stale mutateItem callback waits 1300 ms while a newer CLI update succeeds (exit 0). The stale SDK writer refuses with lock_lost (exit 4); final title remains Newer protected value. Authoritative history contains the newer value and no stale-writer entry.

Bounded local evidence (Node 24.19.0, exact source-built artifact): 65 lock-fencing/owner/GC tests passed, with the 20-process conformance case explicitly skipped. Structured-mutation 15 and restore 25 tests also individually passed in a preceding invocation, but that invocation exited 1 without diagnostics for the fencing file; an earlier default-worker invocation did likewise. Thus 105 individually passing tests, not a clean whole-suite run. The cause of those nondiagnostic exits is not established, and local 20-process conformance remains unverified. The independent public-entrypoint reproduction and history assertions passed separately.

No new defect found in this bounded review. This confirms the source fix for the reproduced lost-update scenario, not npm publication, installed upgrade, whole-release certification, or all extension-provider concurrency. The built-in fencing versus extension-owned lifecycle distinction is now explicit in the SDK docs. Keep the same reproduction plus history assertions as regression protection when the official artifact is published.

Independent bounded reproduction of the existing short-budget finding #1550 (comment) at immutable 6bb2cfd (source-built dist, Node 24.19.0): withTelemetryQueueMutation(async () => {}, isolatedRoot, 250) waits 1169 ms and succeeds while a synthetic lock publisher atomically replaces telemetry-queue generations every 100 ms for ten handoffs, then removes the final lock. The operation is a no-op, with telemetry disabled and no provider/network call. An acquireLock control using the same 250 ms wait plus {maxWaitMs:250} refuses lock_conflict after 264 ms under the same handoff cadence.

Expected: the explicitly short foreground budget remains a hard total ceiling; actual: the changed no-progress semantics extend it with each observed handoff. The 250 ms foreground constant and queue wrapper are at src/core/telemetry/runtime.ts:81 and 2088–2106; the wrapper supplies no maxWaitMs. Static inspection also confirms missing ceilings in acquireItemMetadataDerivedIndexLock (3000 ms) and appendEvents usage ledger (3000 ms). Those two complete consumer paths were not runtime-tested here. The 120 s possible maximum follows from lock.ts's default total ceiling, not from a 120 s measured run. This single synthetic sample is not a production latency distribution.

Acceptance/regression: preserve progress-aware defaults for callers that want them, pass explicit hard caps for latency-budgeted callers, and add deterministic handoff tests for each caller. Keep the original stale-writer/history regression passing. This does not invalidate the earlier verified #1549 lost-update fix, but is a separate remaining behavior on the same source head. No new issue because the inline owner already covers it; no npm publication or whole-release certification claim.

…ocks keep their budget

waitMs had become a no-progress window for every caller, so owner handoffs
could stretch foreground telemetry (250 ms), the context usage ledger and the
derived-index lock (3 s) to the 120 s ceiling, the last while an item commit
holds its fence. acquireLock/acquireLockLease now take waitPolicy:
"total" (default, unchanged fixed budget) or "progress". The item-store
lease, restore and the hierarchy-integrity lock opt in, which covers the
GH-1516 contention paths. PM_LOCK_WAIT_MS still raises the short internal
budgets, which per-caller maxWaitMs caps would have prevented. Elapsed-time
upper bounds in the wait tests are widened to 10 s for loaded runners, and a
test pins the default total budget.

Follow-up at new immutable head b1f1d3b: the short-budget regression above is addressed by making progress-aware waiting opt-in (waitPolicy: 'progress'), while the default remains a fixed total budget. This also retains the documented PM_LOCK_WAIT_MS override rather than clamping it away.

Independent bounded check: executed exact new lock.ts after in-memory TypeScript stripping against the unchanged prior source-built dependencies (Node24.19.0, isolated synthetic locks, telemetry disabled, no provider calls). With the same 250ms budget and100ms owner-handoff cadence, default policy refused lock_conflict after260ms; explicit progress policy succeeded after1201ms. The previous actual telemetry-wrapper negative control on6bb2cfd still records1169ms success instead of the short refusal. Static diff confirms progress opt-in is limited here to item-store, restore and hierarchy mutation paths, while the three short-budget callers keep default semantics.

Limits: this verifies the changed lock module plus caller wiring, not a fully rebuilt new artifact, all consumer wrappers, the20-process conformance case, or completion of new-head CI (still running when observed). Original lease fencing code is unchanged in this delta. Preserve deterministic default-versus-progress handoff tests and existing stale-writer/history checks. Source fix only; no npm publication claim.

item_store_write and non-dry-run item_store_delete are extension code that may
persist the payload themselves, but they ran before the fence, so a commit
refused with lock_lost or stale_revision could already have been written by an
override despite "Nothing was written". Both now run inside the fenced commit,
after lease and revision verification; the dry-run delete preview still
consults its override before returning. Delete override resolution moves into a
helper. A test proves neither override is reached when the fence refuses.

Docs: EXTENSION_AUTHOR_CONTRACTS.md records the fence ordering, and
CONFIGURATION.md names the hierarchy-integrity lock scope (parented creates,
parent changes, integrity-sensitive dependency additions).

When the commit fence cannot enter the stale-cleanup gate, ownership was never
checked and the lease may still hold the lock, so the takeover wording ("no
longer owned", "newer value was kept", raise the TTL) was wrong. That reason
now says the lease could not be verified and advises a retry plus
pm gc --scope locks --dry-run. SDK.md now requires extension lock providers to
exclude or fence concurrent commits, or compare-and-write atomically at the
storage boundary, instead of offering a pre-write revision check as an
alternative. pm-iop1pg's acceptance criteria, close reason and resolution now
name the opt-in progress wait policy and its callers.

Comment thread src/core/lock/lock.ts
Restore stopped its heartbeat, copied the live workspace lock into the staging
root and renamed that root over the tracker without the lock's cleanup gate,
so a writer that took over an expired lease in that window lost its lock to
republished stale bytes. The workspace lock is now a lease and the lock copy
plus root swap run inside lease.fence, refusing with lock_lost when ownership
was lost. The heartbeat's own check-then-write renewal now runs under the same
gate (bounded wait via the newly exported waitForStaleCleanupGate), so a
renewal can never overwrite a lock another writer took. Tests cover a takeover
before activation (lock_lost, workspace not swapped) and a renewal that cannot
enter a held gate.

Post-merge reconciliation: merge commit abdbc50 has exactly the same tree (4c83febb60028e2189000555c6c1a3b688d68fb6) as tested head bbb9f09. The 11-assertion synthetic workspace root-swap exclusion failure in #1550 (comment) therefore remains applicable to main; it was not fixed by the merge or the review thread's resolved flag.

This is a residual workspace-activation boundary, not a claim that the original item-mutation fix failed, and not evidence of actual user data loss. The canonical pm-gh1549 record and #1549 are closed; retaining this follow-through here avoids silently treating closure as regression proof. Acceptance remains safe serialization of takeover attempted after staging becomes live but before activation completes, plus ordinary restore/rollback controls. No duplicate test rerun or npm/publication claim.

SDK: a mutation that outlives its lock TTL silently overwrites a newer committed write (no fencing)
